We have studied the influence of cold exposure on
itch
, erythema and wheal, in response to histamine scratch tests, in 14 volunteers. Cooling of the skin to less than 20 degrees C, by application of an ice cube for 30 min on the inside of the forearm, abolished
itch
and reduced erythema by approximately 50%, whereas the size of the wheal was unaffected by cooling. The observations bear significance for an explanation of the well-known observation that cold relieves
itch
. A normal

response seems to require a continuous metabolic process in the skin, which is inhibited at temperatures less than 20 degrees C. The skin symptoms,
itching
and erythema, among workers in the fish processing industry are mainly localized to the forearms and backs of the hands, but only seldom to the fingers and palms, although they are in direct contact with fish products. Skin temperature measurements have shown that the temperature on the fingers and palms is less than 20 degrees C, while the temperature on the backs of the hands and forearms ranges from 25 to 30 degrees C. We therefore conclude that the skin temperature is an important factor for the location of skin symptoms among workers in the fish processing industry.

In a randomized, double-blind study of 39 mothers in labour, we have compared a loading dose of 0.5% bupivacaine 6.0 ml and fentanyl 100 micrograms given extradurally, followed by an infusion of 0.08% bupivacaine 15 ml h-1 plus fentanyl 37.5 micrograms h-1, with a loading dose of 0.5% bupivacaine 6.0 ml and saline 2.0 ml, followed by an extradural infusion of 0.08% bupivacaine alone, per hour. Analgesic levels were more consistent and sustained in mothers who received fentanyl in addition to bupivacaine, and the duration from the time of the loading dose to the first top-up was extended considerably in this group. The only significant side effect was a high incidence of mild
pruritus
in the fentanyl group. The addition of fentanyl to the extradural loading dose and subsequent infusion of local anaesthetic is a satisfactory alternative to giving higher doses of local anaesthetic alone.

PMID:Comparison of bupivacaine and bupivacaine with fentanyl in continuous extradural analgesia during labour. 280 82
The author describes the therapeutic results in 48 patients with chronic anal fissure by means of internal lateral sphincterotomy (ILS). He operated patients where the complaints did not recede after conservative treatment supplemented by divulsion of the anus. In 43 patients (89.58%) in the course of two to three weeks the anal fissure healed. Only four patients (8.33%) experienced after operation a slight discomfort such as

in the perianal region. A relapse after temporary improvement occurred in one patient (0.49%). ILS is a simple and safe operation performed under general anaesthesia after precise differentiation of the internal sphincter. The results of surgical treatment are very satisfactory the marked painfulness recedes immediately after operation, the fissure heals rapidly after relaxation of the spasm of the internal sphincter. Postoperative treatment is simple, the percentage of complications is small. None of the patients operated in the authors' department developed incontinence.

The feasibility of using infra-red thermography for early and out of season detection of summer seasonal recurrent dermatitis (sweet
itch
) was tested on 13 sensitive and six insensitive horses in Israel. In summer (June, July and September) six affected mares and two unaffected sensitive mares (one of them a pony) were clearly 'warmer' in the affected zones than three insensitive mares. In winter (February and March) unaffected sensitive horses could be differentiated from insensitive mares by the warm areas detected in the affected zones. Small numbers of Culicoides imicola bites may have been the cause of the local heat reactions of the body that were detected by the infra-red radiometer.

A 78-year-old woman with extramammary Paget's disease presented with vaginal bleeding and vulvar
pruritus
. Paget's cells were demonstrated initially in a cervical cytosmear and, subsequently, in the epithelia of the vagina and cervix, in addition to those of the vulva, periurethral area, and perineum. Such extensive intraepithelial spread of Paget's disease in the genital tract has rarely been reported previously. This is the first case, to our knowledge, in which the cells of cervicovaginal Paget's disease were detected by Papanicolaou smear.

The effects of extradural sufentanil 50 micrograms were investigated in 10 normal volunteers. Eight of these were studied at a second session when adrenaline 1:200,000 was added to the sufentanil. Well-defined segmental analgesia developed rapidly after plain sufentanil and lasted approximately 3 h. Respiration was depressed for about the same period and was greatest in the first 2 h, as shown by a 15% increase in PECO2, while the slope and VE50 of the carbon dioxide response curve were depressed by 45% and 55%, respectively. Moderate drowsiness occurred in most subjects, while other side effects of
itching
, nausea and urinary retention occurred less frequently and were not severe. Addition of adrenaline 1:200,000 intensified segmental analgesia and prolonged duration to 5 h, while side effects were lessened. It is concluded that extradural sufentanil shows considerable promise for clinical use, and that the risk: benefit ratio is improved by adding adrenaline 1:200,000.

