A heavy infestation of the tussock moth caterpillar (Orgyia pseudotsugata McDonnough) in forested areas of Oregon was associated with itching of the skin and eyes, nasal discharge, cough, and, at times, respiratory difficulty, Personal interviews and inspection of forty-one occupationally exposed persons were supplemented by a questionnaire administered to 428 individuals, composing three groups at various degrees of risk and a control group. There was a cause and effect relationship between the adverse symptoms and the exposure to tussock moth larvae. The specific etiologic agent was not identified, but it was felt that the secretion, hairs, or other substances in the larvae or cocoons of the moth acted as a potent allergenic substance and, for some persons, was also a primary skin irritant.

Nine Nigerians with severe onchocerciasis who were treated with diethylcarbamazine developed clinical changes, ranging in severity from mild itching to distress, cough, and syncope. Physiological changes (fever, tachypnoea, tachycardia, or hypotension) were seen in eight. In five patients the systolic blood pressure fell by more than 25 mm Hg, and one patient collapsed on attempting to sit up. Circulating eosinophils decreased profoundly in all cases, reaching their lowest levels just before or during the clinical and physiological changes. A fall in serum complement (c3) accompanied the reaction but there was no fall in antibody titre. Diethylcarbamazine probably acts on the parasite's cuticle, thus exposing it to the body's defence mechansims. The reaction coincides with the death of microfilariae, and the accompanying physiological changes may be so severe, even in generally healthy patients, the treatment should perferably be started in hospital.

Adverse effects of opioids are multiple. They are most often receptor-mediated and inseparable from their desired effects. The most severe mishaps with opioids are related to their respiratory depressant effect, which is widely influenced by factors such as pain, previous opioid experience and awareness. Other relevant central nervous system effects of opioids include cough suppression, nausea and vomiting, rigidity, pruritus and miosis. The cardiovascular adverse effects of opioids are mainly related to histamine release and differ widely between agonists and agonist-antagonists. Gastrointestinal effects such as constipation, reflux and spasms of the bile duct are well described. Adverse effects on endocrine, immunological and haematological functions are possible, while allergic reactions are extremely rare. The adverse effects of long term use are overestimated. Systemic toxicity is negligible and development of tolerance is minimal while treating pain. In the clinical setting of pain control, addiction and withdrawal do not pose significant problems. Nevertheless, the possible effects of opioids on the unborn child should always be considered. Overall, opioids show a good record of safety. Their use should not be unduly limited by unfounded fears of adverse effects, but these effects should be avoided by anticipation and prevention.

A report from Kampala, Uganda, compares the situation in 1991 to the state of chaos 10 years earlier when the regime of Idi Amin had been overthrown by Milton Obote's soldiers with the help of Tanzanian troops. Soldiers went on looting sprees, and 1 victim of their marauding became a 12-year old boy who got shot for refusing to part with his bike. In contrast, in 1991 things were much more peaceful; however, the AIDS epidemic was the new threat. The government radio transmits hourly warnings on HIV. Since President Museveni came to power, economy and security have improved radically. Shops and markets are open until late at night; public transport is reliable, and small scale industry flourished. There would be optimism about the future, if AIDS was not here. There is no doubt that the economy will soon be affected. According to the Kampala blood bank, 40% of the healthy population is already seropositive. In the hospitals the majority of admissions suffer from AIDS with diarrhea and an itching dermatitis; there is more cancer of the cervix and lymphoma; appendicitis is on the increase; and tuberculous lymph nodes are now quite common. Many of these patients have clinical AIDS. The government is frank about the situation and is active in preventive measures and education. Private charities and foreign aid organizations contribute. But the epidemic is so overwhelming, that some Western organizations might soon lose interest owning to meager returns on their efforts. A 6-year-old boy has grossly swollen lymph nodes around his neck, both parotids are painfully swollen, pus pours from the ears. A nonspecific cough and mild diarrhea are also present with an itching and sore herpes zoster on his left chest. the mother is frightened of losing him, and demurs at the hint of AIDS, since for her, AIDS means sexual promiscuity.

Thirty-eight workers from a factory producing nickel-cadmium and other types of batteries came to us for medical evaluation. They included 21 women and 17 men (seniority 2-20 years, age range 31-63 years), and represented a self-selected subset of 700-900 ever-employed and 200+ recently or currently employed workers in the factory. Thirty-four worked on the nickel-cadmium assembly line. Symptoms and signs included: headache in 34; weakness, fatigue and lassitude in 26; dizziness in 16; pruritus and skin eruptions in 37; gingivitis, teeth loss and caries in 34; nasal congestion, nosebleeds and anosmia in 30; cough, phlegm production, wheezing and shortness of breath in 26; "asthma" in 14; bone pain in 18; urinary frequency, beta 2 microglobulinuria and kidney stones in 17; and sterility or multiple abortions (33) in 8 of 21 women. One additional patient had died from an "amyotrophic lateral sclerosis-like syndrome", while CT scans in six workers revealed brain atrophy. One other worker had leukemia, and two had died from cancer (lung and pancreas). Those who had worked for more than 10 years had more symptoms and signs than shorter-term employees, especially neurological illness, bone pain and urinary tract problems, including beta 2 microglobulinuria. Past blood and urinary cadmium levels were in the range of 1.6-8.7 micrograms/dl and 8-306 micrograms/l, respectively. Our findings indicated that: a) health risks for workers were not confined to the nickel-cadmium assembly line or to older workers, b) hazardous exposures still existed and illness appeared in new workers after a clean-up and intervention program, and c) exposures involved increased risks for renal disease and cancers. Finally, there is a need to control exposures and determine health risks in the full cohort of those ever employed, in the workers' children, and in the surrounding environment (air, ground, water) due to the dumping of waste from the plant.

