A case is described in which Fournier's gangrene was the presenting feature of diabetes mellitus and in which extensive subcutaneous emphysema prevented the use of spinal anaesthesia for debridement. In the literature four cases have been reported in which Fournier's gangrene was the presenting feature in patients with diabetic ketoacidosis. Diabetes may predispose to a form of Fournier's gangrene in which subcutaneous gas formation is marked, though subcutaneous emphysema to the degree described in this case has not previously been reported in this condition. Diabetes mellitus and subcutaneous emphysema must be sought in patients with Fournier's gangrene as both may have a profound influence on management.

A 44-year-old male with a history of diabetes mellitus and alcoholism for 20 years, was admitted to our hospital complaining of urinary retention and high fever. The scrotum was swollen and became necrotic partially. The inflammation and emphysema was around the anus, bilateral inguinal region, and right flank. The laboratory data showed disseminated intravascular coagulation (DIC). According to the clinical and radiological findings, we diagnosed the illness as Fournier's gangrene. Cystostomy and surgical debridment were performed in conjunction with the use of broad-spectrum antibiotics and anti-DIC therapy. After the general condition improved, the broad defect of perineal skin was covered with a free skin graft by using the negative-pressure bolster method. The graft was successful.

Fournier's gangrene is a fearsome disease with a bad prognosis and a mortality rate ranging between 10 and 80% according to the literature. It is extensive in 13 to 54% of cases. Up to date, cervico-facial extension has never been reported. We describe the case of a 51-year-old overweighed woman with a history of type 2 diabetes and a narrow lumbar canal who was referred to our institution for significant fatigue and increasingly painful legs. A diagnosis of Fournier's gangrene was made after correlating the physical findings with the results of a full body scan. Diffuse subcutaneous emphysema involving the face, neck, mediastinum, abdominal wall, right buttock, perineum and the right thigh was identified. Treatment included multiple surgical debridements, admission to intensive care unit, and an efficient antibiotic therapy that enabled preservation of the patient's life. To our knowledge, this is the first case of cervical and mediastinal extension of Fournier's gangrene to be reported. No clear guidelines exit on the management of this complication (cervico-facial and mediastinal drainage). We share our experience of this unusual case.

A 73-year-old man with a history of diabetes was urgently referred to our emergency department because of severe pain of his genitalia and pubical region. Physical examination revealed an extensive subcutaneous emphysema and edema of his abdomen. Computer tomography confirmed an extensive necrotizing fasciitis, diagnosed as Fournier's gangrene. Despite the efforts made, the patient did not survive. And a review of the medical approach of this short of a life-threatening form of necrotizing soft tissue infection with a high mortality rate.

We present a case of Fournier's gangrene secondary to rectal cancer that was managed with perineal reconstruction using a posterior thigh flap after debridement and tumor resection. A 67-year-old man was admitted with disturbed consciousness as well as hip and right thigh pain. His perineal and gluteal skin was necrotic. CT revealed subcutaneous emphysema that had spread from the rectum to the gluteal region and right thigh. We diagnosed him with Fournier's gangrene, and then removed the necrotic tissue and constructed a sigmoidostomy. A diagnosis of rectal cancer was later confirmed. Fifty-nine days after the initial operation, a laparoscopic abdominoperineal resection was performed. The perineal defect was repaired using a posterior thigh flap with the gluteus maximus. There were no postoperative complications, and the patient was discharged ahead of schedule. We concluded that the use of a posterior thigh flap is a good choice for perineal reconstruction, because it is simple to perform, can cover a large area, and has a low risk of infection.

Fournier's gangrene is a life-threatening type of necrotizing fasciitis associated with a high rate of morbidity and mortality. The patient is a 29-year-old incarcerated male who presented to the ED with left-sided scrotal crepitus extending into the axilla and testicular swelling. The patient endorsed a pimple on his left scrotum accompanied with groin pain. He had a prior history of foreign body removal and self-mutilating behaviors. The patient was taken for surgical exploration out of concern due to a subcutaneous emphysema secondary to a necrotizing soft tissue infection.
