Sera from 103 fasting individuals 3 to 76 years of age and free of clinical infectious disease and sera from 183 patients with infectious disease were assayed for serum total non-esterfied fatty acids (tNEFA) and compared. Data were also separated into five groups according to age of donor: 3--7, 8--19, 20--35, 36--60, and 61--76 years. The mean group serum levels of tNEFA increased with age. Among patients with infectious diseases sixty-five were diagnosed as having hepatitis, 41 with infectious mononucleosis, 18 with cellulitis, 12 with pulmonary tuberculosis, 11 with non-pneumococcal pneumonia, 9 with pneumococcal pneumonia, 8 with pharyngitis, 6 with
pyelonephritis
, 6 with aseptic meningitis, 4 with Gram-negative sepsis, and 3 with
encephalitis
. The sera from 23 non-fasting patients with gonorrhea were also tested. The serum tNEFA levels were found to be altered, in fact depressed from normal group values, only in patients with pneumonia or tuberculosis. This depression may be related to aberrant pulmonary metabolism during pneumonia.

This study compared the clinical features of diarrhea in breastfed and bottle-fed infants in Indonesia. Study subjects included 375 infants (221 breastfed, 88 bottle-fed, and 66 fed by a combined method) under 2 years of age admitted to Gunung Wenang Hospital with diarrhea. Both the duration of diarrhea and the prevalence of prolonged diarrhea were significantly greater in bottle-fed than breastfed infants. Diarrhea persisted for 8-14 days among 17% of breastfed infants, 28% of bottle-fed infants, and 35% of infants fed by a combined method. The mean duration of diarrhea was 5.5 days among breastfed infants compared with 7.2 days among bottle-fed infants. Also noted was a significantly greater incidence of acute otitis media and rhinopharyngitis among bottle-fed infants. The prevalence rates of pneumonia, bronchitis,

were not associated with type of infant feeding. It is suggested that acute infantile diarrhea may damage intestinal mucosa, causing an increase of foreign protein of cow's milk absorption and leading to the development of further intestinal mucosal damage and prolonged diarrhea. During episodes of diarrhea in bottle-fed infants, it is advisable to substitute low allergic milk or chicken meat porridge to avoid cow's milk intolerance.

The chemotherapeutic properties of the new aminothiazolyl cephalosporin HR810 were investigated in experimental animals. Unlike other cephalosporins of the third generation, HR810 had good activity against Staphylococcus aureus as well as some activity against enterococci. In murine protection tests with these strains, it was clearly superior to ceftazidime, cefotaxime, ceftriaxone, cefoperazone and latamoxef. The compounds most effective in protecting mice from infections caused by Enterobacteriaceae were HR810 and ceftriaxone followed by ceftazidime, latamoxef and cefotaxime; cefoperazone was less active. HR810 was less active against Pseudomonas aeruginosa than ceftazidime but was considerably more effective than the other cephalosporins tested. HR810 also proved effective against localised infections such as thigh lesions, as well as against meningo-
encephalitis
in mice and
pyelonephritis
in rats. These results in laboratory animals make HR810 a promising candidate for clinical studies.

A total of 82 episodes of diabetic ketoacidosis were analysed in 70 adult patients. Population characteristics can be seen in Table 1. It was possible to determine the causes of 74 episodes (Table 2); infections, insulin reduction or suppression and psychic stress included 89 % of these causes. The most frequent infection sites were airway, urinary tract and skin surface. The most important symptoms and signs shown by patients on admission (Table 3) were digestive and those derived from dehydration and acidosis. Figure 2 shows laboratory data on admission: average glycemia, 395 mg %, 90 % with pH values below 7.30; the majority revealed high hematocrit urea and kaliemia values. Unusual treatment performed in the classical way (Figure 3) can be divided into two periods: the first of eapid expansion and insulinization (first three hours) and the second of slow replenishment (4 to 24 hours) consisting of two stages in which the velocity of liquid infusion is diminished while glucose and potassium backing is started. No difference was found between the results of those who received bicarbonate and those who did not (Table 4). Response to treatment is shown in Fig. 4. On pointing out the decrease in kalemia (1.18 mEq/l in the first 6 hours), however, it must be kept in mind that on admission 10 % of the patients were in a state of hypokalemia with less than 3.5 mEq/l. Table 5 shows complications that arose during treatment: hypokalemia, 32 %; hupoglucemia, 11 % and phlebitis, 17 % (catheterized). Five patients, (7 5) died. ). The analysis of this paper shows the importance of an adequate diabetic education and briefing both for the patients, to be aware of the unleashing factors, and for the physicians, in order to avoid the complications of treatment.
