Echocardiographic investigation of 110 patients with different forms of stable arterial hypertension demonstrated a moderately close correlation between left-ventricular myocardial weight, and systolic and diastolic arterial blood pressure in patients with essential hypertension and chronic diffuse glomerulonephritis, and a weak correlation between left-ventricular myocardial weight and systolic pressure in patients with renovascular hypertension and chronic unilateral or predominantly unilateral pyelonephritis. Inadequate left-ventricular hypertrophy has similar incidence (15-20%) in patients with different forms of arterial hypertension, whereas excessive hypertrophy only occurs in patients with essential hypertension. The frequency of asymmetrical hypertrophy differs in the two groups.

Erythrocyte Na-Li countertransport was assessed in 99 patients with various arterial hypertensions. Mean Na-Li countertransport rate was 476 +/- 43 mumol/l 1 cells/hour in essential hypertension, or nearly twice as high as mean rates for hypertensive chronic diffuse glomerulonephritis, chronic pyelonephritis, renovascular hypertension and nonspecific aortoarteritis, primarily affecting extracranial arteries. In 2 patients, operated on for renal arterial stenosis, Na-Li countertransport was high and the operation had no hypotensive effect, suggesting that renal arterial stenosis was combined with essential hypertension. In 1 patient, the original diagnosis of chronic diffuse glomerulonephritis and aldosteronoma was not confirmed at surgery and renal biopsy, and the diagnosis of essential hypertension was made instead.

The progression of renal failure was analyzed in 108 patients with mild to moderate renal impairment, none of whom had received any form of dietary protein, phosphate restriction or immunosuppressive treatment. The reciprocal of plasma creatinine was plotted against time using a minimum of six plasma creatinine values taken over at least six months (mean 13 values over 41 months). Plots indicated there was linear deterioration in 70 patients, non-linear deterioration in 15 and stable renal function in 24. Progressive renal failure was common in patients with glomerulonephritis, diabetic nephropathy, chronic pyelonephritis and polycystic kidney disease. Most patients with hypertensive nephrosclerosis, analgesic nephropathy and renal impairment following acute renal failure were stable. Among those with progressive impairment the mean rates of deterioration were significantly faster for patients with glomerulonephritis and diabetic nephropathy compared to those with chronic pyelonephritis, polycystic kidney disease and undiagnosed renal disease (p less than 0.01). Hence the underlying renal pathological changes appear to be important in determining progression of renal failure and also the subsequent rate of deterioration. For those with linear progression of renal failure there was a significant correlation between 24-h urinary protein excretion and the rate of deterioration. This relationship held for glomerulonephritis and chronic pyelonephritis as separate diagnostic groups only. Proteinuria, therefore, may be a useful prognostic index for the rate of progression of established renal failure. Calcium phosphate product correlated poorly with the rate of deterioration. We were unable to demonstrate a relationship between spontaneous protein intake and deterioration of renal function. However, patients prescribed high protein diets were not included in dietary analysis and we cannot, therefore, exclude the possibility that a high dietary protein intake may accelerate renal failure. Similarly we were unable to show a significant relationship between blood pressure and progression of renal failure although there were weak correlations between mean arterial pressure and rate of deterioration for chronic pyelonephritis and glomerulonephritis.

Novel approaches to managing refractory arterial hypertension (AH) have been tested in 130 patients aged 28 to 59 years with severe or malignant hypertension. Hemosorption was performed in 70 patients in whom AH was caused by chronic diffuse glomerulonephritis (49 cases) or chronic pyelonephritis (21 cases) accompanied by the appearance of chronic renal failure. In all patients, blood pressure after hemosorption decreased by 15% to 16% on the average, resulting in progressively improved renal function and a nearly 2.0-fold reduction in plasma aldosterone concentration (PAC), and allowing the doses of antihypertensive drugs to be reduced. Plasmapheresis was performed in 31 patients with refractory severe or malignant AH due to essential hypertension or parenchymatous diseases of the kidneys. After two to four plasmapheresis sessions with up to 2 L of plasma exchanged, blood pressure dropped by 24% compared to baseline while the doses of antihypertensive drugs were diminished and some were discontinued completely in several cases. Analysis of the sensitivity to antihypertensive drugs after plasmapheresis using the rosette technique revealed a significant decrease in the number of rosette-forming cells. The level of angiotensin II and urinary excretion of aldosterone-18-glucuronide declined progressively by nearly 50% after plasmapheresis, correlating with the antihypertensive effect of plasmapheresis. In 32 patients with severe AH complicated by refractory cardiac failure, isolated ultrafiltration was used. After one to eight sessions and the removal of 1.0 L to 35.8 L of fluid, the signs of cardiac failure diminished, the blood pressure level responded to drug therapy, and the PAC level decreased significantly. Although the mechanisms of the antihypertensive actions of hemosorption, plasmapheresis, and isolated ultracentrifugation are still not completely elucidated, these data suggest that hemosorption may act by removing nitrogenous residues from the body and reducing PAC, plasmapheresis by deblocking receptors for antihypertensive drugs and reducing the concentration of angiotensin II and the synthesis of aldosterone in the body, and isolated ultrafiltration by eliminating hyperhydration and edema of the parenchymatous organs.

