One hundred adult Sudanese patients who presented to Soba University Hospital (SUH) with established chronic renal failure (CRF) were studied to determine the aetiology. Thirty-eight had chronic glomerulonephritis, 12 renal calculi, nine diabetic renal disease, seven chronic pyelonephritis, five sequelae of acute renal failure (ARF), four renal vascular disease, three polycystic disease of the kidneys, and two obstructive uropathy. In 20 patients the aetiology was not determined because of late presentation to hospital. The results were compared with those of the developed countries, which differ greatly from Sudan in climate, diet, race, culture and social habits. The main differences were in the prevalence of renal calculi which, although being the second commonest cause of CRF in the Sudan, were rare in European countries. Also, diabetes mellitus was a much commoner cause of CRF in Sudan than Europe. Other aetiological factors were similar.

Renal functional and structural studies were performed in 46 patients with arterial hypertension: out of them 12 had hypertensive disease, 13, chronic pyelonephritis, 21, a hypertensive type of chronic glomerulonephritis. In each case, the clinical diagnosis was evidenced by one of the invasive techniques. Dynamic computed tomography was conducted by the original methods; the findings were analyzed by taking into account time-density curves which made it possible to gain an insight into the status of blood flow and filtration in each individual kidney. Computed tomography and dynamic computed tomography revealed that hypertensive disease was characterized-by normal volume and thickness of the renal cortical layer and symmetric time-density curves, whereas a hypertensive type of chronic glomerulonephritis featured lower renal cortical layer thickness, reduced renal volume, symmetrically decrease amplitudes of the first and second peaks of the time-density curve, chronic pyelonephritis showed asymmetric time-density diagrams due to the lower density areas in the afflicted kidney.

We report the results of clinicopathological and histochemical studies on 64 renal adenomas found in 22 patients. We selected typical renal adenomas for these studies, discarding borderline lesions. The ages of the patients ranged from 42 to 84 years, with an average of 61 years. Male to female sex ratio was 6.3:1. The background conditions included renal cell carcinoma, long-term dialysis, chronic glomerulonephritis, pyelonephritis, hydronephrosis and polycystic kidney. The sizes of the tumors ranged from 0.1 to 2.3 mm in maximum diameter, with an average of 0.9 mm. All the tumors were located in the cortex, especially in the superficial one-third. The cytoplasm of the tumor cells was predominantly eosinophilic, and the tumor structure was predominantly papillary. Histochemical study of 19 adenomas from 13 patients demonstrated that all of the adenomas from 13 patients demonstrated that all of the adenomas were positive for EMA, and for at least one marker of the distal tubulus, i.e., DBA, PNA, SBA and PKK1, and that 9 adenomas were positive for at least one marker of the proximal tubulus, i.e., LM1 and LTA. Renal adenomas had predominant histochemical features of the distal tubulus, suggesting differentiation to a distal tubulus-like histology.

Two new laboratory diagnostic tests for chronic glomerulonephritis and pyelonephritis have been developed. The first one is based on electrophoretic mobility of urinary lysozyme under certain conditions, such as the use of 12% polyacrylamide gel with pH of 4.3 and acid electrode buffer with pH of 4.0. After electrophoresis was discontinued, lysozyme position was determined by lysis of Micrococcus lysodeikticus, used as test agents and added to the gel as a suspension prior to polymerization. Urinary lysozyme was found to be in the anode area of the gel in 95% of patients with chronic pyelonephritis, and in its cathode area in 92% of patients with chronic glomerulonephritis. There was no lysozyme in the urine of normal subjects. The other laboratory technique, the ethanol test, is based on comparative assessment of the degree of urinary opacification after ethanol is added in conditions of neutral reaction (following the addition of physiologic saline) and marked alkaline reaction (following the addition of sodium hydroxide solution). The ratio of optic density of the alkaline specimen to that of the neutral specimen was above 1 in patients with chronic glomerulonephritis, and below 1 in patients with chronic pyelonephritis and normal subjects. Examination of biochemical mechanisms of the proposed tests has demonstrated that the pattern of proteinuria is the most important factor affecting the results.

Correlation between cortico-medullary contrast (CMC) and dynamic CT or MRI for chronic renal parenchymal diseases was evaluated compared with serum creatinine level (Cr). Forty two kidneys of 23 patients were chronic glomerulonephritis (12), chronic pyelonephritis (12), chronic renal failure (4), and other diseases (14). Thirty two kidneys with low Cr (under 1.6) were demonstrated the good CMC by dynamic CT and T1-weighted image (IR) of 0.5 T. Six with middle of Cr (between 1.6 and 3.0) were showed a decrease in CMC and four with high Cr (over 3.0) had no CMC. CMC correlated with Cr.