Seventeen oncological patients reported unpleasant but clinically harmless skin sensations directly after intravenous injection of dexamethasone, persisting for 2-4 minutes. In 16 cases, these paraesthesias (tingling, burning,

, twinging) occurred in the genital or perineal area. In one case, the symptoms started in this area and then extended over the entire truncus integument for a short period. The cause of this obviously drug-induced side effect is unknown.

Patients with a history of recurrent candidiasis and who were using Depo-Provera (medroxyprogesterone acetate, DMPA) for contraception were reviewed in order to determine the time relationship between episodes of proven candidiasis, episodes of
pruritus
vulvae suggestive of this infection (but unproven), and injection of DMPA. Recently, patients were included in the study who had been given DMPA specifically to prevent recurrences of candidiasis even when the drug's contraceptive action was unnecessary, such as after sterilization. In all cases, the infection was initially treated with a vaginal candidacide, most commonly 1 week of an imidazole. The patients ranged in age from 19-37 years at the time of the 1st injection. Diabetes had been eliminated in all the cases. DMPA was given intramuscularly at a dose of 150 mg every 12 weeks. Prior to 1983, an estrogen supplement was prescribed in most cases in an effort to produce monthly menstrual periods. Estrogen supplementation is no longer used routinely, with amenorrhea the aim, although it is occasionally given to women who experience breakthrough bleeding. Candidal infection was considered proven when the branching filaments of the species were seen on a stained vaginal smear or when the species were cultured in a laboratory from a vaginal swab taken a symptomatic patient. With the exception of 2 patients, clinical candidiasis did not occur within the time in which 150 mg of intramuscular DMPA is known to suppress ovulation in all women, i.e., 12 weeks -- except in the presence of exogenous estrogen (cases 1, 2, and 14) and in one case (15) in which the patient had an unplanned conception prior to the injection. Both patients who experienced clinical despite the use of DMPA alone (cases 8 and 13) asked remain on the drug because believe it was responsible for their longest remissions in the past few years. The study seemed to provide evidence that DMPA will prevent a recurrence of clinical candidiasis in many women who are prone to this condition. The study further indicated that estrogens may predispose women to this infection.

Although the fingers are in direct contact with fish meat and juice, skin symptoms only seldom occur here, but instead almost exclusively on the forearms (70%) and the backs of the hands (26%). The skin temperature of the fingers and palms of the hands ranged from 17 degrees C to 20 degrees C, while the temperature on the backs of the hands and forearms ranged from 25 degrees C to 30 degrees C. Skin temperatures less than 20 degrees C abolish
itch
and reduce vasodilation by half. We suggest that the low temperature on the fingers affords protection against the development of some irritant skin reactions and that differences in skin temperature may be an important reason for the location of skin symptoms.

We performed clinical trials to evaluate the therapeutic effects of Foltene in patients of the several types of hair fallings. Thirty patients with male pattern baldness, alopecia areata and seborrheic alopecia were included in this study. Foltene was applied every other day for 40 days, and followed by maintenance therapy of twice application a week. The duration of whole therapy was 6 months. We conclude that Foltene is an effective and agent for male pattern baldness, alopecia areata and seborrheic alopecia from the following results. Ten patients with male pattern baldness was treated with Foltene for 6 months. Foltene had therapeutic effects of 50% in hair regrowth, 70% in decreased hair falls, 30% in decreased dandruff, 50% in decreased seborrhea. Thirteen patients with alopecia areata was treated with Foltene for 6 months. Foltene had therapeutic effects of 61.6% in hair regrowth, 53.9% in decreased in hair falls, 53.9% in decreased dandruff, 77.0% in decreased seborrhea. Seven patients with seborrheic alopecia was treatment with Foltene for 6 months. Foltene had therapeutic effects of 85.8% in hair regrowth, 57.2% in decreased hair falls, 42.9% in decreased dandruff, 85.8% in decreased seborrhea. The degree of therapeutic success was related to the duration of therapy. The side effects were as followed:

sensation developed in 2 patients (6.7%); tingling sensation in 3 patients (10.0%); burning sensation in 1 patient (3.3%); erythema in 3 patients (10.0%).
