A case with
multiple endocrine neoplasia
was reported, including parathyroid adenoma, of the main cells of the gland, multiple small adenomas in the tail of pancreas, (cytologically and electron-microscopically determined as A cellular) and light-cellular adenoma of adrenal. The hyperfunction of parathyroid adenoma was manifested with hypercalcemia (3.75 mmol/l), and morphologically--with the multiple calcium metastases in lungs, kidneys and heart, established at necropsy. The cause for the death was the acutely advanced ischemic disease of myocardium, on the background of chronic
pyelonephritis
and renal insufficiency. The timely diagnosis of such morbid states is concluded to be important for the clinical practice and could lead to the saving of the patients by operative removal of the tumour.

A patient with
pyelonephritis
developed multiorgan failure resulting in death. Clinical findings were consistent with
multiple endocrine neoplasia
type II, with bilateral pheochromocytomas identified by computed tomography scan. We hypothesize that either the infection or the administration of radiocontrast media led to a massive release of catecholamines from the pheochromocytomas. As a result, tissue perfusion was severely compromised, and multiorgan failure developed. This exceedingly rare complication of pheochromocytoma has been termed pheochromocytoma multisystem crisis.

A 48-year-old man was admitted under the care of urologists with acute renal failure and septicaemia secondary to
pyelonephritis
. Upon investigation, he was found to have renal stone disease secondary to a parathyroid adenoma. Further tests revealed high pituitary hormone and gastrin values, confirming the diagnosis of
multiple endocrine neoplasia
type 1 (MEN 1) and Zollinger-Ellison syndrome. Soon after this he experienced a series of renal complications due to his renal stone disease and multiple complications of his gastrinoma, including two gastrointestinal perforations and three episodes of significant upper gastrointestinal bleeds (two of which required laparotomies), and a full length oesophageal stricture-all within the span of 9 months. His complications were managed appropriately and the oesophageal stricture was treated with a full length metallic stent. He was discharged home in a reasonably good condition with normal swallowing, but unfortunately died of aspiration pneumonia 3 weeks later.
