Scabies is a relatively common
skin disease
caused by the
itch
mite Sarcoptes scabiei var. hominis. Because of its strong contagious capacity, social problems especially as sexual transmission or massive infection in school children arise quite frequently. Since we have experienced many scabies patients in our outpatient clinic of Urakawa Red Cross Hospital, statistical analysis was attempted on patients from April 1976 to March 1987, in order to contribute to prevention and control of the disease. Results have been obtained as follows: (1) The total number of scabies patients was 496 that is 1.96% of the total number of new outpatients seen in this period. (2) Sex ratio was 1.33 (male): 1 (female), slightly higher in male. (3) As to the age distribution, the highest peak was 3rd decade of life. (4) Compared with previous reports, the ratio of children and elders was remarkably high. (5) Massive infection was observed in one nursing home and one hospital. (6) As to the seasonal distribution, the number of patients was high from October to March, and the highest peak was November. (7) We compared three kinds of scabies and discussed about the useful effect of gamma-BHC in the cure and prevention of the disease, comparing with crotamiton and benzyl benzoate.

The present paper deals with skin lesions on the unprotected parts of the body of a 27-year-old woman which has been attacked by blood-sucking insects while staying in the Leningrad region at the end of May when the air temperature varied within 15-30 degrees C range. High biting activity of the insects resulted in the development of skin allergies. The first signs of inflammation after biting included pronounced exudative processes followed by the formation of bullous elements secreting serous fluid. Persistant time-course of epizoonosis as well as resistance to all kinds of general and local treatment resulted in the allergic contact
dermatosis
featuring nodular formations, especially on the skin, and solid papulous elements, accompanied by
pruritus
. Such persistant duration of the disease may be due, probably, to previous allergic background, resulting from her parents' allergic diseases. Identical clinical patterns of epizoonosis may be induced by the bites of various blood-suckers (sandflies, mosquitoes etc) while the human organism's response may depend upon its general state of health and immunological reactivity.

A series of 72 women with polymorphic light eruption (PLE) seen between 1970-1988 was reviewed with regard to onset of the disorder and timing of oral contraceptive use and pregnancy. PLE is a light-induced
dermatosis
consisting of
itching
erythematous papular, plaque-like or eczematous skin lesions on exposed skin. It occurs mainly in women--79% in this series. Appropriate tests were performed to rule out systemic lupus erythematosus and porphyria. The mean age of onset was 25 years. 63.9% had used oral contraceptives during their disease. PLE and pill use was correlated in 15.3% of patients. No consistent relationship was found between PLE episodes and starting or stopping pills. Eruptions developed in 3 women during pregnancy; they worsened during pregnancy in 2; and resolved during pregnancy in 1 woman.

Neurodermatitis atopica is a
skin disease
caused by various different factors and which is characterized by the simultaneous presence of different clinical features. The treatment is directed towards the various aetiopathological factors and oriented on the clinical picture. The main symptom is dry skin, which has to be rehydrated and provided with an artificial film of grease. Dermosteroids are the most efficient antiinflammatory agents, but they can only be used as one of various components of a complete neurodermatitis therapy. New knowledge gained over the past few years has shown the important role of Staphylococcus aureus in this condition, and as a result antibiotic therapy has increased in importance. The use of soap, which was previously not recommended, is now considered to be a useful and beneficial addition to the treatment, thanks to the availability of new alkali-free, disinfectant soaps. In certain cases there is a causal connection with exposure to allergens. Besides treatment of the

, which is crucial for the patient, stabilization of the psychological factor and, if necessary, changes in the patient's environmental situation are extremely important. Substitution of essential fatty acids and UV therapy are discussed.

We reviewed 82 cases of erythroderma, including clinical, laboratory, and biopsy material. The diseases most commonly associated with erythroderma were drug eruptions (34%), preexisting dermatoses (30%), and lymphoreticular neoplasms (20%), especially cutaneous T cell lymphomas (CTCL). The prognosis was related to the associated process; all deaths occurred in the drug or lymphoma groups. The clinical presentation of erythroderma was fairly uniform, with malaise (34%),
pruritus
(36%), and a subjective chilly sensation (34%) the most common symptoms. Keratoderma of the palms and soles (37%), mild generalized adenopathy (26%), and hepatomegaly (9%) were also common. Splenomegaly was rare (1%). Skin biopsy revealed the cause in 43% of the patients, but lymph node biopsy was not generally helpful in the initial evaluation. Although numerous laboratory values were abnormal, most findings were nondiagnostic and were related to the inflammatory process. A major exception was the Sezary cell preparation, which was selective for CTCL. A previously undiagnosed chronic erythroderma in a patient with no underlying
skin disease
may be the initial manifestation of CTCL. Thus, repeated evaluations and close follow-up are recommended.

Infantile seborrhoeic dermatitis is a
dermatosis
affecting infants characterized by little or no
itching
and by completely typical skin symptoms, distribution and evolution. Infantile seborrhoeic dermatitis should therefore be differentiated from atopic dermatitis and from diaper-rash although interferences be possible. In fact the high frequency of the atopic dermatitis and of diaper rash may cause these two manifestations to sometimes superimpose on infantile seborrhoeic dermatitis. This in turn causes rather complex clinical situations that may be very hard, but not impossible to diagnose if use is made of laboratory findings to support clinical features. The pathogenesis and etiology of infantile seborrhoeic dermatitis are still debated although recent results assign the dominant role to Candida albicans. This mycete also plays a relevant role in the genesis of diaper rash and particularly of Leiner's exfoliative erythrodermas, which can be interpreted as a generalized and erythrodermic expression of infantile seborrhoeic dermatitis. Systematic cultural investigations and appropriate antimycotic treatments, that would stimulate antibody response, will allow to clarify in the near future this interesting etiological problem, with important therapeutical consequences.

The prevalence of occupational
skin disease
was found to be 3.8% in a study of 479 sanders in the furniture-making industry in Singapore. 17 species of wood imported from South East Asia were used. The most common dermatoses from wood dust were
pruritus
(1.6%), irritant contact dermatitis (1.6%) and xerosis (1.4%). 2 sanders had miliaria. None had allergic contact dermatitis from wood dust. The arms and trunk were the most common site for
pruritus
and dermatitis from wood dust. It appeared that the woods commonly used in the furniture making industry are weak sensitizers. Appropriate preventive measures against irritant dermatitis such as dust control, protective clothing, and good personal hygiene should be adequate to prevent occupational dermatoses among the sanders.
