Fleas are small, reddish-brown, wingless insects with a laterally compressed body and a pronounced third pair of legs adapted to leaping. Of the 100 species found in Middle Europe, hardly a dozen are of medical importance, they concern mainly people in contact with domestic animals. The cat flea, Ctenocephalides felis felis, and the bird flea. Ceratophyllus gallinae, are the most important human-pathogenic species in our region. A flea bite shows first as a haemorrhagic punctum, accompanied by
itching
, and leads to an erythema with or without central wheal. After 12--24 hours a papule appears which persists up to 2 weeks. Linimentum zinci with 10% Neocid alleviates the
itching
and prevents further infestation. The fleas are destroyed in their hiding places and on their animal host by applying Toxical-, Neocid- or Noflo-powder. The tropical sand flea, Tunga penetrans, is a permanent ectoparasite of man. It is seen in people returning from the tropics. Fleas may, even in our region, be vectors of bacteria, viruses, rickettsiae and intestinal parasites.

The infection with the mite Cheyletiella yasguri Smiley in 3 dogs and 5 out of 6 persons exposed to infestation is reported. Two of the dogs appeared to have been infected during a stay in a kennel. The dogs presented with moderate

and slight to massive floury squamation on the back. In the infected persons, excoriated papules were present on the body and the extremities. The
itching
, which had persisted for 4--12 months, disappeared in all cases a few days after the dogs had been removed from the house. In the dogs, three antiparasitic baths were sufficient to exterminate the mite population.

Patients with prior anaphylactoid reactions (AR) to radiographic contrast media (RCM) are at increased risk for another reaction upon repeat exposure to RCM. One hundred one patients, who had prior AR to RCM, who gave informed consent, and who had an essential need for a repeat RCM study, were pretreated with prednisone, 50 mg orally every 6 hours for 3 doses ending one hour prior to RCM study, and diphenhydramine, 50 mg intramuscularly, one hour prior to RCM study. The repeat RCM study was then carried out using standard procedures with resuscitation equipment readily available. Ninety-six patients had no reaction. Five of the 101 (4.95%) developed AR. These AR consisted only of mild urticaria or
pruritus
. No significant or life-threatening reactions occurred. Pretreatment decreases the risk in this population of patients and is recommended as standard prophylaxis for patients requiring RCM who have had a previous AR.

A man who was suffering from recurrent staphylococcal infection had antecedent symptoms of severe
pruritus
. Laboratory investigations showed leukocytosis with eosinophilia, hyperimmunoglobulinemia of all fractions, but particularly of IgE, and a deficiency of cell-mediated immunity on in vivo testing. Phagocytosis and bactericidal activity of polymorphonuclear leukocytes were normal, but a cellular and serum-associated defect in leukocytotaxia was present. Ultrastructural changes were observed in polymorphonuclear leukocytes. Association of impaired leukocytotaxia and elevated levels of IgE is not uncommon. Recurrent bacterial infections in the patient described are probably related to defective chemotaxis.

Immunoblastic lymphadenopathy was first described in the german literature in 1975 by Lennert as "Lymphogranulomatosis X." The disease is characterized by generalized lymphadenopathy, hepatosplenomegaly, dysproteinemia, fever and hyperergic reactions such as
pruritus
, skin rush and eosinophilia. The first ORL occurrence of the disease in Waldeyer's ring and cervical lymph nodes is reported. The histologic picture is characterized by a vascular proliferation with immunoblasts, plasma cells and interstitial amorphous acidophilic material but without Sternberg cells. The origin of the disease from primary malignant neoplasia or hyperergic reactions has not yet been defined. Although prognosis is uncertain and therapy limited, the best management involves small doses of corticoids, supplemented by antibiotics.

An aromatic retinoid (Ro-10/9359) was used for oral treatment of five cases of ichthyosis (three lamellar, two X-linked. Complete clearing of the skin lesions was achieved in all five patients within 24.2 +/- 3.2 days (X-linked 21.75 +/- 6.5, lamellar 23 days). Histopathology showed reduction of the hyperkeratosis, and thickening of the granular layer. Clinical side effects were of mild intensity and included cheilitis, conjunctivitis and
pruritus
. All side effects were reversible upon reduction of the daily dosage. In three patients treatment was discontinued after clearing of lesions. Fresh lesions re-appeared 6 weeks later. One patient with X-linked ichthyosis developed two recurrences during maintenance treatment; one patient with lamellar ichthyosis was kept in complete remission for 9 weeks on a reduced daily dosage.

Cortisol concentrations in umbilical cord plasma at birth were determined in association with pregnancy complications which may affect the time of pulmonary maturation of the fetus or otherwise influence its survival. No statistically significant change in these cortisol levels was observed in either term or premature pregnancies with the following complications: maternal disorder (preeclampsia, diabetes,
pruritus
), prolonged rupture of membranes, antenatal isoxsuprine treatment, maternal cigarette smoking, and neonatal asphyxia. Significant increases were observed in premature births in association with intrauterine fetal growth retardation and with the appearance of meconium. Cortisol concentrations in more severe forms of diabetes were significantly less than in Class A diabetes, in premature cases, and also tended to be low in association with various fetal congenital abnormalities. The possible physiologic significance of these results is discussed.

Using routine liver function tests, cholestasis of pregnancy was diagnosed in 86 pregnant women with
pruritus
. Serum aminotransferase levels were elevated in all cases, ASAT in 99%, and ALAT in 100%. In these patients serum concentrations of cholic, chenodeoxycholic, and deoxycholic acid were determined using a gas chromatographic method and were compared with those in a group of 40 uncomplicated pregnancies. Of these bile acids, cholic acid levels were most frequently elevated, ie, in 92% of the patients. The frequency of elevation of serum levels of alkaline phosphatase, and total and conjugated bilirubin was lower. Thus, it appears that in addition to serum aminotransferase levels the serum cholic acid concentration is a sensitive indicator of cholestasis of pregnancy. The cholestasis series was divided into 3 subgroups of increasing severity of cholestasis as assessed by maternal serum cholic acid levels, and the occurrence of signs of fetal distress was compared between these subgroups. The only intrauterine fetal loss in the series belonged to the severe cholestasis group. The incidence of meconium-stained amniotic fluid also increased significantly in this group, and 21 of the 24 cases with other signs of fetal distress were in the groups of moderate and severe cholestasis.
