Hammock nonrefluxing ureteroileal anastomosis was performed on 14 patients who had urinary tract reconstruction using ileal conduit (4), Kock pouch (3), modified Kock pouch with plicated efferent limb (1) and ileal neobladder (6). Radiographic examinations showed ureteral reflux of contrast medium in one patient (7.1%), ureteral stenosis in one patient (7.1%) and no urine leakage. Three patients had pyelonephritis (21.4%) and no one had any upper tract urolithiasis. This technique provides a simple and reliable antireflux mechanism into ileal segments without nonabsorbable material.

It is a description of a new technique of putting a removable purse-string hemostatic stitch on the prostatic adenoma bed. Four stitches are made with a double catgut thread. The loop of the thread encircles the ureteral catheter introduced into the urethral tube. Upon tightening the purse, loose thread ends are connected with the urethral tube and fastened, the tube is tracted. Postoperative drop irrigation of the bladder with antiseptic drugs proceeds through the ureteral catheter. The purse-string suture is taken out when the catheter is removed from the urethral drainage at postoperative hour 20-24. The bladder is sutured tightly using double-row buried purse-string gut stitches. A total of 143 patients underwent surgery according to the technique of whom 70% had aggravating chronic cysto-pyelonephritis, 17.5% suffered from urolithiasis, 72% from cardiac diseases and 30% of respiratory disorders. 28% of the patients combined prostatic adenoma with chronic prostatis. 38 patients (26.5%) needed blood transfusions. Mean stay in hospital after one-stage adenomectomy made up 16.4 days at stage I-II of the disease, after delayed adenomectomy 19 days, after two-stage adenomectomy 21.8 days. The analysis is presented of 64 surgical cases of affected dystopic kidneys. Of these, 34 had urolithiasis, 6 renal tumors, 10 hydronephrosis, 14 chronic pyelonephritis. The principal diagnostic facility was radionuclide renal scintigraphy. On deciding on the operative treatment, additional use of angiography, cystography and other methods is advisable. Diagnostic findings and treatment variants are described.

Twelve cases of xanthogranulomatous pyelonephritis (XGPN) complicated by urolithiasis in upper tract were presented in this study. Fifty-four cases of XGPN, including 12 cases of this study, reported in China were clinically analysed in the article. All cases were with one suffered kidney, 57.4% of the cases had both XGPN and urolithiasis, 27.8% had the history of urinary infections, 99.0% had function-impaired kidney or renal failure. XGPN was frequently mis-diagnosed as renal cancer, renal tuberculosis and renal abscess etc. The diagnosis and treatment of XGPN are discussed.

The role of thrombin, fibrinogen, specific and unspecific immunological reactions in the pathogenesis of urolithiasis were evaluated. It was found in the urine of patients with urolithiasis and concomitant pyelonephritis that most bacteria, leucocytes and organic substances fixed on crystalloids contained on their surfaces immunoglobulins M, G and A. The mechanisms of the possible participation of immune reactions in the pathogenesis of urolithiasis are discussed.

The Soviet-made device "URAT-P" for remote lithotripsy was used for the treatment of urolithiasis in children aged from 22 months to 14 years who suffered from various clinical patterns of urolithiasis with solitary or dendritic calculi. Out of 64 treated persons, 21 children had undergone 1-2 surgeries for the disease. The majority of them had pains and frequent aggravations of pyelonephritis. Excretory urography revealed the signs of hydronephrosis and hydrocalycosis. There was no need for additional adjustments to perform the procedure in patients with a height of under 75 cm. The authors employed erosive operating conditions of a shock-wave generator for lithotripsy in case of large or dendritic calculi and to avoid preliminary renal drainage. One treatment session was a success in 43 patients, 2 sessions were needed for 15, 3 for 4 and 4 for 2 patients. A beneficial effect (total disintegration and fragmental discharge) was reached in 64 (98.4%) of the patients. Partial disintegration was recorded only in 1 patient who was exposed to 2 sessions. Radioisotope, immunological, ultrasonic, and biochemical investigations failed to reveal inhibition of the renal functional parameters after remote lithotripsy. No renal injuries associated with hematogenesis were revealed in the parents. The signs of hematuria were observed in the first portion of the postoperative urine only. The only serious complication calling for close attention is acute pyelonephritis observed in 6 patients, one case requiring a surgical application of nephrostoma.

