Seventeen patients with acute vaginal candidosis were treated for 2 days with itraconazole (200 mg daily). During 5 days after the start of the treatment, patients were daily screened for the presence of Candida (culture and microscopy) and the presence of signs and symptoms. Four days after treatment, no Candida could be detected in smears of vaginal fluid and cultures from these patients. At that time leucorrhoea,
pruritus
,
vulvitis
, and erythema were strongly reduced but had not completely disappeared.

Budesonide-containing Apulein ointment has been used for the treatment of 29 women suffering from
vulvitis
,
pruritus
vulvae caused by vaginal discharge, gestational inflamed hemorrhoid, hymenitis, and late postoperative suture inflammation. The ointment rapidly and completely controlled local infiltration and the accompanying subjective complaints. It did not cause local alteration, the consistency of the ointment met the requirements and it did not disturb the patients' making their toilet. According to the author's opinion the product is a potent antiphlogistic which proved to be a valuable adjuvant to target specific treatment of inflammatory processes of the external female genital organs and perianal regions.

The vast majority of prepubertal gynecologic problems fall into three categories. The most serious category includes those "things that bleed": sarcoma botryoides, trauma (including sexual abuse), vaginal foreign objects, condylomata, urethral prolapse, and single organism vaginitis. Very rarely, the clinician will see precocious menarche, metastatic Crohn's disease, vascular vulvar lesions, and factitious cases. The next category contains entities that have an abnormal appearance: ambiguous genitalia, periurethral cysts of the newborn, hymenal variants, and agglutination of the labia and vulva. Rarely, an underlying skin disorder such as lichen sclerosus, seborrhea, or atopic
vulvitis
will be seen. The last and most bothersome category, distinguished by its symptoms of
pruritus
and discharge, includes the most common types of vulvovaginitis.

A randomized double-blind trial was carried out with itraconazole versus placebo in the treatment of vaginal candidiasis, confirmed by clinical evaluation, direct microscopic examination and Sabouraud culture. Fifty patients were studied, 25 in the itraconazole group and 25 in the placebo group. Both groups received two capsules once daily (100 mg itraconazole/cap) for 3 days. One week after treatment patients were re-evaluated according to the same parameters as in selection. The scores for clinical symptoms, leukorrhea, vulvar
pruritus
, vaginitis and
vulvitis
, were compared in both groups before and after treatment. Statistically significant differences were found for the itraconazole group in
pruritus
and vaginitis (P less than 0.05) and
vulvitis
(P less than 0.001), with no significant difference for leukorrhea. As to the mycological evaluation, 7 days after treatment there were negative results for the itraconazole group in 92% of the patients in comparison to 52% in the placebo group (chi-square, P = 0.005).

500 patients of a dispensaire of diabetes were inspected dermatologically. 335 patients had a duration of the glucose metabolism intoleration less than 10 years and 165 patients more than 10 years. 215 patients stated a striking family case history according diabetes. 54 per cent (273) of the cases showed an adiposity, 418 patients (83.6 percent) had pathological changes of the skin and mucosal changes. It is clear that dermatomycoses, eczema and pyodermias occur more often with acute metabolic disorder and adiposity. Neurovascular changes of the skin occur with long persistent glucose metabolism intoleration. Bad healing tendencles of wounds,
pruritus
, intensive dermatomycoses, balanitis,
vulvitis
, furunculosis and pyodermias are important references to a glucose metabolism intoleration. They should give rise to look for a diabetes irrespective of the age of the patient.

Analyses of the possible presence of human papilloma virus (HPV) DNA were performed on 84 biopsies from 50 women with clinically and histopathologically suspected HPV infection of the vulva. The colposcopic criteria for inclusion in the study were: acetowhite flat lesions with or without fissures and/or diffuse hyperplastic mucosa with a granulated or filamental surface. Directed biopsies for histopathology and DNA hybridization (dot-blot and Southern-blot) were taken. All women had histopathological evidence of HPV infection. Twelve women had cervical and/or vulvar neoplasia. HPV DNA could be demonstrated in the vulvar biopsies from 24 women; 13 women had HPV 16, 3 had HPV 18, HPV 31 and HPV 33, respectively. In addition, 10 biopsies contained HPV DNA of unknown types. Fifty-five percent of granulated and filamental lesions and 43% of flat lesions with or without fissures were positive for HPV DNA. 34 women had one or more of the following symptoms:
itching
, burning, dyspareunia, discharge, fissures, dryness and dysuria. Papillomavirus
vulvitis
is a sexually transmitted disease which may cause recalcitrant symptoms and/or concomitant neoplasia. It is thus important to recognize the different symptoms and signs of this disease.

