We report the case of a sixty-eight year old woman with chronic constipation, admitted in the emergency unit for respiratory deficiency and cervical subcutaneous emphysema. Endoscopy of the supradiaphragmatic air and digestive tracts was normal. Acutization of an abdominal syndrome required a laparotomy revealing a fissuration of the subperitoneal rectum on a fecal obstruction. This revealing complication may be explained by the topography of the cervico-thoraco-abdominal diffusion spaces.

We report a case of spontaneous rupture of the rectosigmoid junction demonstrated by surgical and histological examination. This rare complication of chronic constipation is difficult to confirm except in the case of the typical clinical presentation with lower abdominal pain after defaecation and evisceration of small intestine loops through the anus. No radiological procedure is usually performed except for plain abdominal radiograph, to demonstrate inconstant (retro)peritoneal emphysema. CT scan could detect free air outside of the rectosigmoid lumen, suggesting parietal rupture and allowing appropriate surgical treatment on the lower digestive tract.

Constipation with faecal impaction is a common condition, which may lead to serious potential complications. Among such complications, stercoral perforation has been rarely reported in the literature. We report a single case of 75-year-old woman, with a massive faecal impaction, which resulted in a rectum perforation, presented as a pneumoperitoneum, pneumomediastinum and subcutaneous emphysema. We present this case to remind physicians that neglected accumulation of faecal matter in the rectum may lead to ischemia and perforation of the colon and rectum. This case illustrates that severe chronic constipation requires adequate management, including disimpaction and aggressive medical treatment. Appropriate operative treatment may be life-saving.
