The Authors are presenting a concise review on a protein, beta2-microglobulin, that has gained in notable importance in the immunological field during these last years. The story of this protein begins with nephrology because it has a notable importance in the diagnosis of renal tubular diseases; in fact its presence in the electrophoretic pattern of
proteinuria
puts in evidence renal tubular damage. Nextly immunological studies have shown the beta2-microglobulin has strict correlations with the HL-A system; probably it forms a part of this system, as shown by the new immunofluorescent methods applied to the lymphocte. A recent study has shown also the beta2-microglobulin serum levels are very high in some malignant diseases, like in
acute leukemia
; however this low molecular weight protein could also be the expression of malignancy because it can be a product of non mature cells in a large quantity.

A patient with
acute leukemia
and an IgM, kappa (IgMkappa) monoclonal gammopathy, Bence-Jones
proteinuria
, and blasts containing intracytoplasmic vacuoles with peroxidase-positive inclusions is discussed. Special stains, immunofluorescence, and electron microscopy suggested that the vacuoles were autophagosomes containing Auer-body-like inclusions, and that the blast cells did not synthesize the paraprotein. Chemotherapy with cyclophosphamide, vincristine, and prednisone resulted in transient improvement of the leukemia, but the level of the paraprotein was unchanged. acute leukemia
are reviewed and contrasted with this case.

. In both cases, the clinical picture was primarily characterized by
proteinuria
without hypertension or renal failure. Electron microscopy of renal biopsy specimens revealed a similar pattern in the two cases with extensive coalescence of foot processes and intramembraneous deposition of electron-dense material. Our data suggest that the kidney may be a target organ in chronic GVHD.

A phase I trial of diglycoaldehyde (Inox) in children with leukemia established that the maximum tolerated dose of a 5-day schedule was 1.5 g/m2/day. A phase II study was undertaken by the Children's Cancer Study Group to evaluate the efficacy of this dose. Forty-seven children with
acute leukemia
refractory to conventional forms of therapy were entered in the study: 29 patients with acute lymphocyte leukemia/acute undifferentiated leukemia and 18 with acute nonlymphocytic leukemia. Inox was administered at a dose of 1.5 g/m2 as a 4-6 hour iv infusion daily for 5 days every 14 days. Toxic effects included myelosuppression,
proteinuria
, nausea, vomiting, diarrhea, local tissue reactions, hypocalcemia, transitory serum amylase elevation, and transitory hypotension. There was one life-threatening episode of drug-related renal toxicity. Of the 27 patients who received a minimum of two courses, partial remissions were observed in two patients with acute nonlymphocytic leukemia. Inox was inactive against advanced acute lymphocytic leukemia.
