The placental passage and the the therapeutic efficacy of flomoxef (FMOX, 6315-S) were studied in patients in the perinatal period. A summary of the obtained results is as follows: 1. Concentrations of FMOX in maternal serum, umbilical cord serum and amniotic fluid obtained upon one-shot intravenous injections to 12 patients were compared with those obtained upon 1 hour drip infusions to 9 patients. It was found that the former means of administration gave higher concentrations that the latter. 2. Concentrations of FMOX in maternal serum, umbilical cord serum and amniotic fluid at 1 to 6 hours after administration through either method were all higher than MIC80's of recognized bacteria. 3. Clinical efficacies were evaluated in 10 patients with puerperal intrauterine infection, 7 patients with endometritis, 2 patients with pyelonephritis and 1 patient each with endo-cervicitis, amniotic fluid infection, mastitis and perineal wound infection. Clinical efficacies were excellent in 5 patients (21.7%), good in 17 patients (73.9%) and poor in 1 patient (4.4%), thus the overall efficacy rate was 95.7%. 4. Eradication of causative bacteria were obtained in all 8 cases tested, hence the eradication rate was 100%. 5. Mild diarrhea in 1 patient was the only side effect observed. No abnormal clinical laboratory test results were found in any patients.

Clinical effects of aztreonam (AZT), a monobactam antibiotic, on perinatal infections were studied with the following results: 1. Efficacy rate of AZT in the clinical application to 12 cases of perinatal infections via 2-4 g/day drip infusion (total dosage 8-24 g) was 91.7% (11/12). Breakdown of the effects according to diseases were "excellent" for mastitis (n = 3), pyelonephritis (n = 1) and urinary tract infection (n = 1), and "good" for a total of 6 cases, i.e., amnionitis (n = 2), amniotic fluid infections (n = 3) and external genital infection (n = 1). 2. Results of bacteriological studies were "eradicated" (n = 1), "replaced" (n = 4), "appeared" (n = 2) and "unknown" (n = 5). MIC values of AZT (10(6) cells/ml) were in a range of 25- greater than 100 micrograms/ml in 1 case of cervicitis (complicated with amnionitis) in the 13th week of pregnancy where AZT was found non-effective. 3. Neither subjective nor objective side effects nor abnormal clinical values were observed during the treatment with AZT.

Fourteen patients suffering from female genital organ infections including mastitis and urinary tract infections were treated with cefoperazone (CPZ). CPZ was administered intravenously or by drip infusion at a dose of 2.0-4.0 g/day for 3-7 days. Clinical results obtained were as follows: Of 8 patients with genital organ infections, excellent responses were seen in 2 patients, good responses in 5 patients. Of 3 patients with mastitis, good responses were seen in all the patients. Of 3 patients with pyelonephritis, excellent and good responses were seen in all the patients. No adverse effects and abnormal laboratory findings were observed. From the above results, we concluded that CPZ was an effective and safe antibiotic against infections in the field of obstetrics and gynecology.

Cefroxadine (CXD), a new cephalosporin, was orally administered to 22 cases in total; 5 with wound infection, 4 with felon, 3 with acute pyelonephritis, 2 with furuncle, 2 with infected atheroma, 2 with phlegmone, 2 with abscess, 1 with acute mastitis, and 1 with lymphadenitis. The daily dose was 500 to 1,000 mg, and maximal total dose and duration was 5 g and 5 days, respectively. Therapeutic results were good in 20 cases (effectiveness rate: 91%), fair in 1 and poor in 1. No side effect was observed in all cases among 22 patients with CXD.

The urogenital organs and mammary glands of sows, culled because of excessive vulval discharge, milking problems, and urogenital infections (swine urogenital disease, SUGD) in their history (n=1070 sows) were examined. The culled sows were assigned to three groups according to parity: parity 1 (n=356); 2-6 (n=354); and >6 (n=360). Necropsy findings associated with these groups were analysed separately. Bacteriological examination of vulval discharges was performed. Escherichia coli and a large number of Gram-positive and Gram-negative organisms were found in all samples of vulval discharge. Except ovarian degeneration and oedematous endometrium, older sows had more (P<0.05) pathological changes in the oviduct, ovaries, and uteri than younger (parity 1) sows. More (P<0.05) parity 1 sows had hyperaemic and congested vaginal walls and haemorrhages into the vaginal lumen than sows of higher parity, which suffered more (P<0.05) from accumulation of purulent material in the vaginal lumen, fibrinopurulent exudate adherent to the wall of the vagina, multifocal vaginal erosions and ulcerations, and purulent, mucopurulent or purulohaemorrhagic exudate in the vagina or on the cervix. Except acute pyelonephritis, mucosal hyperaemia, and congestion of the urinary bladder, more (P<0.05) sows of higher parity had pathological changes in their urinary organs. More (P<0.05) parity 1 sows had acute or chronic purulent exudative mastitis than sows of higher parity, which had more (P<0.05) mammary gland abscessation, mammary gland cysts, and fibrous mastitis. All parity 2-6 and >6 sows had mammary gland and bladder changes, parity >6 sows had changes in the kidney, and renal pelvis, and parity 1 sows had mammary gland changes. Most parity 1 sows had bladder, kidney, and renal pelvis alterations and most parity 2-6 animals had pathological kidney and renal pelvis changes.
