Inflammatory linear verrucous epidermal nevus (ILVEN) is a type of epidermal nevus with the onset usually at early childhood. It is generally rather persistent with pruritus as a prominent symptom and is resistant to various treatments. We report a case of disfiguring ILVEN of the vulvar area, which was treated successfully with low fluence CO2 laser. The patient remains improved 2 years following laser therapy.

Inflammatory linear verrucous epidermal nevus (ILVEN) is an unusual unilateral eruption with onset usually in infancy or childhood, female predominance, frequent left leg involvement, pruritus, refractoriness to therapy, and a psoriasiform histologic pattern. We review this disorder in describing a 5-year-old girl who also had melanodontia, an association not previously observed to our knowledge.

Inflammatory linear verrucous epidermal nevus (ILVEN) is a pruritic, erythematous scaly epidermal nevus which follows a Blaschko's lines. Lichen striatus, linear Darier disease, linear porokeratosis, linear lichen planus, linear psoriasis, and the verrucous stage of incontinentia pigmenti may all have similar clinical presentations as the linear verrucous epidermal nevus. ILVEN can be distinguished from true nevoid psoriasis by pruritus and lack of response to antipsoriatic treatments. Various therapeutic modalities have been described, but no one therapy has been successful consistently. Though giant ILVEN is a relative contraindication to surgical excision, here we report a case showing effectiveness of full thickness excision and skin grafting for this condition.

Inflammatory linear verrucous epidermal nevus (ILVEN) is a rare cutaneous disease that presents as linear psoriasiform plaques with associated prominent pruritus. The lesions commonly pre-sent on the legs with onset during childhood. Inflammatory linear verrucous epidermal nevus typically is refractory to treatment. Therapies range from topical treatments to lasers and surgical options. It is clinically and histopathologically similar to psoriasis, suggesting it may respond to established psoriasis treatments such as the excimer laser. We report the case of an otherwise healthy 20-year-old woman with dry, pruritic, red lesions on the right leg that had been present since infancy. Biopsy revealed psoriasiform hyperplasia with a verruciform surface. Multiple topical treatments including ablative CO2 laser therapy showed no remarkable improvement. The patient was then treated with a UV 308-nm excimer laser and showed noticeable clinical improvement. Because of its clinical and histopathological similarities to psoriasis, we hypothesized that the excimer laser may be useful in the treatment of these lesions.