Administration of large doses of fentanyl is a popular method to provide postoperative analgesia after thoracotomy. It is however unclear whether epidural lumbar (L) or epidural thoracic (T) administration of fentanyl confers any major advantage over intravenous (iv) infusion. Using a randomized prospective study design, we compared the potential benefits of L, T, and iv fentanyl administration after thoracotomy in 50 patients. Epidural catheters were not injected during surgery. Postoperatively a fentanyl infusion (5 micrograms/ml) was started at 1 microgram.kg-1.h-1 after a bolus of 1 microgram/kg and adjusted to maintain a score < or = 30/100 at rest using a visual analog scale (VAS) for pain. Data were prospectively collected before surgery, at fixed intervals during the 48 h of fentanyl infusions, and the day of discharge. There was no difference between the groups in overall quality of analgesia at rest and after coughing, quantity of fentanyl delivered (L = 1.15 +/- 0.38, T = 1.22 +/- 0.23, iv = 1.27 +/- 0.3 micrograms.kg-1.h-1), incidence of pruritus needing treatment (L = 2, T = 1, iv = 0 patients), need to decrease fentanyl infusion rate because of side effects (L = 2, T = 2, iv = 4 patients), importance of pulmonary infiltrates, or arterial blood gas values. One patient (L group) needed naloxone (0.04 mg iv). A 42-year-old woman was admitted because of cough, sputum, and fever. A chest roentgenogram revealed a nodular density in the left upper lung field with satellite lesions compatible with tuberculoma. Mycobacterium tuberculosis was detected from sputum. Five weeks after starting the treatment with 0.4 g/day of isoniazid, 0.45 g/day of rifampicin, and 0.75 g/day of streptomycin, she showed itching erythema in the trunk. The white blood cell count was 4,500/mm3 with 14% eosinophils, and serum transaminases were slightly increased (GOT 101 U/L, GPT 74 U/L). Although isoniazid and rifampicin were stopped, the erythema with exfoliation spread to her extremities, suggesting exfoliative dermatitis. The white blood cell count reached 15,990/mm3 with 68% eosinophils (10,810/mm3). Stimulation indices measured with the lymphocyte stimulation test (LST) were 109% with rifampicin, 140% with isoniazid, and 275% with streptomycin, suggesting streptomycin-induced allergy. After cessation of streptomycin, the symptoms gradually improved. After the reaction had subsided, the treatment with isoniazid, rifampicin, and ethambutol was resumed, but she showed no further adverse reactions. LST seems to be very useful to identify the drug or drugs responsible for the reactions occurred during the treatment by antituberculosis drugs.

Exposure to environmental tobacco smoke (ETS) is a potential occupational carcinogen according to guidelines of the Occupational Safety and Health Administration (OSHA) carcinogen policy (1). Exposure to ETS in the workplace may represent a substantial contribution to lifetime ETS exposure (2). For many persons, ETS irritates the conjunctiva of the eyes (accompanied by reddening, itching, and increased lacrimation) and the mucous membranes of the nose, throat, and lower respiratory tract (accompanied by itching, coughing, and sore throat) (3). As part of the 1988 National Health Interview Survey-Occupational Health Supplement (NHIS-OHS), CDC measured the degree of discomfort caused by ETS in the workplace. The NHIS-OHS collected information on cigarette smoking, workplace smoking restrictions, and perceived discomfort caused by ETS at the workplace. This report summarizes survey findings and describes efforts to reduce ETS at the workplace.

The aim of the randomized multicentre double-blind study was to establish whether nonionic contrast media differ in tolerability. The controlled comparison was conducted at 4 centres on a total of 798 patients with indication for i.v. DSA and computed tomography. The nonionic contrast media Iopamidol and Iopromide with an iodine content of 300 mg/ml each were studied. The osmolality of both contrast media is virtually identical. Both groups of patients were largely homogeneous in terms of demographic data and case history. The history of allergies was positive in 32.4% of the Iopamidol group and in 32.3% of the Iopromide group. The arithmetic mean of the doses administered to each patients was 148.3 and 149.3 ml, respectively. At all 4 centres there was a higher incidence of adverse events with Iopromide than with Iopamidol. The main differences with regard to pseudo-allergic reactions was the incidence of urticarial reactions, pruritus, nausea, vomiting and coughing. 89 of the 399 patients (22.3%) receiving Iopamidol and 120 of the 399 patients (30.1%) receiving Iopromide experienced pseudo-allergic reactions. The difference between both contrast media is statistically significant (p less than 0.05). There is a 95% certainty that less pseudo-allergic reactions will occur after an Iopamidol injection than after Iopromide. Possible explanations for the differences in tolerability between the two nonionic contrast media with identical osmolality are discussed.

An outbreak of Gnathostoma larva migrans occurred among guests of a New Year's party in Chachoengsao, Thailand. Nine people who consumed a raw fish dish called 'Hu-sae' contracted the disease. Five of them developed gastro-intestinal symptoms consisting of nausea, vomiting, abdominal cramps and diarrhea as early as within the first 24 hours, while in the other four, symptoms started on the following day. After the initial symptoms pertaining to the gut, malaise, chest discomfort, cough, myalgia, weakness, itching and migratory swellings were experienced. Eosinophilia was demonstrated in every patient with a mean (+/- SE) count of 5,516 +/- 1,010 cells/cu mm. Detection of antibody against aqueous extracts of G. spinigerum adult antigen using an enzyme-linked immunosorbent assay showed a titer of 1:1,600 or greater in every patients except one who had a titer of 1:400 (positive greater than or equal to 1:400). This outbreak illustrates the high attack rate when heavily infected fish are consumed.
