Pediculosis ciliaris, though not as common as in the past, is still not a rare disorder. Infestation has become more prevalent among adolescents within the past few years. Severe
itching
, irritation, secondary
conjunctivitis
, and preauricular adenopathy may accompany the louse infestation. A brief review of the etiology and treatment of pediculosis ciliaris has been presented. Every physician should be aware of this disorder and be adequately prepared to treat it.

Our patient was a 24-year-old female textile cutter who had a
conjunctivitis
and an erythematous
itching
dermatitis of the eyelids, nasal mucous membranes, and the corners of the mouth. The patch tests with the standard series and a pharmaceutical test battery were negative. Further epicutaneous testing revealed a strong positive reaction to the textile cutting patterns she handled, which were duplicated by diazo processing. Specifically, the contact dermatitis reaction was caused by dimethylthiourea, an additive in diazo-sensitized paper.

An aromatic retinoid (Ro-10/9359) was used for oral treatment of five cases of ichthyosis (three lamellar, two X-linked. Complete clearing of the skin lesions was achieved in all five patients within 24.2 +/- 3.2 days (X-linked 21.75 +/- 6.5, lamellar 23 days). Histopathology showed reduction of the hyperkeratosis, and thickening of the granular layer. Clinical side effects were of mild intensity and included cheilitis,
conjunctivitis
and
pruritus
. All side effects were reversible upon reduction of the daily dosage. In three patients treatment was discontinued after clearing of lesions. Fresh lesions re-appeared 6 weeks later. One patient with X-linked ichthyosis developed two recurrences during maintenance treatment; one patient with lamellar ichthyosis was kept in complete remission for 9 weeks on a reduced daily dosage.

Both hard and soft lens wearers develop a syndrome of decreased tolerance, increased mucus, mild
itching
, and giant papillary excrescences in the upper tarsal conjunctiva that resemble a varnal
conjunctivitis
. In the fully developed syndrome, the upper tarsal plate has an increase in stringy mucus and is covered by large papillae crowded together. The syndrome develops after months to years of otherwise successful lens wear and occurs in users of all types of soft and hard lenses. Histologic examination of tissues from 55 patients with well-developed giant papillary
conjunctivitis
compared with tissue from 15 normal people showed three findings characteristic of the syndrome: (1) mast cells in the epithelium, (2) eosinophils in the epithelium and substantia propria, and (3) basophils in the epithelium and substantia propria. Plasma cells and lymphocytes per cubic millimeter were not increased in detailed counts of 15 patients and 15 normal individuals. It is proposed that the number of plasma cells and lymphocytes cannot increase much beyond the level already present in normal conjuctiva and further influx of mononuclear inflammatory cells is the impetus for growth of the papillae. It is proposed that giant papillary
conjunctivitis
is a generalized response of the upper tarsal conjunctiva.

The eye reacts to foreign substances through a variety of specific and non-specific defense mechanisms. Constantly exposed to a great variety of microorganisms, the eye is capable of protecting itself without altering its own structure and function. Its resistance relies upon anatomic and physiological properties of its external components (eyelids, tears, conjunctiva and cornea). Most of the times, the conjunctiva becomes affected, resulting in a clinical picture of
conjunctivitis
where allergy predominates, expressed as allergic rhinoconjunctivitis, allergic conjunctivitis, vernal keratoconjunctivitis, giant papillary
conjunctivitis
and flictenular
conjunctivitis
. The physiopathology is considered to be of type I reaction (IgE mediated). Frequent complains often associated with other allergic diseases are:
pruritus
, tearing, photofobia and ocular redness. The diagnosis of allergic conjunctivitis in done by means of a throughout clinical history, conjunctival citology and evaluation of specific IgE with immediate skin tests. Treatment is symptomatic (antihistamines, antibiotics and/or topic steroids), but prevention (environmental control and sodium cromoglycate) and specific immunotherapy must be considered.

Animal studies indicate that 13-cis-retinoic acid (CRA) inhibits bladder tumor growth and is effective in treating patients with serious dermatologic disorders. A trial of CRA in patients at high risk for recurrent Ta, T1 tumors was initiated at an experimental dose of 0.5 mg/kg/d in three divided doses, increasing to 1 mg/kg/d at four weeks. Treatment of twenty eligible patients lasted for six months with an additional 24 month follow-up period. One patient was later excluded due to toxicity resulting in an early dose reduction. Eight patients stopped treatment before three months; of these five, had recurrences within three months, one developed pulmonary metastasis, and one developed a T2G3 tumor. Four patients stopped treatment between three and six months; three of them had recurrences before one year and one had no evidence of disease at seven years. Seven patients completed the course; of these three had recurrences within six months, and three more had recurrences at 8, 15, and 45 months, respectively. Toxicity was nearly universal; cheilosis,
conjunctivitis
,
pruritus
, joint and eye pain, flashing lights, and erythrocyte sedimentation rate (ESR) over 60 were all noted. The lack of positive results and the frequency and severity of toxicity led to termination of the study.

