The study describes the indications and results of combined liver/kidney transplantation in eight patients suffering from end-stage hepato-renal diseases. The causes of primary renal failure were hyperoxaluria type I (2/8), diabetic nephropathy (2/8), glomerulonephritis (2/8), congenital pyelonephritis (1/8), and polycystic kidneys (1/8). Only five of these patients were on chronic dialysis prior to transplantation. The indication for kidney transplantation in the other three patients was low GFR (< 20 mL/min) and the anticipation of further deterioration of the renal function after liver transplantation as a result of cyclosporine toxicity. The end-stage liver diseases were chronic active hepatitis (4/8) and alcoholic cirrhosis (2/8). There was no evidence for liver failure in two patients undergoing combined transplants for primary hyperoxaluria. The 1-year patient survival rate is 75 per cent, and at that time, kidney and liver function were found to be within normal range. In conclusion, excellent long-term patient survival, as well as kidney and liver graft function, can be achieved in patients suffering from complex end-stage disease of both organs who undergo combined liver and kidney transplantation.

Clinical effects of spa treatment on renal function in middle-aged and elderly male and female patients with chronic pyelonephritis and urolithiasis was studied. Combined sanatorium treatment included a course intake of low-mineral sulphate-hydrocarbonate calcium-magnesium mineral water Kazanskaia. Diuresis, especially daytime, was activated in all the patients. Maximum diuresis was observed in cool seasons in the elderly patients. To the end of the treatment proteinuria, oxaluria and uraturia diminished. A course of drinking mineral water Kazanskaia proved effective and is recommended for patients with chronic pyelonephritis and urolithiasis.

The exposure of the lumbar region to interference currents in combination with radon baths conducted in 37 patients with chronic pyelonephritis inhibited activity of the inflammation, hypercalciuria and hyperoxaluria, improved function of the kidneys. The same clinical effects were achieved in 32 patients who had received interference therapy in combination with intake of radon water.

The present study involved 40 children aged 7-14 suffering from chronic pyelonephritis and having an environmentally burdened medical history. The combined spa treatment included either traditional peloid therapy applied to the lumbar region or the specially developed treatment using the adaptive-reflexive method that allowed the therapeutic effect to be gradually spread over various reflex zones. Mud applications under the new scheme were shown to be more adequate and sparing because they ensured gradual sequential involvement of different reflex zones that is of special importance for the patients having an environmentally burdened medical history and characterized by the impaired adaptive capacity. The advantage of the proposed peloid-therapeutic modality is confirmed by the reduction in the frequency of the patients' complaints and the more pronounced dynamics of the signs of mikrohematuria, pyuria, and oxaluria combined with the restoration of the anti-crystal-forming urinary activity in response to calcium phosphates in all patients.