A clinico-laboratory study of 60 patients with chronic alcoholism (stage II) revealed changes of the kidneys and urinary system in a half of all patients. Radionuclide examination revealed renal function disorders in 78.3% of patients indicating its value in the early diagnosis of alcohol nephropathy. Autopsy data in 246 cases revealed morphological changes in the kidneys in 2/3 of them (dystrophy, necrotic nephrosis, chronic glomerulonephritis, pyelonephritis).

Serum gamma-glutamyltranspeptidase (GGTP) and alpha-amylase clearance were determined in a total group of 90 patients of whom 60 with renal diseases and 30 with extrarenal diseases. The renal patients were distributed, according to diagnosis in the following groups: acute glomerulonephritis, chronic glomerulonephritis, acute pyelonephritis, chronic pyelonephritis, nephrotic syndrome and manifest chronic renal failure. The 30 controls were hospitalized for different extrarenal diseases such as: pneumonia, gastroduodenal ulcer, arterial hypertension stage I and angina pectoris. Serum GGTP assay was performed in 60 patients (40 renal patients and 20 controls) using Boehringer monotest kits and in 30 patients (20 renal patients and 10 controls) using Romanian kits (I.C.C.F.). No changes suggesting a particular type of nephropathy were observed. The results obtained by using the two types of kits for the serum GGTP assay have proved to be very close. Alpha-amylase clearance was determined in all the patients with Spofa (R.S.C.) tablets concomitantly with the urea and creatinine clearance. Important decreases of alpha-amylase clearance in concordance with decreases of urea and creatinine clearances were observed in all the patients with severe renal failure. More moderate decreases of alpha-amylase clearance were observed in the patients with acute and chronic glomerulonephritis. The utility of this clearance as a test of glomerular filtration and sometimes as a prognostic test, is discussed.

High diagnostic significance of beta 2-microglobulin determination by radioimmunoassay in the blood and urine of patients with the main types of renal pathology (chronic glomerulonephritis, chronic pyelonephritis, hemorrhagic fever with the renal syndrome, and diabetic nephropathy) was shown. A study of beta 2-microglobulin level permitted accurate determination of the localization of pathology in the nephron and assessment of a degree of its expression as well as the detection of initial renal functional disorders, assessment of therapeutic efficacy and dynamic observation of renal function in patients with renal pathology.

A study of the clinical presentation and conceivable causes of chronic renal failure (CRF) in 61 Sudanese patients in Khartoum is presented. The clinical features involved almost all the systems, however, gastrointestinal and cardiovascular signs and symptoms predominated. The causes of chronic renal failure in Sudan and Sweden are also studied for comparison. The causes of CRF in Sudan are chronic glomerulonephritis, obstructive nephropathy (stone disease), hypertension and diabetes mellitus in that order. The main causes of CRF in Sweden are chronic glomerulonephritis, diabetes mellitus and chronic pyelonephritis. Of the 61 Sudanese patients 16 have kidney transplants, only one in Sudan, three patients are on regular hemodialysis, nine patients are on intermittent peritoneal dialysis, 16 are on conservative treatment and 17 died during the course of treatment.

Since 1981, the Korean Society of Nephrology began annual report on renal replacement therapy in Korea. The annual number of new patients receiving dialysis treatment in 1986 increased to 957 patients (23.3 per million population) from 825 patients (20.4 per million population) in 1985. And the total number of patients on replacement therapy increased from 1,508 patients (37.3 per million population) to 2,534 patients (61.7 per million population). 1,340 patients (32.6 per million population) of these patients were on hemodialysis, 573 patients (13.9 per million population) on continuous ambulatory peritoneal dialysis (CAPD) and 621 patients (15.1 per million population) on functioning renal graft as of December 31, 1986. The common causes of renal failure of new patients were chronic glomerulonephritis (41.6%) followed by diabetic nephropathy (12.6%), nypertensive nephrosclerosis (7.8%), chronic pyelonephritis (2.5%) and others. The annual mortality rate decreased from 21.9% in 1981 to 13.5 in 1986. The common causes of death in patients on dialysis therapy were cardiac (32.8%), vascular (14.7%), infective (14.7%) and social problems (11.2%) in the order of frequency. Recently, the number of patients requiring dialysis is rapidly increasing due to expanded medical insurance support for dialysis and improved economic status of our country. Therefore, it is necessary to draw up counterplan for a rapid growth of the number of new patients.
