In 18 atopic eczema patients with active symptomatology and 15 control subjects a comparison of reactivity to psychological stressors was made. The physiological measures were heart rate, peripheral vasomotor response, skin resistance level, spontaneous fluctuations of the skin resistance, and forearm skin temperature. In addition, self-ratings of subjective state, situational anxiety, and social anxiety were assessed. Mental arithmetic performed undisturbed and in distracting conditions, and anticipation of the latter and of having to speak in public served as stressors. The results show higher reaction values of the
eczema
patients for heart rate, peripheral vasomotor response, fluctuations of skin resistance and subjective tension. A subgroup of patients with extreme skin irritation (
itching
) reacted with an elevation of skin temperature, in contrast to control subjects and patients with less marked
itching
, in whom skin temperature dropped slightly. The results suggest that atopic eczema may have a psychophysiological component.

Patients suffering from atopic dermatitis show increased phosphodiesterase activity in leucocytes. As a result, the level of intracellular c-AMP is reduced, which can produce a number of metabolic dysfunctions: We found strong evidence that an enhanced release of several inflammation mediators can favor
itching
, which in turn promotes the development of
eczema
, since it induces reflectory scratching. In order to solve the question whether papaverine as a phosphodiesterase inhibitor might offer a new approach in the systemic therapy of
itching
, we performed a placebo-controlled study on 51 patients with atopic dermatitis, who were orally treated with papaverin-HCl over 8 weeks. However, we did not observe any improvement of
itching
or
eczema
during treatment. Some other recently developed phosphodiesterase inhibitors are discussed.

Skin infection is common in patients with any skin diseases where
pruritus
is a prominent feature. A retrospective analysis was performed on the results of skin swab cultures from patients with
eczema
and a variety of other conditions. This paper presents the findings of bacteriologic culture and sensitivity testing of 131 swabs from 122 patients over a period of 22 months and examines the results in terms of the reported relationship between scabies and acute poststreptococcal glomerulonephritis. Staphylococcus aureus was most often isolated, and this appears to indicate that antistaphylococcal antibiotics should be the first line of treatment in the absence of the results of cultures.

The prevalence of skin diseases in elderly and senile patients was analyzed basing on the consultation rate records. Allergic and
itching
dermatoses (dermatitis,
eczema
, neurodermatitis, urticaria, skin
itching
, prurigo) rank first in this patient population; their share in the total incidence of skin diseases has made up 49.4%. These data call for thorough studies of this group of dermatoses in elderly and senile patients.

A case of lymphonodular hyperplasia as a cause for lower gastrointestinal bleeding in a 6 year-old child is reported. Bleeding associated with peribuccal
eczema
and buccal
itching
after eating some foods, all regressing on restricted diets, suggest the possible role of a food allergy. Digestive and cutaneous signs improved with oral sodium cromoglycate associated with restricted diet.

Fifteen cases of otomycosis in 11 women and 4 men attending the Clinic Hospital in Salamanca during the period from 1984 to 1988 are discussed. A. niger was the species isolated on the majority of occasions (73.3%). The commonest form of presentation of otomycosis was simple chronic (73.3%). The clinical pictures was characterized by serous secretion and
pruritus
. Otomycosis with associated bacterial infection (20.0%) was the cause of the most intense symptoms, featuring suppuration and hypoacusia. In 5 patients, local antecedents of interest were observed: three cases of chronic cholesteatomatose otitis in which surgery had been performed; retroauricular
eczema
, and perichondritis of the ear. Despite topical treatment with clotrimazole, 4 cases of relapse occurred (26.7%) and in none of these was associated bacterial infection observed.

The presenting features of 236 thyrotoxic patients seen in the thyroid clinic were reviewed. 18.65% of these patterns had one or more dermatological complaints at presentation. There was no specific difference in this group of patients when compared with the general hyperthyroid population with regard to age, race, sex, duration of hyperthyroidism or biochemical indices of thyrotoxicosis. The two major complaints were
itching
and alopecia. The prevalence of
pruritus
at 6.4% in our series was identical to that of other workers, but we had a much lower occurrence of alopecia at 2.6%. The diagnosis of thyrotoxicosis was delayed in two patients in whom the only major complaint was
pruritus
. These symptoms cleared quickly when these patients became euthyroid. However there were other patients who noted hair loss with anti-thyroid medications. The incidence of vitiligo,
eczema
, onycholysis in our series was much lower those quoted in the Western literature The occurrence of pretibial myoxoedema in our series is similar to that of other workers from this region. The other miscellaneous manifestations include urticaria, xanthelasma and systemic lupus erythematosis. In conclusion we feel the cutaneous manifestations of hyperthyroidism are common in our patients.

Examinations of 518 refinery workers have helped define the dermatologic morbidity structure. Immunologic examinations have been carried out in 82 workers. 70 of these have developed skin abnormalities (toxicoderma,
eczema
,
itching
, hyperkeratosis, vascular spots, abnormal pigmentation, etc.). The studies have revealed reduced levels of the cell-mediated immunity parameters: T-lymphocytes, T-theophylline-sensitive lymphocytes, and decreased functional activities thereof); this has been parallelled by activation of the humoral immunity.

The purpose of the present investigation was to study the nature and frequency of adverse reactions to materials and procedures among orthodontists and their patients. A questionnaire about this topic was mailed to practicing members of the Norwegian Orthodontic Society, of which 137 (about 75%) responded together with 127 chairside assistants. About half of the orthodontic personnel had experienced adverse reactions. The majority of the problems were dermatoses, comprising dryness, redness,
itching
, thickening, reduced tactile sensitivity, fissuring, soreness/desquamation and pain of hands and fingers. The residual were respiratory reactions, eye reactions or reactions of a general nature. Many of the dermatoses were of moderate severity, attributed to (seasonal) air/ventilation associated problems. Other frequent causes were hand washing procedures, work involving composites and acrylics (orthodontists), or work involving model materials, alcoholic disinfectants, latex gloves, alignates etc. (assistants). For both groups the most severe dermatological reactions were associated with unspecified allergies, acrylics and composites. Non-dermatological reactions also reflected the different working pattern and exposure to materials in orthodontics; acrylic monomer topping the list. The orthodontists had observed 425 patients with dermal reactions and 67 patients with intraoral/systemic reactions, indicating a prevalence of about 1 per cent. Dermal reactions included redness,
eczema
,
itching
and fissuring in facial, neck or perioral areas, mostly attributed to metallic parts of extraoral appliances, with some exceptions (elastics, neck pillows, head caps). Intraoral reactions consisted of redness, soreness and swelling of the oral mucosa, gingiva and/or lips and were associated with metal brackets, labial wires, bonding procedures or acrylic appliances.

Protein contact dermatitis, described as "gut eczema" or "fat eczema" by slaughterhouse workers, was first described a decade ago and presents as an
itching
, vescular
eczema
on the hands and fingers, a few hours after contact with animal material, especially gut material. In a cross-sectional study of 144 slaughterhouse workers, a cumulative prevalence of current and anamnestic cases of protein contact dermatitis of 22% was found, with the highest prevalence in workers eviscerating and cleansing gut. The scratch patch test was the only skin test showing positive results in workers with protein contact dermatitis, positive reactions being found, however, in less than half the cases.