We screened 392 women attending the Laboratory of Microbiology of Toulouse Purpan Hospital for symptoms of lower genital tract infection for six microorganisms. Rates of isolation of Neisseria gonorrhoeae, Chlamydia trachomatis, Candida albicans, Mycoplasma hominis, and Gardnerella vaginalis were 1%, 7.7%, 29.8%, 23.8%, and 37.5%, respectively. Trichomonas vaginalis was detected by direct examination in specimens from 3.1% of patients. Another goal of this study was to determine the relative prevalence of these microorganisms in relation to clinical and epidemiologic findings. C. trachomatis was isolated significantly more often from patients under 25 year old (P less than 0.001), from those who use oral contraceptives (P less than 0.01), from those who have a male sexual partner with symptoms of sexually transmitted disease (P less than 0.001), and from those who present signs of cervicitis (P less than 0.05). Isolation of C. albicans was significantly associated with vulvar
pruritus
(P less than 0.01),
vulvitis
(P less than 0.01), and vaginitis (P less than 0.001). G. vaginalis and M. hominis are significantly associated with bacterial vaginosis (P less than 0.001). This syndrome was diagnosed clinically in 27.2% of our study population and was highly correlated with use of an intrauterine device (P less than 0.05).

Condom sales rose in the 1980s because condoms protect against the spread of sexually transmitted diseases and AIDS. In anticipation of corresponding increase in the incidence of condom dermatitis, a dermatologist prepared a brief overview of the condition and ways to treat it to prepare health professional for the anticipated increase. In men, allergic reactions may consist of edema of the penis, especially the prepuce; penile
itching
; and eczematous dermatitis which extends to the scrotum, inguinal areas, and the inner thighs. Symptoms in women vary and may include pubic and groin dermatitis,
vulvitis
,
pruritus
vulvae, a burning vaginal sensation, vulval redness and edema, and eczema on nearby skin. Antioxidants or accelerators in the rubber are generally responsible for condom dermatitis. When a health professional believes the dermatitis to be allergic rubber condom dermatitis, he/she should request patch tests for the same make of condom that came in contact with the patient and for rubber chemicals known to induce an allergic reaction, e.g., mercaptobenzothiazole (MBT). To assist, the dermatologist includes a patch test series for such allergens. Natural latex condoms and condoms made of processed sheep intestine cecum may prevent most allergic reactions. Sometimes patients are allergic to condom lubricants, and not the rubber itself. Health professionals must be sure to inform dermatitis patients allergic to the lubricant to not switch to oils or petroleum jelly because they destroy the rubber. Spermicidal contraceptive agents also can cause contact dermatitis, particularly nonoxynol 9. A condom with corn starch can produce urticaria. Further, paralyzed male patients are especially prone to allergic dermatitis from either the rubber in the condom urinals and/or the medical adhesive.

We report six cases of plasma cell balanitis and four of plasma cell
vulvitis
. The patients' ages ranged from 26-88 years in males and 30-60 in females. All the men were uncircumcised and presented with a discharge and erythematous lesions on the glans penis and prepuce. In females the main presenting complaints were vulvar soreness,
pruritus
, smarting/burning, discharge and bleeding. Two were on hormone replacement therapy, two had undergone a mastectomy for carcinoma of the breast, and one was an insulin-dependent diabetic. Another patient subsequently developed diabetes mellitus. Topical and intralesional corticosteroid preparations resulted in satisfactory improvements in all cases, male and female. Two males subsequently underwent circumcision, which was curative.

Four cases of
vulvitis
circumscripta plasmacellularis (plasma cell
vulvitis
) are presented. One case was associated with cutaneous lupus erythematosus and another with a history of desquamative vaginitis. Two patients were postmenopausal, and two were premenopausal. The presenting symptoms were
pruritus
, tenderness, superficial dyspareunia and vulvar dysuria. The lesions were situated in the introitus in three patients and on the lateral aspect of the labium minus in the fourth and appeared as well-circumscribed, glistening, erythematous patches with a faint orange hue. Histologically, epidermal edema and inflammation, a dense upper dermal band of chronic inflammatory cells, including many plasma cells, dilated capillaries, extravasated red blood cells and hemosiderin deposition, were seen. There was a variable response to local steroid therapy, but one of the postmenopausal patients responded to local estrogen alone. The term
vulvitis
circumscripta plasmacellularis is useful to describe an idiopathic form of erosive
vulvitis
with a characteristic clinical and histologic appearance.