In the pathogenesis of infectious diseases, great importance is attached to the problem of adhesion of bacteria to cells. In 100 urine specimens from normal test persons, patients with infections of the lower urinary tract, with chronic pyelonephritis and glomerulonephritis were studied. The adhesion of strains of Escherichia coli to human uroepithelial cells depended on the concentration of single urinary factors. While increased concentrations of urea and creatinine favored the adhesion process, a statistically significant negative influence was found with regard to potassium, immunoglobulins and pH value. Considering the multifactorial effects of the urinary constituents, we found in a multivariate comparison that none of the studied physiological features alone exerts a fundamental influence upon adhesion, but in their entirety they determine the environmental conditions for the adhesion of bacteria to cells in the urine.

Some neurophysiological techniques have been employed in clinical nephrology to record abnormalities of nervous conduction in central and peripheral pathways. The electrical monitoring on the peripheral and central nervous systems has allowed the detection of uremic neural injury, the diagnosis of specific electrophysiological abnormalities, the evaluation of various treatments employed and the identification of those abnormalities that uremia can induce. A group of 156 subjects subdivided into four groups were examined: 100 healthy subjects (64 M, 36 F); 56 patients (21 glomerulonephritis, 14 pyelonephritis, 5 nephrolithiasis, 5 polycystic kidney, 4 nephroangiosclerosis, 7 undetermined) with chronic renal failure treated with a conventional low nitrogen diet (CLND, 0.6 g/kg b.w./d. of proteins), 8 of whom passed from CLND to a very low nitrogen diet supplemented with alpha-keto-analogues; a group of 22 of these 56 underwent a regular dialysis treatment for 12 to 15 hours/weekly for 40.5 +/- 10.2 months. Three patients of the CLND group and 13 patients underwent renal transplantation after a variable period of RDT. In the uremic patients we found different populations of motor unit potentials; a decreased MNCV was found in 35% of the CLND patients, RDT patients had slowed MNCV in 42%. The SNCV was compromised more frequently than the MNCV. An increased duration of evoked potentials was sometimes observed in CLND and RDT patients inducing us to consider this a hallmark of uremic syndrome. Na-Li-countertransport was determined in 117 patients aged 16-17. The main groups included 22 patients with essential hypertension accompanied by minor urinary syndrome; 12 patients with hypertensive form of chronic diffuse glomerulonephritis; and 21 patients with chronic pyelonephritis either with or without urinary syndrome who exhibited no severe alterations in function, shape and structure of kidneys according to instrumental findings. 23 patients with chronic diffuse glomerulonephritis and 24 patients with chronic pyelonephritis reliably diagnosed by instrumental methods as well as patients with essential hypertension without urinary syndrome were included into groups of comparison. Patients with essential hypertension from both groups had individual Na-Li-countertransport values over 300 mumol/l RBC/hr; nevertheless, mean values of countertransport as well as systolic and diastolic BP levels were higher in the main group. Individual Na-Li-countertransport rates in patients with parenchymatous renal diseases did not exceed in overwhelming majority of cases 300 mumol/l RBC/hr. Main groups and groups of comparison did not differ in mean values of countertransport.

HBsAg was detected in the blood serum of patients with glomerulonephritis (GN) in 21.1% of cases. During examination of patients with other diseases of the kidneys HBsAg was detected in the blood serum much less frequently (8.3%). In patients with systemic autoimmune diseases it was found in 12.1%, in patients with pyelonephritis and amyloidosis in 7.4%. Anti-HBs and HBsAg in GN patients were defined with equal frequency (20.3%). Among patients with other renal diseases anti-HBs were found more frequently in systemic autoimmune diseases (36.7%). The presence of the other hepatitis B virus marker (HBeAg) was noted in 3 out of 15 patients with chronic GN with HBSAg in the blood serum and was undetectable in either of 3 patients with HBsAg in the group with some other renal diseases. Anti-HBe were detected in 6 patients with chronic GN. Of 16 GN patients HBsAg was detected in a kidney biopsy specimen of one patient.

The diagnostic value of ultrasound kidney biometry in the diagnosis of parenchymatous kidney disease was evaluated in 277 children. Kidney enlargement of more than 140% of normal volume proved to be a sensitive criterion for differentiation of acute bacterial interstitial nephritis (pyelonephritis) and isolated lower urinary tract infection. Mean kidney volume in acute pyelonephritis was 175%. Normalization of kidney volume under antibiotic treatment required 4-6 days. Kidneys with non-bacterial nephritis and glomerulonephritis showed a 63% pathological size increase at biopsy; the kinetics of kidney volume in the acute phase of the disease paralleled the clinical course.

A study of the level of beta 2-microglobulin (beta 2-MG) in the blood serum and urine was conducted in 67 patients: 22 with chronic pyelonephritis, 13 with gout with renal lesion, 25 with chronic glomerulonephritis (5 without hyperuricemia, 20 with hyperuricemia) and 7 with amyloidosis accompanied mainly by renal lesion. A raised level of beta 2-microglobulin was found in the patients with chronic pyelonephritis, gout, latent glomerulonephritis with hyperuricemia, and in over half of the cases its raised level was found in the urine. The results obtained indicate a frequent and in some cases predominant involvement of the tubules as well as interstition in the patients with hyperuricemia.
