Thirty-seven patients with moderate or severe persistent atopic dermatitis gave a history of skin reactions following the ingestion of certain foods. These reactions fell into three groups: immediate reactions (within one hour) which included angio-oedema, contact urticaria, generalised itching and erythema, or urticaria; late reactions, where patients experienced late urticaria, late angio-oedema, itching, or exacerbation of the dermatitis; dual reactions where immediate reactions were followed by exacerbation of the atopic dermatitis. The foods implicated were either potential allergens or recognised nonimmunological histamine-release agents. Foods which caused an IgE-mediated reaction tended to produce early or dual reactions whereas histamine releasing foods tended to result in late reactions.

Swimmer's dermatitides occurring in natural--accordingly polluted--waters are most probably of chemical nature. In clean waters, however, they are primarily caused by larvae of duck-flukes, the so called cercariae. In a natural pool near Biberach undetermined cases of swimmers' dermatitis have appeared. Hence a cercarial dermatitis was suspected and specific examinations were carried out. At the biological examination of the pool, many watersnails of the species Radix ovata have been found near the shore. Out of 180 collected snails 8 produced cercariae of different trematode species. Hence an infective cycle between duck and snail could be ascertained here. However, the special cercariae, producing dermatitis, could not be found. Having taken that into account the author was standing in the snail biotop without protective clothes for about 1 hour to provoke cercarial dermatitis. A few minutes later the corresponding clinical picture with an extremely strong itching developed. Papules were to be seen 12 h later, the symptoms lasted 2 weeks altogether. By different serological methods (Cercarienhuellenreaction) indirect immunofluorescent test with complete cercariae and cercarial sections antibodies against cercariae could be demonstrated in the serum of the author 14 days later. As a therapy only the symptomatic treatment of the itching is possible. In principle etiologic measures against cercarial dermatitides are possible by chemical snail control. But as a rule this is to be refused because of the severe ecological damages. However, in case of swimmers' dermatitis it is nonetheless indispensible to clarify eventual parasitogenic reasons because of the specific hygienic consequences.

Contact dermatitis form Hippomane mancinella L. (manchineel) is commonly observed in Caribbean and Central American Coast land. Its sap from leaf exudate and from the fruit induce severe dermatitis that may involve oral, ocular, genital mucous membranes and skin, depending on the mediate or direct type of contact. Intense itching or burning sensation occurs within an hour and is followed by painful erythema, vesicles, bullae or pustulae, the latter being highly suggestive of mancheneel. Oropharyngeal and ocular lesions may by very severe. The brief delay and the burnlike appearance of the lesions suggest an orthoergic mechanism due to a caustic that could be similar to the substance identified in an other species of Euphorbiacae as phorbol esters. Traditional and native remedies are presented herein.

A series of 955 persons aged 24-44 years, with atopic dermatitis in childhood, were interviewed in order to identify factors which increase the risk of developing hand eczema in adult life, or aggravate already existing hand eczema. Endogenous (constitutional) factors were in general of greater importance than exogenous factors, viz. chemicals, water, soil and wear (friction). Eczematous involvement of the hands in childhood was of predominant importance. In individuals without such involvement, severe (widespread) dermatitis in childhood was a dominant factor. Other factors, each of them significantly more important than the exogenous ones, were persistent eczema on other parts of the body and dry/itchy skin. The factors female sex, family history of atopic dermatitis and simultaneous bronchial asthma/allergic rhinitis were associated with increased risk of developing hand eczema in adult life, but were of limited importance compared with the other endogenous and the exogenous factors.

Many workers exposed to glass fibers experience an intense pruritus (itching), sometimes accompanied by erythema, erosions, and dermatitis. Patch tests to glass fibers provoked a papulovesicular dermatitis in about 25% of normal persons. Workers in the glass wool industry, whether or not clinically symptomatic, had the same levels of patch-test reactivity. A battery of skin tests failed to identify persons with an increased susceptibility to glass fiber pruritus. Some workers' skin becomes hardened by continuous exposure. This hardening is lost after a one-month holiday but is quickly regained. Barrier creams had no protective value in preventing glass fiber dermatitis.

A study was conducted to determine the effects of an oral contraceptive containing 250 mcg of d-norgestrel with 50 mcg of ethinyl estradiol on lactation and other functions. 134 healthy women of childbearing age who were breast feeding their young completed more than 3 cycles each, for a total of 1377 cycles for the study. The reported side effects were 1) 12 instances of disturbances in menstruation, 2) 4 instances of abdominal distress, 3) 55 instances of weight change, 4) 11 cases of headache and giddiness, and 5) some dermatitis, pruritus, and breathlessness, none of which was severe enough to warrant discontinuance of therapy. A random sample of 21 women was taken to determine the effect on lactation. 18 of the 21 reported no change in milk production and continued to lactate until the end of the treatment period. Lactation decreased in the other 3 women. Further intensive studies are required before any definitive conclusion can be reached on the effect of this combination on the quality of the breast milk and duration of lactation.

The authors present a survey of the side effects of beta-blocking agents. Practolol has been linked with a lupus syndrome characterized by biological abnormalities and eventually clinical signs; later on, other beta-blocking agents have been able to induce such manifestations. Thus, all beta-blocking agents proved hazardous and they must be prescribed with extreme caution in patients with immunological imbalance. Furthermore, pruritus and lichenoid or psoriasiform dermatitis have been documented. This must be kept in mind in the presence of any unspecific rash in elderly patients.

A 1981 outbreak of dermatitis in Austin, Tex, was traced to occupational exposure to wheat infested with the straw itch mite, Pyemotes ventricosus; the wheat was being sold for decorative purposes by an imported-goods store located on the second floor of a large, modern, shopping mall complex. In addition to an extensive varicelliform skin eruption, one employee also had chills, fever, malaise, diarrhea, and anorexia associated with her exposure to these mites. The straw itch mite has been associated with several large epidemics of dermatitis during the 19th and 20th centuries. This outbreak is the third reported in Texas since 1961. Physicians should consider the possibility of straw itch mite infestations of products brought into the home or places of employment when they observe patients with a varicelliform or chigger-bite-like dermatitis, which may be accompanied by constitutional symptoms.

In a prospective study of thirty-seven infants presenting with dermatitis in infancy it was found that pruritus, a positive family history of atopy, and the age of onset of skin lesions were of little value in distinguishing atopic dermatitis from infantile seborrhoeic dermatitis. The most useful distinguishing features were the increased incidence of lesions on the forearms and shins in atopic dermatitis, and in the axillae in infantile seborrhoeic dermatitis. The onset of skin lesions solely in the napkin area favoured a diagnosis of infantile seborrhoeic dermatitis, but when more than one site was involved the distinction was less clear. Infantile seborrhoeic dermatitis carried a much better prognosis than atopic dermatitis. In thirty-six out of thirty-seven infants the definitive diagnosis could be made with confidence on clinical grounds by the age of I year, but was often difficult at first presentation in early infancy.

Scabies is an inflammatory skin disease caused by the mite Sarcoptes scabiei. The mite itself provokes little dermatitis. The rash and itch associated with scabies is probably a manifestation of immune response. The immune response may help to limit the number of infesting organisms either directly by toxic products generated during the reaction or indirectly by evoking scratch. Critical studies using purified antigens from scabies mites in vivo and in vitro will be necessary in order to clarify the nature of the antigens involved and the role of different types of immune responses in producing clinical disease.
