In three experiments, cattle, mice and guinea pigs were inoculated with viable cultures of Moraxella bovis or fractions of this organism. Fractions were obtained by disruption of cells with a fractionator at 20,000 pounds per square inch, and separating the cell wall and cell sap fractions by differential centrifugation. Cell sap fractions were further separated by ultra-centrifugation, heating and precipitation with (NH(4))(2) SO(4). Different fractions induced different pathophysiological manifestations. The cell wall fractions caused localized lesions (necrosis) at the site of injection, and emphysema and congestion of the lungs. Cell sap fractions induced a "shock syndrome," as well as hemorrhage and inflammation of the intestines, hemorrhage and congestion of lymph nodes, liver, adrenal and spleen. Cell sap also induced
conjunctivitis
in mice and guinea pigs, and periocular edema, myosis, ocular
pruritus
and lacrimation in cattle. The authors suggest that M. bovis probably produces endotoxins and exotoxins as well as possibly a specific oculopathic substance, but more definitive work is needed to confirm this. They caution that consideration of these toxins should be made in any application of M. bovis for vaccines or other immunological studies.

Noninfectious rhinitis and the clinical pharmacology of drugs used in its treatment, including specific treatment recommendations, are reviewed. Characterized by hyperreactivity of the nasal mucosa to a variety of stimuli, noninfectious rhinitis can be classified either as seasonal or perennial allergic rhinitis (when antigens can be identified) or as vasomotor or nonallergic rhinitis (when antigens are not identifiable). However, noninfectious rhinitis is probably better viewed as a continuum between these extremes rather than as definitive categories. Treatment measures include removal of offending agents when possible, and injection of allergenic extracts to decrease sensitivity to inhalant allergens. Among the pharmacologic alternatives, the classical H1 antihistamines competitively inhibit the action of mast-cell histamine at its receptor sites and thus decrease sneezing, nasal

. Orally bioavailable alpha-adrenergic agents such as pseudoephedrine and phenylpropanolamine decrease nasal congestion that responds poorly to antihistamines. Topical vasoconstrictors are contraindicated in chronic rhinitis because of the complications of rebound congestion. Systemic corticosteroids are effective but rarely appropriate for chronic rhinitis. Potent topical corticosteroids such as intranasal beclomethasone dipropionate are useful for severe nasal congestion. Cromolyn sodium, an inhibitor of histamine release from mast cells, appears to have some efficacy in suppressing symptoms of allergic rhinitis and
conjunctivitis
when used topically. Anticholinergics have occasionally been recommended to reduce rhinorrhea, but little data on their efficacy are available.

Workers in the coffee industry were investigated. Study I comprised 50 selected cases of whom 25 had work-related symptoms and 25 had not. Prick tests and RAST investigations with different factory dust extracts were performed. Study II was a cross-sectional study comprising 129 workers who were prick-tested with one factory dust extract and with castor bean (CB). More than 40% described occupationally related asthma, rhinitis,
conjunctivitis
or
pruritus
, and in about half of these cases sensitization with one or two allergens was found. One allergen comes from coffee beans and is found in the factory dust, mainly where the raw coffee is handled. This allergen is destroyed in the roasting process. The other allergen is identical with the allergen from castor bean and its presumed to enter the plant via the sacks. Predisposing factors to developing sensitization were atopic status, degree and length of exposure, and smoking habits.

