The reaction of leucocyte migration inhibition (RLMI), using antigens of autochthonous and allogenic tumors, was utilized to examine 21 patients with renal cancer and in 37 control patients. The antigens of renal cancer (AG RC) would suppress specifically leucocyte migration in all patients with cancer of the kidney, as compared with the migration without antigens or in the presence of normal tissue antigens of the tumor involved kidney. In 5 of 37 cases AG RC as well as those of normal renal tissue inhibited leucocyte migration in patients with urolithiasis and pyelonephritis. Autologous blood plasma in patients with renal cancer would contribute to inhibition of leucocyte migration by cancer antigens. There are some common specific antigens in renal cancer, recognized by lymphocytes from different patients with the tumor in question. RLMI may be used to establish the immune diagnosis of cancer of the kidney.

Pyelonephritis xanthogranulomatosa is a peculiar manifestation of destructive interstitial nephritis which can be differentiated from other types of pyonephrosis exclusively by microscopic examination. Clinical symptomatology often points towards the existence of a renal tumor. The author analyses the clinical and radiological symptomatology of seven patients treated by him. The angiographic picture presented by pyelonephritis xanthogranulomatosa differs according to the character and extension of the inflammatory tissue lesion. The angiographic picture of the circumscribed and diffuse granular proliferative types resembles that of a poorly vascularised malignoma, whereas the destructive type resembles hydropyonephrosis.

A defect in the renal margin secondary to extrahilar entry of renal vessels was detected at urography in three patients. Extrahilar entry of renal vessels is not uncommon and is well known to anastomists and surgeons. It has not been previously reported as a radiographic finding. In the past these marginal defects may have been assumed to be persistent fetal lobulation or scarring from previous renal infarction or pyelonephritis. Arteriography may be necessary to distinguish between these possibilities and occasionally to exclude a renal tumor or cyst.

Mucus-secreting adenocarcinoma of the renal pelvis is an extremely rare tumor. It arises from multipotential transitional epithelium which is capable of undergoing metaplastic transformation when subjected to chronic irritation from calculi, hydronephrosis, or pyelonephritis. Women are affected slightly more often than men. Tumor invasion implies a poor prognosis. Long-term survival occurs but is infrequent. In the case reported, preoperative angiography yielded useful diagnostic information.

Gallium-67 imaging is useful for detecting sites of intra-abdominal inflammation, including subphrenic abscesses, postoperative infection, pyelonephritis or perinephric abscesses, and peritonitis. Positive images may be obtained within several hours of injection, but are more reliable at 24 hr. The results may be used to guide ultrasound or computed tomography studies, which give a detailed anatomic picture of the abscess. Gallium-67 imaging has also been used to detect abdominal neoplasms, but results are less impressive. Abdominal lymphoma is detected in just over 50% of sites. Gastrointestinal, renal, and gynecologic neoplasms are detected even less frequently. However, 67Ga is useful for detecting recurrence of 67Ga-avid neoplasms, and has been reliable in detecting hepatomas and nodal spread of seminoma. When used selectively in patients with neoplastic disease and suspected abscesses, 67Ga is an effective method for diagnosing abdominal disease.

The antibody-coated bacteria (ACB) immunofluorescence test has emerged as the preferred noninvasive technique to distinguish reliably between pyelonephritis and cystitis. Investigators have recently correlated a positive test with chronic bacterial prostatitis and cystitis complicating a bladder tumor or stone. We present data that appear to prove that acute bacterial epididymitis associated with bacteriuria can also evoke a positive ACB determination.

A 47-year-old woman with chronic pyelonephritis since 1950 was prepared for hemodialysis or kidney transplantation. Investigation disclosed secondary hyperparathyroidism and an osteolytic lesion in C VII and Th I causing a subluxation. There were minor neurological symptoms and apparent risk of luxation of the cervical spine with resulting injury to the spinal cord. Operation was performed with total parathyroidectomy and excision of the bone tumor, followed by fixation of the cervical spine with bone graft. There were no major complications during the postoperative period and the patient was mobilized four months after the operation.

We report the tenth case of xanthogranulomatous pyelonephritis, which is a rare cause of supperative proliferative pseudo tumor in children. The disease is more segmental in the child than in the adult and should be considered in the differential diagnosis of renal masses. Early recognition and conservative operation can result in a permanent cure.

One hundred and seventy-five 67Ga-citrate scans were performed to detect suspected occult inflammatory processes. None of the patients had a known malignancy. Renal activity was noted in 12 patients (6.8%) on the 48-hr image. These patients had either pyelonephritis, acute tubular necrosis, vasculitis, or a renal abscess. Since delayed 67Ga uptake in the kidneys may be the first evidence of renal disease, further investigation, including either arteriography or biopsy, is necessary. In patients with a known malignancy, tumor involvement must be considered.

A case of xanthogranulomatous ureteritis is described and the numerous similarities with xanthogranulomatous pyelonephritis in their clinicopathological presentation are discussed. Of the xanthogranulomatous pyelonephritis symptoms our patient manifested a long history of urinary tract infection, weight loss, urinary obstruction, pyuria and gram-negative bacteriuria. The ureteral lesion was a yellowish nodule, granulomatous and was highlighted histologically by foamy histiocytes with periodic acid, Schiff positive cytoplasmic granules. The lesion mimicked an obstructing neoplasm and a specific granuloma.
