All the above listed reasons as well as the experience we have obtained with this method, justify our recommendation of the introduction of the laboratory diagnosis of scabies through the method of lye scraping of the skin into routine practice. A single parasite element is enough for determining the diagnosis while, for the above mentioned reasons, a negative microscope finding does not exclude the parasitic etiology of the given disease. We therefore recommend in the diagnosis of scabies that the following criteria are parallely used: 1. an objective skin finding 2. laboratory proof of the causative agent of

3. subjective feelings of the patient (

at the typical time) 4. positive epidemiological anamnesis, primarily the occurrence of clinical symptoms in several members of a family or a group 5. diminishing of clinical symptoms after specific treatment with an anti-scabies medicament.

Two methods have been used to verify the diagnosis of patients living in the onchocerciasis hyperendemic region of Liberia found to have O.v. negative skin snips. The first was based on a histological and an electronmicroscopic examination of the skin to determine whether the characteristic onchocerciasis dermatitis was absent, and to compare the results with age matched subjects having minimal to densely O.v. mf infested skin. The second method involved the skin response of patients with O.v. mf negative skin snips to topically applied DEC. No O.v. mf were found in skin snips serially sectioned and examined histologically from patients in which the results of incubation studies were completely negative. Skin snips from these seemingly O.v. mf negative patients had a dermatitis, whereas no dermatitis was found in skin snips taken from subjects living in nearby nonendemic areas. Patients with O.v. mf positive skin snips responded to DEC applied topically with increased

and a papular rash whose severity was proportionate to the intensity of

Symptoms are often minimal or vague, yet many such infections carry significant long-term morbidity. Enterobius vermicularis, the common pinworm, is usually manifested in children as anal

and is best diagnosed by using the cellophane-tape test to recover eggs from the perianal skin. Treatment with mebendazole is indicated for the entire family. Trichuris, or whipworm, infection may be found with Ascaris infection, but whipworms are longer-lived and more likely to produce symptoms that mimic those of inflammatory bowel disease. Treatment for these parasites is mebendazole. Other significant helminthic infections, which are currently more common in immigrant populations, include hookworms (Necator americanus, Ancylostoma duodenale), threadworms (Strongyloides) and various tapeworms (Taenia, Diphyllobothrium and Hymenolepis species).

A questionnaire survey of head lice treatment was conducted in four schools--each including a nursery and an elementary school--in the Bordeaux area. Two schools were situated in the centre of the city, one in a suburban area and one in a rural area (50 km from the city). Four-page questionnaires were filled in anonymously by the parents in April 1991; 840 answers were obtained (80 p. 100 response rate). Between January 1990 and March 1991, 48.7 p. 100 of children had at least one episode of head lice infestation (

rates varied from 38.8 to 62.6 p. 100 depending on the schools); 30.5 p. 100 of children were contaminated for the first time during that period. Lice were detected by the parents in 95 p. 100 of the cases. The prevalence of lice was higher in females (60 p. 100) than in males (40 p. 100). The highest prevalence was noted in the suburban school where 17 p. 100 of the parents were unemployed at the time of the survey. The peak age for head lice was 7, but 19.4 p. 100 of nursery school children aged 2-4 years had been contaminated at least once. Impetigo was rare (1.2 p. 100), and

Delusions of parasitosis (DOP) is a psychiatric disorder in which people have an unshakable false belief that they are infested with arthropod parasites. To rid themselves of the "bugs" patients with DOP may become desperate and quit their jobs, burn furniture, abandon homes, and use pesticides dangerously and repeatedly. Data from 11 cases of DOP, reported to the Mississippi Department of Health between 1989 and 1993, are presented in an attempt to identify common factors among the cases. Seven of the 11 patients (64%) were elderly white women. Five patients complained of "mites," 3 of "insects," and 3 of "bugs"; and all characteristically offered "proof" of their
infestation
such as bits of debris or skin scrapings wrapped in paper or in tiny jars. Only one complained of

. Six patients reported extensive use of pesticides. These results elucidate the clinical presentation of DOP cases, thus aiding in diagnosis of the disorder.

This article describes an outbreak of scabies on a 30-bed acute spinal cord injury unit and the measures taken to eradicate the

. Because spinal cord injury patients have sensorimotor deficits, the sensation of

often is absent, making an

of scabies more difficult to identify. A total of 30 patients and 38 employees were exposed and treated for scabies. Educational programs about scabies were instituted immediately for all employees. Routine daily activities and therapy schedules were severely hampered by the outbreak. Despite the efforts of staff to solve the problems associated with the outbreak, frustration and anger were sometimes displayed by many of the staff, patients, and families because of the difficulties they encountered. The outbreak was halted, however, and no further cases were reported, although several employees had to be treated twice.