Auranofin (Ridaura SK and F 39 162) was tested in an open multicenter study with regard to its anti-inflammatory effect in 166 patients suffering from rheumatoid arthritis. The time for treatment lasted for one or two years. The therapeutic effect of the drug was judged by its influence on pain, joint swelling, morning stiffness, grip strength, blood-sedimentation rate and rheumatoid factor etc. The serum gold concentration was measured regularly. With Auranofin the majority of the patients achieved a lasting improvement of the condition. The therapeutical effect was observed gradually. Side-effects were frequent but removal from the therapy was rare. Most of the side-effects were diarrhea, rash,

induced by contact lenses had symptoms of increased mucous production with blurred vision, decreased contact lens tolerance,

When the tears were collected and analyzed for immunoglobulins, the more symptomatic eye was the left in nine patients and the right in seven patients. The fellow eyes were equally symptomatic in two patients. Tear IgE levels in patients with giant papillary

were significantly increased, especially in the more symptomatic eye (geometric mean, 6.9 IU/ml; P less than .01) compared with those in a control group who also wore contact lenses (2.1 IU/ml). Increased tear IgG levels (50.7 micrograms/ml; P less than .01) were found in the more symptomatic eyes of patients with giant papillary
conjunctivitis
. In eight of the 18 patients, tear IgM was measurable (greater than 4.7 micrograms/ml), whereas none of the control groups had detectable amounts of IgM in their tears. Studies with transferrin as a marker for the vascular leakage of serum proteins into the tears showed that local production was responsible for the increased tear immunoglobulin levels.

and blurring. The earlier stages of the condition, as described by other authors, could be defined in the remaining 11 patients. Cytologic tests - pipette samples of tear fluid and tarsal conjunctival scrapings - demonstrated eosinophil leucocytes and neutrophilia in most of the giant papillary
conjunctivitis
cases. The conjunctival scraping proved the more sensitive test for detection of eosinophils, the pipette test being mainly an indicator of inflammatory reaction. The condition seems to be allergic with an element of mechanical irritation. The use of polyamide sutures of the 8-0 size is not recommended.

Thirteen out of 28 persons (14 men and 14 women) employed in a company which imports, prepares and distributes plant products used in spices and as ingredients in so-called health food, had developed work-related symptoms in the form of rhinitis,
conjunctivitis
, asthma,
itch
or urticaria. The symptoms occurred in connection with specific work operations, especially in the grinding and packaging of buckwheat. Seven out of 25 examined persons (28%) had at least one positive allergy test (prick test or RAST) against buckwheat. Furthermore, one person had positive RAST against extract from castor-oil bean. The correlation between positive allergy tests and work-related symptoms is significant. Fourteen persons were also patch tested. All were negative except one person who had a positive patch test against nickel. The nickel allergy was diagnosed before this study was done and had no relation to the work. The levels of airborne dust in the breathing zones of the workers when they performed dust-forming work were around and below 5 mg/m3. When buckwheat flour was packaged, the airborne dust levels were about 1-2 mg/m3. Thus, exposure to comparatively low levels of buckwheat dust may induce a definite risk of rapidly ensuing allergy. It is also obvious that persons without atopic stigmata, such as an earlier history of allergy or the occurrence of allergic diseases in relatives, risk becoming allergic. A conclusion is that health food could be a danger, when it is inhaled.

A young Black woman with mixed connective tissue disease (MCTD) developed an aseptic meningitis after receiving ibuprofen. The meningeal reaction, reported infrequently in systemic lupus erythematosus (SLE) and only once previously in MCTD, was characterized by a predominantly polymorphonuclear cerebrospinal fluid (CSF) pleocytosis and depression of CSF glucose. Reversible renal insufficiency also occurred. Features suggestive of a hypersensitivity reaction included

Giant papillae of the upper tarsal conjunctiva have been found in vernal
conjunctivitis
and, more recently, in giant papillary
conjunctivitis
in patients with contact lenses. A similar syndrome occurred in 11 patients who had undergone penetrating keratoplasty with interrupted nylon sutures. Minimal
itching
, mild mucous discharge, mild blepharoptosis, and giant papillae of the upper tarsal conjunctiva were associated with exposed suture ends. Removal of exposed sutures led to resolution of the condition. Conjunctival scrapings from three of five patients demonstrated eosinophils. Although the syndrome may have an immunologic basis, the findings are also consistent with a mechanical origin related to exposed nylon sutures.
