On the basis of the assumption that Pityrosporum ovale is an important pathogenic factor in the aetiology of Pityriasis simplex capitis (dandruff), the Authors intended to evaluate the efficacy and tolerability of econazole (1% hair-shampoo) in the treatment of this scalp disease. Dandruff could be considered a problem more from a cosmetic than from a medical point of view; for this reason a conventional anti-dandruff hair-shampoo containing zinc-pyrithione was selected as a control. In this open controlled study, seventy patients suffering from different forms of Pityriasis simplex capitis were enrolled at the 1th Dermatological Institute, University of Milan, and randomly assigned to the econazole and zinc-pyrithione treatment groups, respectively. At the end of the four-weeks treatment (two applications/week) a marked decrease in signs (scaling, seborrhea, erythema) and symptoms (burning, itching) was observed in both groups. Regarding the antifungal efficacy, econazole hair-shampoo was slightly better than control. General evaluation on tolerability and cosmetical patterns was favourable for both formulations.

We performed clinical trials to evaluate the therapeutic effects of Foltene in patients of the several types of hair fallings. Thirty patients with male pattern baldness, alopecia areata and seborrheic alopecia were included in this study. Foltene was applied every other day for 40 days, and followed by maintenance therapy of twice application a week. The duration of whole therapy was 6 months. We conclude that Foltene is an effective and agent for male pattern baldness, alopecia areata and seborrheic alopecia from the following results. Ten patients with male pattern baldness was treated with Foltene for 6 months. Foltene had therapeutic effects of 50% in hair regrowth, 70% in decreased hair falls, 30% in decreased dandruff, 50% in decreased seborrhea. Thirteen patients with alopecia areata was treated with Foltene for 6 months. Foltene had therapeutic effects of 61.6% in hair regrowth, 53.9% in decreased in hair falls, 53.9% in decreased dandruff, 77.0% in decreased seborrhea. Seven patients with seborrheic alopecia was treatment with Foltene for 6 months. Foltene had therapeutic effects of 85.8% in hair regrowth, 57.2% in decreased hair falls, 42.9% in decreased dandruff, 85.8% in decreased seborrhea. The degree of therapeutic success was related to the duration of therapy. The side effects were as followed: itching sensation developed in 2 patients (6.7%); tingling sensation in 3 patients (10.0%); burning sensation in 1 patient (3.3%); erythema in 3 patients (10.0%).

A randomized double-blind placebo-controlled cross-over study was made of ketoconazole 200 mg daily in nineteen patients with seborrhoeic dermatitis. All had scalp lesions and sixteen had seborrhoeic dermatitis at other sites. Responses were measured by clinician and patients independently, using a linear analogue scale. Body and scalp lesions and itch regressed considerably and significantly with ketoconazole in all but five patients, three of whom subsequently responded to a higher dose. The patients studied with seborrhoeic dermatitis had been sent by their family doctors in answer to a request for patients with dandruff, and the clinical difference between the two was found to be only of degree. Three patients with dandruff without erythema were studied separately using the same study design: all three responded similarly to those with seborrhoeic eczema. It is concluded that Pityrosporum yeast infection is the immediate cause of seborrhoeic dermatitis and that dandruff is its mildest manifestation.

An alkaline whole coal tar shampoo has been clinically re-evaluated for its therapeutic and cosmetic properties. Its efficacy, as anticipated, is confirmed in psoriasis, dandruff, seborrheic dermatitis, and pruritus. Scanning electron microscopy reveals the whole coal tar shampoo's ability to repair hair similar to protein cosmetic shampoos. Body, luster, and manageability improved throughout this eight week study. A new shampoo action was recorded, described as "corrective;" both oily-haired and dry-haired persons simultaneously exhibit substantial improvement towards the norm. It is postulated that the alkaline-whole coal tar shampoo stimulates natural corrective mechanisms.

A compound having structures of both vitamins E and C, L-ascorbic acid dl-alpha-tocopherol phosphoric acid diester potassium salt (EPC-K), which was proven to have both anti-oxidative3%!d moisturizing effects, has been formulated in the quasi drug hair growing products to mainly prevent dandruff and itching which may be one of the causes for the hair loss. Stabilities of EPC-K in 75% ethanolic solutions with various pHs (2-10) were examined extensively by storing them at 50 degrees C for 30 d and by sunlight exposure for 30 d. Decomposition of EPC-K was only observed under pH 2 at a level of 50% and under sunlight exposure at 25% level. A main decomposition product was identified as tocopheryl phosphate (EP), suggesting that a decomposition route was through hydrolysis. EPC-K was found to decompose by as much as 20% under 30 d storage at 50 degrees C when the concentration of the aqueous ethanolic solution was 0-30%. 1H, 13C and 31P-NMR studies in addition to a micelle formation test using pyrene fluorescent probe revealed that EPC-K formed a micelle at such low concentration of ethanol, which was assumed to be a cause for unstableness of EPC-K in that range. Hydrolytic decomposition of EPC-K was found by the reaction rate study to be a pseudo first order reaction with activation energy of 16.98 kcal/mol.

