The reaction of leucocyte migration inhibition (RLMI), using antigens of autochthonous and allogenic tumors, was utilized to examine 21 patients with renal cancer and in 37 control patients. The antigens of renal cancer (AG RC) would suppress specifically leucocyte migration in all patients with
cancer
of the kidney, as compared with the migration without antigens or in the presence of normal tissue antigens of the tumor involved kidney. In 5 of 37 cases AG RC as well as those of normal renal tissue inhibited leucocyte migration in patients with urolithiasis and
pyelonephritis
. Autologous blood plasma in patients with renal cancer would contribute to inhibition of leucocyte migration by
cancer
antigens. There are some common specific antigens in renal cancer, recognized by lymphocytes from different patients with the tumor in question. RLMI may be used to establish the immune diagnosis of
cancer
of the kidney.

A 31-year-old woman developed multiple synchronous in situ carcinomas of the cervix uteri, vulva and perineal skin, six years after successful renal transplantation for chronic renal failure due to chronic
pyelonephritis
. During this time she had been continuously treated with immunosuppressive drugs (azathioprine and prednisolone). The lesions were treated by local excision and there was no evidence of recurrence nearly two years after the primary treatment. The case history is compared to others recently described and highlights the risk of epithelial as well as lymphomatous
malignancies
in renal transplant recipients.

The results of a retrospective autopsy study of 115 adult patients with haematological or lymphoreticular
malignancies
or who had undergone transplantation procedures, are presented. The overall incidence of infection was 65%, 123 infections being detected in 75 patients. The bulk of the infections involved the gastro-intestinal and respiratory systems, other systems being considerably less frequently affected. Patients who had received allografts and subsequent immunosuppression had the highest incidence of viral inclusions, especially cytomegalovirus. Candida infections were more common than aspergillosis, and severe fungal infections were most frequent in patients with acute leukaemia who had been treated aggressively. The only other mycosis detected was cryptococcosis. Bacterial pneumonia was the most frequent infection over-all (36%). Tuberculosis,
pyelonephritis
and Pneumocystis pneumonitis were also encountered.

Successful treatment of
cancer
of the urinary bladder consists not only in elimination of the tumour process, but also in prophylaxis of various complications. Among complications caused by the tumour process as well as by the applied methods of its therapy a certain place belongs to
pyelonephritis
.
Pyelonephritis
develops in consequence of the disturbance of the function of the urinary bladder after hemicystectomy resulting from the change of the bladder's form. With the view of preserving the anatomo-physiological function of the urinary bladder and preventing complications the author suggests a method of reconstruction of the bladder consisting in shaping it in a spherical form after hemicystectomy. Such shape of the urinary bladder ensures nearly normal conditions for both urination act and urinary flow from the upper urinary pathways, this being the prophylaxis of
pyelonephritis
.

Free ileoureteral reflux in patients with an ileal conduit has been indicated as the etiology of late renal complications. A new technique for ureteroileal anastomosis was devised in an attempt to prevent reflux without causing obstruction. This anastomosis has been done on 20 ureters from 11 patients with pelvic
malignancies
. Satisfactory results have been obtained in patients with normal as well as dilated ureters, with no serious operative complications, no episodes of
pyelonephritis
and no renal deterioration. The ureteroileal junction created with this technique has been demonstrated radiographically and histologically.

One hundred and seventy-five 67Ga-citrate scans were performed to detect suspected occult inflammatory processes. None of the patients had a known
malignancy
. Renal activity was noted in 12 patients (6.8%) on the 48-hr image. These patients had either
pyelonephritis
, acute tubular necrosis, vasculitis, or a renal abscess. Since delayed 67Ga uptake in the kidneys may be the first evidence of renal disease, further investigation, including either arteriography or biopsy, is necessary. In patients with a known
malignancy
, tumor involvement must be considered.

A case of acute
pyelonephritis
associated with venous encasement and amputation is described. Although venous encasement and amputation have been considered previously as specific signs of
malignancy
, it is now clear that these signs are non-specific, occurring also in inflammatory diseases of the kidney.

