The significance of magnesium in the etiology and treatment of diseases of the circulatory system has been investigated for several decades. Experimental, clinical and epidemiological data concerning this problem are abundant, however their findings do not let--at least so far--to draw unequivocal conclusions as to the significance of this element in the treatment of cardiovascular diseases. Mg++ is known to be responsible for the function of about 300 enzymes, and its deficiency results in necrosis of the cells due to depletion of energetic stores of phosphates, leads to disturbances in fat metabolism, increased aggregation and shortened survival time of platelets, increased level of factor III, disturbed synthesis of proteins (mitral valve leaflet
prolapse
) as well as of prostanoid function. Despite the fact that serum level of Mg++ differs from its concentration in tissues, majority of clinical studies relies on serum levels of the element. The effects of changes in serum levels of Mg++ resulting from various pathophysiological processes should be differentiated from the effects of magnesium supplementation or from administration of the element in excess for therapeutic reasons when the levels in the organism have been considered normal.

Anchoring of spinal cord at S2 level is described in a female patient aged 46, associated with bilateral equinovarus deformity and spina bifida in lumbar segment. The onset of the disease was sudden due to
prolapse
of intervertebral disc at L2-L3 and L3-L4 levels followed by bilateral flaccid-spastic paraparesis. In view of congenital spinal deformity lumbar tap was abandoned and MRI was carried out which showed spinal cord reaching down to S2 level. Diagnostic and therapeutic management is described and literature review is presented.

Emergency cerclage was performed on 27 years old patient with membrane
prolapse
in 21 weeks' gestation. Ultrasonographically guided amniocentesis was performed intraoperatively in order to decrease intraamniotic pressure. Aggressive prophylactic management (wide spectrum antibiotics and tocolysis-nifedipine) resulted in pregnancy prolongation up to 30 week of gestation. At that time healthy premature newborn (1520 g, Apgar score 7 in 1st minute) was delivered by caesarean section. The mother was discharged from the hospital on the 4th postoperative day, whereas the child after 5 weeks weighted 2130 g.

According to many investigators, lumbar disc herniation is formed by the nucleus pulposus displaced to the vertebral canal through the cracks in the annulus fibrosus as a final result of disc degeneration. Specimens obtained during 187 disc surgeries were studied to identify the morphological nature of various forms of disc herniation, especially protrusion and
prolapse
. In 29% of cases the herniated part was annulus fibrosus and in 21.5% cases the annulus fibrosus was seriously involved. Simultaneously 308 autopsy specimens were studied. On gross examination the signs of degeneration were advanced in 82, and strongly pronounced in 42. In 15 specimens, aged 40 years or older, fibre bundles were reversed to normal convex orientation producing annulus fibrosus bulge. Microscopic examination showed pronounced myxomatous degeneration within the annulus fibrosus. In these circumstances, we believe that general opinion must be verified, because in some herniations the nuclei pulposus are not involved and herniations are solely prolapses of annulus fibrosus. So, the lumbar intervertebral disc herniation should be classified on the basis of histological examinations as a herniation of nucleus pulposus, herniation of annulus fibrosus or as a mixed type herniation where both disc elements are involved.

In 12 patients aged 9 to 17 years (14.1 +/- 2.3 SD years) (11 girls, 1 boy) complaining of abdominal pain and chronic constipation, barium large bowel X-ray examination showed the following anatomical and positional abnormalities:

of the transverse colon to the small pelvis was found in 11/12 patients,
ptosis
of the hepatic flexure of the transverse colon (2/12),
ptosis
of both the hepatic and lienal flexures (2/12), dolichocolon (2/12), and dolichosigma (2/12); 10 of 12 children had symptoms characteristic of irritable colon syndrome. It seems that clinical symptoms accompanying these abnormalities begin to manifest mainly in girls during adolescence because at that time the final length of the colon is established, the transit time reaches its maximal value, and the shape of the pelvis becomes characteristic for this gender. These abnormalities are rather constitutional than acquired since previously it has been reported that the vertical
ptosis
of the kidney(s) occurs also mainly in adolescent girls.

A case of pulmonary thromboembolism with transient pulmonary hypertension of a rare cause is presented. In 24-year-old woman myasthenia was recognised on the ground of
ptosis
and fixation of eyes muscles from the 14th year of age. The treatment with mestinon was ineffective. Before planned thymectomy serious disturbances of heart rhythm and conduction were confirmed. Stimulator was implanted and thymectomy was done. No improvement of neurological state was observed despite the treatment with mestinone and prednisone. When she was 30 years old disease of lung appeared with fever, cough and parenchymal and pleural lesions of right lung. Antibiotic therapy was ineffective. CT scan and US examination revealed large thrombus inside the right auricle connected with stimulator electrodes. Embolisation of right pulmonary arteria was confirmed also. Treatment with heparin was ineffective and thrombectomy was performed. Exact neurological examination stated that the patient had no typical symptoms of myasthenia and that symptoms related with eyes and heart could be result of mitochondrial myopathy. Diagnosis was confirmed by EMG examination and muscle biopsy.

Transglabellar, transorbital approach through the bottom of the frontal sinus for tumours invading anterior skull base in presented. Exposure was improved by rhinectomy and lateral swing of the nose. The most common tumours in the group of 11 patients were squamous cell carcinoma (4) and olfactory neuroblastoma (4). Skull base was excised en block with partial maxillectomy in 8 patients and with total maxillectomy in 3 patients. Orbitectomy was performed in 6 cases. Dura defect was closed with fascia lata and fibrin glue. No serious complications after surgery developed. Rhinoscopy and computed tomography demonstrated clear cavity and no dura

We present a case of vaginal vault
prolapse
after hysterectomy associated with cystocoele with central and lateral defect and stress urinary incontinence, that was treated surgically with employment of sacrospinous colpopexy through anterior approach (from paravesical space), combined with anterior colporrhaphy by double TOT approach method (that is a butterfly-shaped polipropylen mesh, which arms were carried through upper and lower parts of obturator foramens by tension-free method). There were no postoperative complications. A control examination at 1 and 3 months after the operation showed maintenance of normal anatomic relations, which were obtained as a result of repair, total control of urinary continence and full patient's satisfaction from the operation.

The aim of the publication is to assess the effectiveness of the treatment of type I disc extrusion according to Hansen by using the method of hemilaminectomy without fenestration. The evaluated clinical material comprised 36 chondrodystrophic dogs in which 38 surgical procedures were performed. All dogs were administered corticosteroid during the operation. Dachshunds made up to 79% of patients. In 34 cases grainy mass was extracted from the vertebral canal. In two cases it was plaster-like mass and in other two--granulo-chondral. Most often (12 cases) the
prolapse
of the nucleus pulposus was observed between the first and second lumbar vertebra. There was one case of the postoperative pyogenic infection of the wound. Recovery was observed in all dogs with 3rd grade clinical signs, 17 out of 18 dogs with 4th grade and 8 out of 13 dogs with 5th grade of dysfunction severity. The recovery time of the dog depended in the statistically significant way on the severity of symptoms and the grade of the present disease (p<0.00005). There did not appear to be any statistically significat relationship between the recovery time and the duration of symptoms pre operatively (p=0.42), the time of non-ambulatory status (p=0.27) and the breed of the dog (p=0.81).