An adult domestic short-hair cat from south Texas was examined because of excessive dandruff on the back, neck, thorax, and hind limbs. Removal of a few hairs for microscopic evaluation revealed Lynxacarus radovskyi, the cat fur mite. The small (< 0.5 mm) mite could be readily identified by its laterally compressed body and its characteristic grasping of the hair shaft between the gnathosoma and palpi. Thus far, this mite has been identified as a parasite of cats in warm, humid environments. The number of parasites and apparent discomfort in cats varies considerably, from massive infestation with little discomfort to few mites and marked pruritus. Acaricides that are effective against other ectoparasites of cats apparently are effective in controlling L. radovskyi.

A clinical case of Cheyletiella infestation on a dog born and raised in Korea is reported. A three-year old female Whippet was hospitalized due to a multiple fracture and displacement of the left scapula caused by a recent car accident. The mite infestation was not noticed at the time of hospitalization. The dog underwent multiple operations involving internal fixation of the fractured scapula with wire and a plate, followed by extensive chemotherapy with antibiotics and prednisolone. After two months of hospitalization, a pruritic dermatitis near the left scapula developed. Multiple white dandruff-like flakes were seen on the hair coat, especially over the dorsal spine and neck, and the dog expressed increased pruritus by frequently licking and scratching the affected areas. Local dense accumulations of skin debris that became crusty were also observed. Microscopic examination of a skin scraping revealed a heavy infestation of cheyletiella yasguri, as identified by the presence of hoks of the palpi and the heart-shaped sensory organ on genu I. Immunosuppression elicited by the extensive administration of prednisolone was suspected for the initiation of the generalized mite infestation.

A postmarketing study was conducted on 236 patients from 23 centres suffering from moderate to severe dandruff with a combination of ketoconazole and zinc pyrithione (1%) for a duration of 4 weeks with 2 weeks further follow-up. Scoring of dandruff was done on a 0-10 scale for each of the 6 regions of scalp at each week up to 6 weeks. The results indicate that there was a consistent improvement in dandruff scores over the treatment period and a reduction of > 90% was seen for all areas of scalp individually as well as collectively as compared to baseline. The treatment also showed significant improvement in other signs and symptoms such as erythema and itching, with a highly favourable adverse event profile. The overall assessment for global improvement by investigators showed good-excellent results with high acceptability amongst the patient population for the treatment. A combination shampoo of ketoconazole (2%) and zinc pyrithione (1%) offers a safe and effective option in the treatment of dandruff.

Honey has antibacterial, antifungal and antioxidants activities and has high nutrient value. In this study we investigated the potential use of topical application of crude honey in the management of seborrheic dermatitis and dandruff. Thirty patients with chronic seborrheic dermatitis of scalp, face and front of chest were entered for study. Twenty patients were males and 10 were females, their ages ranged between 15 and 60 years. The patients had scaling, itching and hair loss. The lesions were scaling macules, papules and dry white plaques with crust and fissures. The patients were asked to apply diluted crude honey (90% honey diluted in warm water) every other day on the lesions with gentle rubbing for 2-3 mins. Honey was left for 3 hr before gentle rinsing with warm water. The patients were followed daily for itching, scaling, hair loss and the lesions were examined. Treatment was continued for 4 weeks. The improved patients were included in a prophylactic phase, lasting six months. Half patients were treated with the topical honey once weekly and the other half served as control. All the patients responded markedly with application of honey. Itching was relieved and scaling was disappeared within one week. Skin lesions were healed and disappeared completely within 2 weeks. In addition, patients showed subjective improvement in hair loss. None of the patients ( 15 patients) treated with honey application once weekly for six months showed relapse while the 12/15 patients who had no prophylactic treatment with honey experienced a relapse of the lesions 2-4 months after stopping treatment. It might be concluded that crude honey could markedly improve seborrheic dermatitis and associated hair loss and prevent relapse when applied weekly.

Pityrosporum ovale appears to play an important role in the pathogenesis of dandruff as a symptom of seborrheic dermatitis. Climbazole is an antimycotic agent with a high in vitro and in vivo efficacy against P. ovale. In the presented work we investigated the efficacy and safety of a climbazole 0.65% shampoo on seborrheic dermatitis of 30 volunteers. Subjects were diagnosed as having moderate to severe seborrheic dermatitis of the scalp. After a 1-week washout and a 4-week treatment the clinical evaluation showed a successful reduction of dandruff, skin redness and itching in 80% of the volunteers and a mild improvement in 20% of the volunteers. The cosmetic acceptability was very good by the majority. It is concluded that the formulation tested is effective in the treatment of moderate to severe dandruff.