One case of xanthogranulomatous pyelonephritis diagnosed following nephrectomy is described, considering its outstanding characteristics such as male adolescent, right kidney, involvement with normal function and without pathological history for which there is no explanation to date. This disease usually occurs in women 60 years or older, suggesting a renal tumor. In 50% of the cases, urine cultures are positive for Escherichia coli or Proteus mirabilis; in accordance with different reports renal tissue cultures are positive in more than 90%. Malnutrition, calcification, urolithiasis and renal failure with hydronephrosis are common findings. The left kidney is more frequently involved. The etiology is unclear: many hypotheses are discussed, with malnutrition and peroxidase deficit as important causes.

A cystadenoma or a cystadenocarcinoma arising from the renal pelvis is a rare neoplasma and, so far as is known, only two cases have been reported in the literature (Arcadi, 1956, and Ross, 1985). Discussed herein is a case of a 63-year-old woman who developed a mucinous nephrosis, due to a marked retention of mucin produced by a tumor. The ureter as well was found to be involved. The histogenesis of an adenocarcinoma, which includes a cystadenoma and a cystadenocarcinoma, most likely is due to a glandular metaplasia, associated with urolithiasis and pyelonephritis. In this specific case, the patient revealed a history, fifteen years earlier, of renal calculi and pyelonephritis.

In 19 urological patients with pyointoxication and urosepsis 49 plasmaphereses for the purpose of detoxication were performed in the complex with other therapeutic measures. The causes for the pyoseptic complications were as follows: urolithiasis in connection with acute pyelonephritis, acute pyelonephritis (among others in pregnancy, cystic renal dysplasia, carcinoma of the urinary bladder), renal insufficiency in the terminal stage. The treatment of these diseases with haemodialysis and haemoperfusion was complicated by a pyosepsis. Two methods of the plasmapheresis were used: the intermitting plasmapheresis with use of a refrigeration centrifuge K-70 (GDR) and the permanent membrane plasma separation with the device A2008 RG of the firm "Fresenius" (FRG). The plasma perfusion was experimentally proved and in 5 cases used on 5 columns with activated charcoal. The efficacy of the plasmapheresis and the plasma perfusion was apart from the clinical condition judged according to the values of the middle molecules in the blood, or urea, creatinine and the normalisation of the hypoproteinaemia as well as of the humoral immunity. To this are added the increase of diuresis, the normalisation of the haematological parameters and the bacteriological findings of blood and urine. Furthermore, several pathogenetic mechanisms of the positive effect of the plasmapheresis were analysed (mechanic removal of bacteria and their toxins, effect of "deplasmation" with tissue dehydration, improvement of the functional state of the kidneys within the first 3-4 days: reduction of the azotaemic intoxication, the DWS-syndrome, improvement of the rheological properties of the blood and of the microcirculation, increase of the antitoxic function of the liver).(ABSTRACT TRUNCATED AT 250 WORDS)

A nine year retrospective study of hematuria in 14 New Zealand White rabbits was conducted to classify possible etiologies of this clinical finding. Physical examination, laboratory tests, radiography and postmortem examination were utilized in most cases to verify the presence of hematuria and to determine its etiology. Uterine adenocarcinoma was diagnosed in two rabbits. Three rabbits had uterine polyps with hemorrhage. Renal infarction with hemorrhage was diagnosed in three rabbits. Urolithiasis with secondary urethral obstruction and hemorrhagic cystitis was identified as the cause of hematuria in four rabbits. Other causes of hematuria included chronic cystitis, disseminated intravascular coagulation, bladder polyps and pyelonephritis. Hematuria of undetermined origin was observed in one rabbit. This last [corrected] case was negative for both blood and porphyrin in the urine, but positive for excess levels of urobilin, the oxidative product of urobilinogen. This case illustrates that hyperpigmented urine should be a rule out in all cases of suspected hematuria in rabbits.
