Eight patients received PUVA for mastocytosis. Five women had typical adult-onset urticaria pigmentosa, without evidence of systemic disease. Another woman had suspected hepatic involvement while the remaining female had early-onset familial urticaria pigmentosa with morphologically atypical mast cells. The only male patient had cirrhosis with hepatic deposits of mast cells in addition to
polycythaemia rubra vera
. In all patients, except the man with systemic disease, there was reduced
pruritus
and wealing and partial to almost complete fading of the macules. The manifestations of urticaria pigmentosa recurred after treatment was discontinued. In both lesional and uninvolved skin there was no significant change in either the mean mast cell counts or mast cell ultrastructure after an average of twenty-seven PUVA exposures. In addition, PUVA did not cause a significant alteration in the histamine content of the skin. The beneficial effect of PUVA in urticaria pigmentosa therefore does not appear to be directly related to a change in mast cell numbers or morphology, or to the histamine concentration in the skin.

Thirty-four patients with
polycythemia vera
complicated by
pruritus
were treated with 900 mg of cimetidine daily for 30 days and their responses to treatment were evaluated. The conditions of 15 (44%) were improved, with 12 patients stating that
pruritus
completely disappeared. Nineteen patients either showed no improvement or had increasing symptoms. No toxic effects were reported. The positive responses seen are encouraging and suggest that controlled studies are indicated to further evaluate the effectiveness of H2 antagonists.

Psoralen photochemotherapy (PUVA) was effective in the treatment of five patients with aquagenic
pruritus
, associated in one with
polycythaemia rubra vera
and in another with the myelodysplastic syndrome. Relapse occurred within 2-24 weeks when treatment was discontinued. Maintenance therapy or a further course of PUVA was necessary to maintain remission. This requirement may limit the value of the therapy.

To investigate the routes of neural spread of pseudorabies (Aujeszky's disease) virus (
PRV
), the effects of chemical sympathectomy by 6-hydroxydopamine (6-OHDA) on clinical signs and viral spread in mice inoculated intraocularly with
PRV
were examined. Similar to non-treated mice, the treated mice developed
pruritus
as a major clinical sign, followed by peracute death, but the time of death tended to be slightly delayed in about half of the treated mice. On immunohistological examination, viral antigens in treated mice were found to be markedly reduced in all the ipsilateral retinae; they were detected diffusely in the forebrain area with a localization in the mammillary area. In the treated mice, viral antigens were also reduced in the ipsilateral trigeminal nerve ganglia as well as in its central nuclei. These findings indicate that both the sympathetic nervous system and the trigeminal nervous system play an important role in the neural spread of
PRV
. Possible involvement of the dopaminergic nervous system, particularly in the eye, as the main site of viral growth was discussed.

Aquagenic pruritus has been reported in association with a variety of underlying disorders including
polycythaemia rubra vera
, myelodysplastic syndrome, the hypereosinophilic syndrome and juvenile xanthogranuloma. However, in most cases the aetiology is unknown. The onset of intractable aquagenic
pruritus
is reported in a fit 71-year-old lady; however, 10 months later she was found to have hepatic metastases arising from a squamous cell carcinoma of the cervix treated 15 years previously. The occurrence of these two relatively rare conditions is likely to be coincidental. None the less, the close temporal relationship between the onset of aquagenic
pruritus
and the hepatic metastases raises the possibility of a true association. Aquagenic pruritus may be associated with a wider variety of underlying disorders than previously reported. Intractable symptoms of recent onset in a patient with a past history of cancer should be investigated.

The mechanism of water-induced

is unknown. Evidence has been presented previously that bathing or showering may trigger mast cell degranulation and that release of a mediator by mast cells may be responsible for the
pruritus
. Tryptase is a specific marker of human mast cell secretory granules and its presence in body fluids indicates mast cell degranulation. In this study, serum tryptase levels were measured both before and one hour after showering in 11 patients suffering from
polycythemia vera
and water-induced
pruritus
. Tryptase was not found in the serum of any of the subjects one hour after showering, when levels would be expected to be near peak had significant mast cell degranulation occurred. These results argue against mass cell degranulation with systemic release of a mast cell product as the mechanism for water-induced

Following reports that recombinant interferon alfa (rIFN alpha), besides inducing clinical and hematologic remission in
polycythemia vera
(PV), can also resolve intractable
pruritus
, we used rIFN alpha to treat 13 PV patients complaining of severe
pruritus
refractory to conventional treatment (venesection and/or cytostatics). rIFN alpha was administered intramuscularly three times a week at a dosage of 3.0 x 10(6) U. Eight patients (61.5%) reported a > or = 50% reduction of
pruritus
, which occurred within 2-8 weeks from the start of rIFN alpha treatment, leading to a substantial improvement in their quality of life. Three patients had to stop rIFN alpha within the first month of therapy because of unacceptable side effects. Thus rIFN alpha seems to be capable of providing considerable relief of otherwise intractable
pruritus
in a good proportion of PV patients.

without primary skin manifestations may be the presenting symptom of serious internal diseases. Five characteristic cases of
pruritus
are discussed: Hodgkin's disease, primary sclerosing cholangitis,
polycythemia vera
, iron deficiency (with pica), and uremia. Other important causes must be considered; all forms of cholestasis, including primary biliary cirrhosis, drug-induced, pregnancy-related, and extrahepatic cholestasis; other hematologic and malignant disorders such as non-Hodgkin's lymphoma, leukemia, multiple myeloma, solid tumors, and myelodysplastic syndromes; metabolic and endocrine diseases, most notably diabetes mellitus, hyperthyroidism, hypothyroidism, and carcinoid syndrome; focal neurologic diseases such as brain tumors, cerebral infarctions and multiple sclerosis; adverse drug reactions without rash; infectious diseases, especially parasitic and HIV infections. A diagnostic laboratory screening for

of undetermined origin is suggested.

The efficacy and tolerability of rIFN-alpha has been evaluated in 17 selected patients with symptomatic polycythaemia vera, diagnosed according to the
PRV
Study Group criteria. Complete disease control (CR) was achieved, after 1-12 months, in nine patients, with partial control in a further five cases. Three patients failed to respond.
Pruritus
significantly improved in 83% (10/12) of cases, following 1-28 weeks of treatment. Six patients (35%), however, were unable to tolerate rIFN-alpha, on account of weight loss, myalgia and mental changes. Overall, alpha-interferon therapy significantly improved venesection requirements, MCV and PCV values, platelet counts,
pruritus
scores and the degree of splenomegaly. Analysis of pooled published data (100 evaluable patients, including the present study) revealed an overall CR of 60%, a PR of 27%, and a failure rate of 13%. Significant
pruritus
control (> 50% improvement) occurred in 77% of cases. rIFN-alpha appears to be an effective therapy for PV-associated myeloproliferation and/or
pruritus
, although side-effects remain a concern. Long-term studies are now indicated to determine if the natural history of the disease is altered, in particular whether the incidence of myelofibrosis and/or leukaemic transformation is reduced.