A double-masked, randomised, placebo-controlled study was conducted to evaluate the effectiveness of nedocromil 2% eyedrops, a mast cell stabilizer, in 20 symptomatic patients with vernal
conjunctivitis
. A 1-week baseline period was followed by 6 weeks of treatment. Clinical examination and cytological evaluation of tear fluid were performed weekly, and the patients recorded their subjective assessment on a daily diary card. The nedocromil group showed significantly less hyperaemia in the course of treatment than did the placebo group, and significantly less
itching
at all visits compared with baseline
itching
. In the nedocromil-treated group, but not in the placebo group, the number of neutrophils, eosinophils and lymphocytes in tears decreased significantly during some treatment weeks when compared with baseline. The overall assessment of treatment efficacy by both clinician and patient was significantly in favour of nedocromil treatment over placebo.

Histamine is the key mediator producing
itching
, redness and chemosis in allergic conjunctivitis. Histamine levels in tears are increased ten-fold in patients with this allergic condition. Levocabastine is a newly synthesized histamine H1 antagonist which has been formulated as both eye drops and nasal spray. In well established assays of antihistamine activity, levocabastine was found to be the most potent antihistamine compound available, being 15,000 times more potent than chlorpheniramine. Ocular provocation studies in man have shown that levocabastine protects against the symptoms of allergen-induced
conjunctivitis
. Ophthalmological examinations, including slit lamp and ophthalmoscopy showed no adverse effects. Data from therapeutic studies are available for more than 1700 patients with allergic conjunctivitis treated for 2-16 weeks. One drop of levocabastine (0.5 mg/ml) per eye given two to four times daily provided significantly better symptom control than placebo, with good to excellent results in 71% of patients on levocabastine compared to 55% on placebo (p < 0.001). Levocabastine has a fast onset of action. In one study 94% of patients experienced symptom relief within 15 minutes after the first instillation. The effects observed with levocabastine were at least as good as those with ocular cromoglycate or oral terfenadine. The incidence of adverse experiences was not different from placebo. Levocabastine promises to be a valuable treatment for patients with allergic conjunctivitis.

Limbal Vernal kerato-
conjunctivitis
is the 6th most frequent diagnosis in the Ophthalmology service of CHU, Yaounde, Cameroon, and accounts for 2.8% of all clinic consultations. Only grades I and II of the disease (Diallo's classification) are found in this forest zone of Africa. Advanced stages which may cause corneal blindness being probably due to absence of xerophthalmia in this part of Africa. Both sexes are equally affected. Association with other allergic diseases is very rare and there is no hereditary tendency. Serum and tear Immunoglobulin assays in patients and controls of the same parent-hood showed no statistical differences (P > 0.05). Mast cell immunocytological studies might be more rewarding. Relief of
pruritus
is best with sodium cromoglycate and Naaga. However, in severe cases interrupted systemic steroids (not exceeding one week each time) are utilized. Poor drug compliance, due to costs is frequent; probably cautious cryo application in severe cases, might together with infrequent Naaga or sodium cromoglycate, procure longer remissions.

Living lice and eggs were found in 19% of 2643 children examined in Israel. Boys and girls were equally infested. Bite reactions,
pruritus
, excoriations, lymphadenopathy, and
conjunctivitis
were the most common signs and symptoms. Bite reactions,
pruritus
, excoriations, and
conjunctivitis
were found more frequently in infested children than in noninfested children. Lymphadenopathy, eczema, influenza, herpes, crusts behind ears, blepharitis, seborrhea, psoriasis, erythema, vitiligo, secondary infections, and scars were equally common in both groups. The sequence of the skin reactions of a volunteer to continuous exposure to lice bites was as follow: phase I, no clinical symptoms; phase II, papules accompanied by
pruritus
of medium intensity; phase III, wheal formation immediately after the bite, followed by a delayed papular reaction and intense
itching
; phase IV, papular reaction with diminished reactivity of the skin and mild
pruritus
. Healed bite reactions reappeared when other parts of the skin were again exposed to the lice.
