A recent U.S. Public Health Service report compared the incidence of complications with 3 methods of midtrimester abortion. The comparison was based on a 4-year study of abortions done in 32 institutions. Major complications were least frequent with cervical dilatation followed by curettage or suction evacuation, more frequent with hypertensive saline injected into the amniotic sac, and most frequent with instillation of prostaglandin F2. By the second trimester, dilatation and evacuation is a difficult procedure. Major complications considered were: fever for 3 or more days; pelvic infections; pneumonia; thrombophlebitis; hemorrhage requiring transfusions; psychiatric hospitalization;
hypernatremia
; injury to bladder, ureter, or intestines; wound disruption; unintended major surgery;
pulmonary embolism
; convulsions; endotoxic shock; cardiac arrest; and death. The report is difficult to evaluate. Most medical letter consultants prefer intraamniotic administration of prostaglandin F2 or saline for midtrimester abortions.

, is a rare complication after successful cardiopulmonary resuscitation with therapeutic hypothermia, but may be potentially fatal if recognition is delayed. We describe here a patient who experienced cardiac arrest due to a
pulmonary embolism
, followed by successful resuscitation after induction of therapeutic hypothermia. The patient, however, suddenly developed unexpected
hypernatremia
with increased urine output and was diagnosed with central DI as a complication of cerebral edema, and eventually died. Our findings suggest that central DI should be considered as a possible complication following unexpected
hypernatremia
with increased urine output during therapeutic hypothermia and that desmopressin acetate should be used to treat central DI.

We describe a case of 65-year-old obese female patient with
pulmonary embolism
and life-threatening
hypernatremia
after removal of craniopharyngioma. On the 18th day after neurosurgical procedure,
pulmonary embolism
developed abruptly. Immediately after placement of inferior vena cava filter, surgical removal of the pulmonary thrombus was performed under cardiopulmonary bypass. Although mechanical ventilatory support and infusion of noradrenaline were required postoperatively, the trachea was extubated on the 10th postoperative day. Meanwhile, daily serum Na level increased gradually and reached 178 mEq x l(-1). We suspected that dehydration and pituitary dysfunction were mainly responsible for the
hypernatremia
. Human atrial natriuretic peptide (hANP) was infused from the 2nd to the 4th postoperative day, and her urinary Na excretion became increased and serum Na level became normal. After discontinuation of hANP, urinary Na excretion became decreased again and serum Na levels increased transiently. However, her consciousness level and cardiopulmonary condition improved and she was discharged from the ICU after twelve days of ICU stay. HANP may be useful for treatment of life-threatening
