Isoelectric focusing in thin-layer polyacrylamide gel is a method with high resolving power, e.g. for urinary proteins. Therefore more information can be obtained with this method than with ordinary electrophoresis. The present version of the method has a high capacity and allows study of up to 25 samples in parallel on each gel. Only a little more than one hour is required for separation. A sensitive staining procedure is used which allows detection of as little as 0.1 mug of protein in each zone. When effects on the kidney are present more than 20 different protein zones can sometimes be separated. Evaluation is made by densitometry as well as by comparison and classification of patterns. Urine samples have been studied from workers with varying exposure to cadmium lead and various chlorinated hydrocarbons, and patients with glomerulonephritis,
pyelonephritis
,
nephrosis
and mixed tubular and glomerular diseases. Typical tubular patterns, i.e. as developed in association with cadmium exposure were characterized by elevated excretion of beta1-microglobulin. Other types of kidney malfunction showed different features.

Renal tissues were studied using electron microscopy (EM) and immunofluorescence microscopy (IFM) from three patients who were found to have chronic interstitial nephritis (
pyelonephritis
) by light microscopy (LM). By LM, 90% of the glomeruli in two patients and all glomeruli in one patient were normal. By EM, glomerular capillaries in all patients revealed generalized fusion of epithelial foot processes. In two patients, IFM for immunoglobulins, third component of complement and fibrinogen were negative. These two patients received corticosteroids for 6 to 12 weeks. In one, proteinuria markedly decreased (from 17.9 to 1.1 gm) in four weeks and in the other follow-up studies of renal histology revealed normal glomeruli and partial restoration of foot processes by LM and EM respectively. Thus, this study offers evidence for lipoid
nephrosis
(or minimal lesion disease) as an etiology of nephrotic syndrome in chronic interstitial nephritis (
pyelonephritis
). The impaired renal function in these patients is attributed to tubulo-interstitial disease rather than glomerular pathology. It remains to be determined whether the two disparate pathological conditions have coexisted or chronic interstitial nephritis had led to the appearance of lipoid
nephrosis
through an unidentified mechanism.

Enzymuria, urinary cell excretion and proteinuria were simultaneously determined in renal diseases of female Wistar rats in order to investigate the diagnostic value of urinary enzymes. Investigations were carried out on rats with E. coli-
pyelonephritis
facilitated by oestradiolundecylate, aminonucleoside
nephrosis
, aminoglycoside induced renal lesions and pyelonephritic animals treated with therapeutic and toxic doses of tobramycin. --From the results of these studies it was concluded that the main diagnostic value of urinary enzymes is detection of drug induced tubular lesions in individuals with preexisting renal diseases.

Infection of the urinary tract in childhood is common. It may start already in neonates or infants. Clinical signs are not always present but asymptomatic socalled bacteriuria may occur at any age. The earlier a child contracts the infection the more common are uncharacteristic clinical signs like loss of appetite, a raised temperature, parenchymatous icterus and, finally signs of involvement of the central nervous system. During the last few years purposeful research has established the important fact that, if there has been a urinary tract infection particularly in the first months of life, recurrence may more often be asymptomatic than on the first occasion. A mid-stream specimen is diagnostically valuable. If this does contain any bacteria, then supapubic aspiration from the bladder is useful in enabling us to start a specific, if possible bactericidal, therapy. Asymptomatic bacteriuria may at any time lead to clinical signs of
pyelonephritis
, particularly during periods of stress. Such silent unrecognized cases of asymptomatic bacteriuria may, we feel, lead to pyelonephritic
nephrosis
in a so far unknown percentage of cases.

Fibroepithelial polyp of the trigone of bladder conditioning an occlusive syndrome at the bladder cervix level causing secondary multidiverticular bladder with ureterohydro-
nephrosis
and chronic bilateral
pyelonephritis
. Besides the rare vesical location for the fibroepithelial polyps, the case also presents some atypical stromal cells.

