Renal allografts were transplanted into 20 dogs (12 beagles, eight mongrels) following a prescribed protocol for pre-transplantation blood transfusions and kidney exchange. Immunosuppressive therapy (azathioprine and prednisone) was modified as needed for each dog. Seven of the beagle dogs survived for 1 year and were then euthanized; all other dogs died or were euthanized prior to 1 year post-transplantation. Graft rejection and

An enhanced frequency and morbidity of urinary tract infections (UTI) have been observed in association with alcoholism and liver disease. The causes of these phenomena may relate, in part, to the defects in humoral and cellular immune mechanisms that occur in alcoholism. Urinary catheterization is the most common cause of UTI in hospitalized alcoholics. The severity of the sequelae of UTI in alcoholism is demonstrated by the unusually frequent occurrence of renal papillary necrosis (RPN) in conjunction with

in these patients. Indeed, in over 90% of the reported cases of RPN occurring with alcoholism or liver disease,

has been a contributing factor. The proclivity to medullary ischemia and RPN in this patient group may be, at least in part, a result of interstitial renal edema secondary both to infection and the effect of ethanol per se and to renal arterial vasoconstriction that occurs in cirrhosis. was done. During six years i.v. urography has been performed in 936 patients, 297 (31.7%) of whom had hypertension, and 123 (13.1%) had radiological signs of chronic

87 (70.7%) subjects (57 men, 30 women) had arterial hypertension. Bilateral chronic

was radiologically confirmed in 61 (70.1%) and unilateral parenchymal renal disease in 26 (29.9%) of the patients with hypertension. When the diagnosis has been postulated on the basis of radiologically evident changes of kidney parenchym (renal scarring) or the combined calyx-parenchymatous lesions, it could be shown that the frequency of hypertension in these patients was statistically higher (p less than 0.001) than in the group of patients that displayed only isolated calyx lesions. We report a case of bilateral metachronous xanthogranulomatous
pyelonephritis
(XGP) in a hemodialyzed patient with end-stage
renal failure
due to bilateral nephrolithiasis. The XGP was initially diagnosed by computed tomographic (CT) scan, and right nephrectomy was performed. The patient remained clinically stable for the next year. Involvement of the contralateral kidney was again confirmed by CT examination. Both kidneys showed histologic changes compatible with XGP. A coexisting nephrogenic adenoma of the bladder has been attributed to chronic urinary tract infection.

(ESRF) Gm and Km typing has confirmed a significant increase in the frequency of the Gm1,2,17;21 haplotype. This was confined to an increase in the number of heterozygous Gm(1,2,3,17;21,5,10,13,14) individuals. A concomitant decrease in the frequency of presumptive Gm(3,5,10,13,14) heterozygotes was also confirmed. Analysis of individual renal diseases revealed significant immunoglobulin allotype, phenotype and haplotype frequency deviations in those patients presenting with hypertension, glomerulonephritis or
pyelonephritis

Our current knowledge of the long-term outcome of uncomplicated urinary tract infections in women is based on several things: a reevaluation of the criteria for defining
pyelonephritis
at autopsy, careful description of the causes of renal disease among patients entering dialysis and transplant programs, long-term observation of patients, and epidemiologic studies attempting to determine the association of bacteriuria with mortality. The weight of the evidence favors the conclusion that, although urinary tract infections can severely impair renal function, this is rare in the absence of a major predisposing factor such as obstruction, calculus, reflux, abnormalities of the voiding mechanism, or diabetes. The predisposing lesions, however, may go undetected until heralded by episodes of acute
pyelonephritis
or by
renal failure
. Unfortunately, urinary tract infections are so common that it is difficult to distinguish the population at greatest risk. The possible role of renal damage produced by autoimmune mechanisms following infection needs continued study.

The plasma levels of tocainide have been followed after oral administration of 600 mg p.o. to 20 patients with
renal failure
due to various causes, and to 8 healthy controls. The peak plasma concentrations in the patients with
pyelonephritis
(3.80 micrograms/ml) and interstitial nephritis (3.74 micrograms/ml) but not in those with glomerulonephritis (3.17 micrograms/ml) differed from that in healthy volunteers (3.24 micrograms/ml). The renal clearance of tocainide was well correlated with the endogenous creatinine clearance and was dependent on urine pH. No difference in renal clearance was observed between the patients groups. It is suggested that the changes in plasma levels are a consequence of decreased renal clearance. Creatinine clearance was shown to be a poor estimator of tocainide clearance, which suggests that extrarenal clearance plays an important role in the handling of the drug in the body. The findings are used to suggest a safe dosage regimen.
