Six cases of bacteremia due to serotypes of Salmonella enteritidis are described in patients with the acquired immunodeficiency syndrome (AIDS). In four instances the bacteremia was recurrent despite appropriate antimicrobial treatment. Neither a gastrointestinal tract source nor any other focus of infection could be identified in four of the six patients. In one patient an unusual Salmonella infection, ie,
pyelonephritis
, was noted. The discovery of Salmonella sepsis led in four cases to the initial diagnostic consideration of AIDS, which was ultimately confirmed. When unexplained
Salmonella bacteremia
occurs in populations known to be at high risk for the development of AIDS, a thorough evaluation for this disorder should be undertaken.

Biogenesis of tetrahydrofolate cofactors essential for bacterial growth and survival is blocked by sulfamethoxazole-trimethoprim. An intravenous form of the antimicrobial combination has recently been approved for the treatment of acute, symptomatic, bacterial
pyelonephritis
, recurrent urinary tract infections, shigellosis, and Pneumocystis carinii pneumonia. Intravenous sulfamethoxazole-trimethoprim has emerged as an invaluable agent for the management of selected infections, including bacterial meningitis and
Salmonella bacteremia
, where limited therapeutic alternatives exist. In addition, co-administration of intravenous sulfamethoxazole-trimethoprim with a carboxypenicillin provides an empiric treatment for the infected granulocytopenic patient that compares favorably with standard combinations. Adverse events unique to the intravenous form of the drug consist of phlebitis and fluid imbalances. Fluid overload results from the relatively large volumes of 5% dextrose solution required as diluent.

This report describes a 56-year-old man with a ruptured infected abdominal aortic aneurysm secondary to
Salmonella bacteremia
, initially presenting as acute
pyelonephritis
. Spike fever with severe back pain continued despite empiric antibiotic treatment at a local hospital. Hypotension with a sudden hemoglobin drop was observed on the second hospitalization day. Abdominal computed tomography to further examine the bleeding focus confirmed a rupture of the mycotic abdominal aortic aneurysm. This case was successfully treated through prompt surgical intervention and aggressive protracted antibiotic therapy. The case presented herein raises concerns about the uncommon but life-threatening mycotic aneurysm presented initially as acute
pyelonephritis
. Early diagnosis and appropriate surgical and antibiotic treatment of the Salmonella mycotic aortic aneurysm is crucial for a satisfactory outcome.