Levamisole (2.1-3.1 mg/kg twice a week) was administered to 6 children aged between 20 months and 6.5 years for 1 to 4 months. All children suffered form the renal diseases exacerbation (nephrotic syndrome and
pyelonephritis
--2 children, lipoid
nephrosis
and
pyelonephritis
--2 children,
pyelonephritis
with glomerular reactions--1 child, recurrent
pyelonephritis
--1 child) due to recurrent respiratory infections levamisole improved both clinical and biochemical parameters of renal disease. Subsequent respiratory infections were milder and less frequent. Transient decrease in thrombocyte count was seen in 4 children after 2-8 weeks of therapy with levamisole. An increase in AspAT and (or) AlAT was noted in 2 children after 1-2 months of therapy. Levamisole was withdrawn in 2 children after 30 days due to an increase in AspAT activity, prolongation of blood clotting time and thrombocytopenia.

A cystadenoma or a cystadenocarcinoma arising from the renal pelvis is a rare neoplasma and, so far as is known, only two cases have been reported in the literature (Arcadi, 1956, and Ross, 1985). Discussed herein is a case of a 63-year-old woman who developed a mucinous
nephrosis
, due to a marked retention of mucin produced by a tumor. The ureter as well was found to be involved. The histogenesis of an adenocarcinoma, which includes a cystadenoma and a cystadenocarcinoma, most likely is due to a glandular metaplasia, associated with urolithiasis and
pyelonephritis
. In this specific case, the patient revealed a history, fifteen years earlier, of renal calculi and
pyelonephritis
.

From 1980 to 1988 235 koalas were necropsied and 67 were found to have urinary tract disease. Six affected koalas out of 48 were derived from wildlife parks around Sydney while 61 of 187 were derived from free living populations on the central and north coasts of New South Wales. Sixteen had cystitis alone, 5 had cystitis and associated renal disease only, 16 females had cystitis with genital disease, 23 had urinary disease in combination with other systemic disease and 7 had renal disease only. Overall 49 animals had cystitis (30 females and 19 males; 47 being free living) with 12 of these having renal extension (all free living). Cystitis tended to be active but chronic while associated renal disease was mainly designated as hydronephrosis and
pyelonephritis
. Other forms of renal disease included lymphosarcoma, oxalate
nephrosis
, acute and chronic nephritis, and microabscessation related to septicaemia. Female genital disease associated with cystitis was commonly vaginitis and metritis. Paraovarian cysts were detected with and without metritis. Other diseases occurring with urinary tract disease included conjunctivitis, dermatitis/stomatitis, pneumonia and hepatic disease. The higher prevalence of urinary tract disease in free living koalas, especially cystitis, is in contrast to captive koalas and may reflect the interaction between disease cause and habitat.

A clinico-laboratory study of 60 patients with chronic alcoholism (stage II) revealed changes of the kidneys and urinary system in a half of all patients. Radionuclide examination revealed renal function disorders in 78.3% of patients indicating its value in the early diagnosis of alcohol nephropathy. Autopsy data in 246 cases revealed morphological changes in the kidneys in 2/3 of them (dystrophy, necrotic
nephrosis
, chronic glomerulonephritis,
pyelonephritis
).

Renal allografts were transplanted into 20 dogs (12 beagles, eight mongrels) following a prescribed protocol for pre-transplantation blood transfusions and kidney exchange. Immunosuppressive therapy (azathioprine and prednisone) was modified as needed for each dog. Seven of the beagle dogs survived for 1 year and were then euthanized; all other dogs died or were euthanized prior to 1 year post-transplantation. Graft rejection and renal failure were the greatest causes of mortality. Renal lesions which contributed to the death of some animals included renal vein thrombosis,
nephrosis
, and
pyelonephritis
. Inflammation of the lower respiratory tract (bronchitis, pneumonia, and pleuritis) was a contributory cause of death in some dogs. Cystitis and ureteritis occurred in almost half of the dogs. Prostatitis was seen in six of the 16 male dogs. Adrenal cortical atrophy, parathyroid gland hyperplasia, and bone marrow hypocellularity were seen in a majority of the dogs which survived 1 year.
