Replacement
lipomatosis
of the kidney is the end result of severe atrophy of renal parenchyma with secondary marked proliferation of renal sinus and perirenal fatty tissue. Although ultrasonography may suggest the diagnosis, CT demonstrated the distinctive features most accurately. We report a case of renal replacement
lipomatosis
with coexistent xanthogranulomatous
pyelonephritis
and multiple perinephric and parietal wall collections.

Renal replacement
lipomatosis
of the kidney is a rare entity characterized by renal sinus and perirenal fat proliferation often caused by renal calculi. Most of the renal parenchyma is replaced by fat and the kidney is usually small, atrophic and nonfunctioning. We report magnetic resonance imaging and ultrasonography findings of a pregnant woman with xanthogranulomatous
pyelonephritis
and renal replacement
lipomatosis
coexisting in the same kidney.

Renal replacement
lipomatosis
(RRL) is a rare condition that occurs as an end result of renal atrophy or destruction of renal parenchyma by excessive lipomatous tissue in renal sinus and perinephric space. We report a case of RRL presenting as a right renal mass clinically. Intra-venous pyelography and renal scan revealed a left non-functioning kidney. A left nephrectomy was performed. After histopathological examination and extensive literature search, a diagnosis of RRL was given. In majority of cases, it is associated with renal calculi. Idiopathic variety is not common. It is a rare entity and the diagnosis may be missed due to lack of experience. It has to be differentiated from other fat-containing tumors such as renal lipoma, xanthogranulomatous
pyelonephritis
, angiomyolipoma and liposarcoma.

Renal replacement
lipomatosis
is a condition characterized by varying degrees of renal parenchymal atrophy and perirenal fibrofatty proliferation secondary to chronic inflammation such as xanthogranulomatous
pyelonephritis
. In severe cases, imaging findings can be misdiagnosed as retroperitoneal liposarcoma.

Acute pyelonephritis is a bacterial infection of the renal parenchyma and collecting system. Diagnosis is based on clinical findings of fever, flank pain, and urinary tract infection. Computed tomography findings include renal enlargement with wedge-shaped heterogeneous areas of decreased enhancement, known as a "striated nephrogram." Imaging is primarily used to diagnose complications such as emphysematous
pyelonephritis
, renal abscess, and pyonephrosis. Chronic pyelonephritis can have varying appearances on imaging ranging from xanthogranulomatous
pyelonephritis
or, in extreme cases, renal replacement
lipomatosis
.