The effect of vitamin A deficiency and hypervitaminosis A on the urothelial carcinogenicity of N-[4-(5-nitro-2-furyl)-2-thiazolyl]formanmide (FANFT) was determined in female weanling Sprague-Dawley rats. Vitamin A deficiency resulted in squamous metaplasia of the urinary bladder and high incidences of cystitis, ureteritis, and
pyelonephritis
. Administration of FANFT to vitamin A-deficient rats appeared to accelerate the carcinogenic process, with earlier appearance of urinary bladder tumors and the development of ureteral and renal pelvic carcinomas. Most of these tumors were squamous cell, occasionally with transitional cell foci. Hypervitaminosis A prevented the appearance of squamous metaplasia and squamous cell neoplasia in rats fed FANFT, but it did not inhibit the formation of transitional cell hyperplasia or neoplasia in comparison to rats receiving normal levels of vitamin A and FANFT.

The relationship between selected aspects of medical history and the risk of non-Hodgkin's lymphomas (NHLs) was investigated using data from a hospital-based case-control study conducted in northern Italy on 177 cases of NHL and 561 controls in hospital for acute conditions, other than nonneoplastic or immunological. Among six viral diseases considered, only herpes zoster (shingles) had a relative risk (RR) significantly above unity [RR = 2.7; 95% confidence intervals (CI), 1.5 to 4.7]. The association, however, was restricted to subjects whose diagnosis of herpes zoster dated back to less than 10 years, suggesting that this slow-acting virus could be reactivated by the early development of NHL. Six of eight bacterial diseases considered showed RR above unity, and the estimate was significant for scarlet fever (RR = 1.9; 95% CI, 1.1 to 3.5) and
pyelonephritis
(RR = 5.3; 95% CI, 1.8 to 16.2). These associations were not restricted to the few years before lymphoma diagnosis. When various classes of infectious or inflammatory diseases were grouped together, no association was evident for viral infections (RR = 0.8; 95% CI, 0.6 to 1.2), acute bacterial diseases (RR = 1.0; 95% CI, 0.7 to 1.5), or allergic conditions (RR = 1.0; 95% CI, 0.6 to 2.1). The risk estimates were nonsignificantly above unity for chronic bacterial diseases (RR = 1.2; 95% CI, 0.7 to 1.2) and autoimmune conditions (RR = 1.4; 95% CI, 0.9 to 2.2), and significantly elevated for chronic inflammatory disease (RR = 1.9; 95% CI, 1.2 to 3.0).(ABSTRACT TRUNCATED AT 250 WORDS)

Advanced age is frequently considered a contraindication to radical exenterative surgery. We reviewed the outcomes of 63 patients age 65 years or older who underwent pelvic exenteration between 1960-1991 at The University of Texas M. D. Anderson
Cancer
Center. Sixty-three percent had preexisting medical illnesses. Major or potentially life-threatening complications were noted in 38% of the patients. An additional 38% experienced minor complications. Sixty percent experienced one or more infectious complications, including
pyelonephritis
, wound infection, sepsis, and flap necrosis. When both major and minor complications were considered, infectious morbidity was the single largest category. Although they are not life-threatening, nonspecific infectious morbidity and transient confusion were the most frequent individual complications, occurring in 26 and 24% of patients, respectively. Twenty-four percent of the patients experienced no complications. Thirty-four percent of the postoperative survivors suffered late major morbidity. Operative mortality was 11%; multisystem failure was the most frequent cause of death. After a mean follow-up of 4 years, 22 patients were alive with no clinical evidence of disease. Twenty-one patients died of recurrent disease, with a median time to recurrence of 9.6 months. The 5-year survival rate for the group was 46%. In comparison, 363 patients younger than age 65 who underwent exenteration during the same period experienced an operative mortality rate of 8.5% and a 5-year survival rate of 45%, neither of which were significantly different from the rates found for the older group (P = .51 and .52, respectively).(ABSTRACT TRUNCATED AT 250 WORDS)
