The own method of uterine
prolapse
treatment with the use of beeswax pessary is presented. The described method is applied in cases where general health conditions do not allow to perform the surgery. The study revealed that the beeswax is a good material to make a vaginal pessary. It's great plasticity allows individual modelling in compliance with anatomical conditions of treated patient. That provides the proper stability of uterine position. The risk of possible urinary stress incontinence due to uterine reposition is also diminished.

The authors describe the method of non-operative treatment of widely occurring low back pain syndromes diagnosed as due to isolated degeneration of the nucleus pulposus. In this phase of degenerative spinal disease pain is not due to nucleus pulposus

. The patients are dissatisfied with the results of physiotherapy and rehabilitation, and surgical treatment brings no satisfactory results. The method of dissolving of nucleus pulposus, widely used in the USA, Australia and West European countries, has been used in Poland only in the clinical centre of the authors. The clinical experience gathered during these years made possible delineation of the syndrome of isolated nucleus pulposus degeneration and strict qualification for chemonucleolysis which gives success in about 90% of cases.

Exercise tolerance in children with the primary bicuspid valve

syndrome has been evaluated with ergometric test. Additionally, an effect of environmental factors on exercise tolerance has been assessed in children from the urban and rural areas. A good tolerance of submaximal loads has been shown in both groups. However, the obtained results have been significantly more favourable in children from rural areas. The authors have concluded that limitation of exercise in children with bicuspid valve

is not necessary during an intensive growth.

In 1985-1993 there were 37,046 deliveries in the Maternity and Gynaecological Ward of District General Hospital in Radom. In 4873 (13.15%) cases the caesarean section (c.s) was performed. Primiparas constituted 39.3% and the repeated c.s. was carried out on 1160 (39.2%) patients. In the analysed period the increase in percentage of the c.s. was observed from 11.79% in 1985 to 14.81% of the total number of deliveries in 1993. The most frequent indications to perform the c.s. were the imminent intrauterine asphyxia of the fetus--25.36%, the abnormal position of the fetus--17.73% and the nonprogressive labour--9.89%. In all these groups the operations were performed in over 70% of primiparas. In the case of multiparas the indications to operate were:

of the loop of umbilical cord--71%, placenta previa in 85.8% and the prematurely detached placenta in 72.2% of cases. In 4.45% of cases the c.s. was performed as the result of other than obstetric indications.

of small intestine through vagina in 66 year old woman, who had hysterectomy by laparotomy four years earlier. The intestines were repositioned by the vaginal route and then the opening at the top of vagina was sutured by laparatomy. In the next stage colpo-perineoplasty with formation of very high perineum was made.

Fifty-five patients were treated with botulin injections into the muscles showing dystonia, contracture or tremor. Twenty two of them had torticollis, 21 had blepharospasm, 10 had hemifacial spasm, and 2 had tremor. In all, 112 injections were done with good result in 64%, slight effect in 27% and without effect in 9% of the cases. Similar results have been reported from other centers in the world. Adverse effects were not significant and disappeared after several days or weeks. They included
ptosis
, speech and deglutition disturbances, general weakness and neurotic reactions. These adverse effects developed in 12 cases. In cases of tremor the dose as well as the technique of injections must be individualized. The method is an important therapeutic advance and can be applied in outpatient clinics.

The concentrations of collagen, hydroxyproline, glycosaminoglycans and estradiol were determined in 60 women aged 51 to 60 years (mean: 55.6 +/- 4.4). When compared with values from women with normal statics with women with lowered statics of the genital organ demonstrated significantly lower concentration of collagen (p < 0.05) in the teres ligaments. of the genital organ had significantly lower concentration of estradiol in serum (p < 0.001) and of collagen (p < 0.01) in the teres ligaments, with significantly higher daily excretion of hydroxyproline (p < 0.01) and glycosaminoglycans (p < 0.001) in urine. In women with

of the genital organ a significantly positive correlation between the concentration of estradiol and collagen (r = 0.76; p < 0.05) and negative correlation between the concentration of estradiol and hydroxyproline (r = -0.59; p < 0.05) and glycosaminoglycans (r = -0.61; p < 0.05) has been found.

Four cases (2 females and 2 males aged 18-22) of lumbar spine osteochondrosis with massive defects within end-plates and vertebral bodies are presented. In 3 cases bony overgrowth of posterior vertebral body wall with marginal spur occurred. Huge
prolapse
of annulus fibrosus occurred in one case. Neurological deficits appeared in all 4 cases. Bony overgrowth has been excised in 3 cases, nucleotomy accompanied one of them. The remaining patient required excision of the disc and segmental fusion. Good results were achieved in all above patients. The author recommends this treatment in similar cases.

In adult humans moral intervertebral disc (id) is an avascular tissue and becomes so called sequestrated autoantigen. Any acquired defect of anulus fibrosus may potentially lead to contact of immunocompetent cells circulating in the blood with id antigens thus inducing autoimmune reaction. 34 patients operated on because of lumbar discopathy were studied. The id injury was divided into: a) protrusion, B) simple

, c) subligamentous

, d) sequester. The samples of surgically removed id were subjected to histopathological and immunohistochemical study. Presence of granulation tissue, neovascularization and humoral response (confirmed by immunopositive reaction to factor VIII and IgG) was found in decreasing pattern in the following groups: I) sequesters, 2) simple prolapses, and 3) subligamentous prolapses. Among protrusions there were only two cases positive for IgG. A negative reaction to C3bR was seen in all the groups of id. The obtained results suggest that immune reaction against lumbar id is rather an effect than a cause of its herniation.
