The advantages of Fletcher's operation for vaginal pouch
prolapse
after previous hysterectomy or total extirpation are reported with a slightly modified method in eight cases. The following results show this surgical procedure to be especially advantageous: A permanent good fixation of vagina without shortening, no recurrence no
pain
and neither so-called antefixation complaints. A complete rehabilitation of cohabitation and orgasm was effected. Urological complications, such as obstruction of urine or micturition difficulties are eliminated by this surgical intervention.

Chronic low back pain is more significant from the psychiatric point of view than the acute pain. As a persistent
pain
of organic origin it leads to typical psychopathological symptoms ("algogenic psychosyndrome"). The algogenic psychosyndrome must be differentiated diagnostically from neurotic developments and masked depressions. Psychosomatic low back pain is a warning of failure in the accomplishment of professional or family duties and chronic and chronic inhibited aggression. - An interdisciplinary study of the courses of 50 patients operated on for
prolapse
of the lumbar intervertebral disk showed that the psychopathological picture and the
pain
syndrome are usually determined by several factors. The majority of neurotic disordered personalities developed symptoms of new syndromes after the operation.

The article reports on twenty patients with thoracic prolapsed discs treated by surgery within a period of ten years. The incidence in relation to prolapses at all levels was 1.2%. The average age was 50.3 years and there was no definite sex predominance. The segments Th 10-11 and Th 11-12 were the preferred sites. The period of the disease varied from a few hours to nine years. Among the most frequent subjective complaints were radiating
pain
, paraparesis of the legs and disturbed micturition. A definitely traumatic cause of the disease was present in one case only. Besides exact neurological diagnosis it is imperative to effect myelography with positive contrast medium, revealing the typical circular or oval-shaped filling defects. In our patients, laminectomy was performed 17 times, and hemilaminectomy three times. Besides removal of the
prolapse
, the dentate ligament was always divided. In some cases we also performed rhizotomy. Operation improved 15 patients, whereas in two patients the condition got worse and three patients showed neither improvement nor deterioration.

Traumatic rupture of the diaphragm is diagnosed during the acute stage in only 50% of the cases. The reasons for this failure are that in many of these cases clinical symptoms of damage to abdominal organs or of injuries to the limbs predominate. Traumatic rupture of the diaphragm is usually the result of an indirect trauma. Clinical signs of extensive
prolapse
of viscera into the thoracic cavity are: dyspnoea, tachycardia, cyanosis or intestinal sounds heard over the thorax during auscultation. Once the diagnosis has been established surgical repair should follow. During the acute stage laparotomy is preferable; thoracotomy is indicated during the chronic stage. Post-operative symptoms are: dyspnoea during exercise,
pain
in the affected half of the chest and roentgeno-kymographically demonstrable restrictions of movement in the ruptured side of the diaphragm.

A questionnaire was sent to members of the American Society of Colon and Rectal Surgeons. Four hundred and forty-five (43.6%) were tabulated to establish criteria and results of surgical treatment of hemorrhoidal disease. The most frequent indication for surgery, was internal and external hemorrhoids with mucosal
prolapse
. Significant differences were found in time required for complete healing, frequency of wound dehiscence, frequency of postoperative stenosis, and frequency of postoperative infection. Results did not support opinions that the closed technique was associated with significantly less
pain
, fewer complications, shorter hospital stay or earlier resumption of work.

In case ischias-type
pain
persists or recurs following surgical removal of lumbar disk prolapses, it will be necessary to carefully weigh against one another as possible causes: disk
prolapse
recidivation, epidural or intradural cicatrization, and pseudoradicular
pain
in myotendinoses. Postoperative diskitis or spondylitis must also be considered as a possible cause.

Vertical and medial nephroptosis was assessed on 60 consecutive excretory urographic examinations.
Ptosis
, both vertical and medial, was seen more commonly in females, and vertical
ptosis
was more frequent than medial
ptosis
. In our series there was no significant evidence of predominance on the right side. Dietl crisis, nausea, vomiting, hypotension, oliguria, or orthostatic hypertension were not encountered. Nephroptosis was mostly asymptomatic. In those patients with symptoms, lumbar
pain
was common and could be either aggravated or relieved by change in position. A new sign, paradoxic displacement, is described. This could be of value to the surgeon and radiotherapist in evaluating enlargement of a huge abdominal mass - a difficulat task to assess clinically.

To minimize the risk of visual loss in diabetic patients, recognition of early signs of oculopathy is essential. Diabetes-associated third-nerve palsy is manifested by unilateral
ptosis
and exotropia. Symptoms of closed-angle glaucoma are intense
pain
, halos around lights, and blurred vision. Open-angle glaucoma does not necessarily produce symptoms and is treated medically. A gradual decrease in visual acuity, sometimes associated with photophobia and difficulty in night driving, and monocular diplopia, are manifestations of cataract. The patient with "background" retinopathy usually complains of blurred or distorted central vision. Once the macula is involved, vision progressively decreases. Although the relationship of metabolic control to retinopathy has not been settled, evidence indicates that good medical control of the disease may delay onset of vascular complications.

Internal anal sphincter activity has been studied in 84 patients with haemorrhoids and 40 asymptomatic subjects. Activity was estimated by measuring maximum resting anal pressure with a water filled anal balloon probe 7 mm in diameter connected to a strain gauge pressure transducer. There was greater activity of the internal sphincter in patients with haemorrhoids than in controls, but there was no significant relationship between sphincter activity and duration of symptoms, predominant symptom (bleeding or
prolapse
), severity of symptoms, history of
pain
, history of straining at stool, or size of haemorrhoids. Straining at stool occurred significantly more often in patients whose main complaint was
prolapse
than in those whose main complaint was bleeding. Anal dilatation reduced sphincter activity and the best clinical results were obtained in those with the most active sphincter. An internal sphincter abnormality may be an aetiological factor in some patients but there must be other factors as well. Straining at stool may determine whether bleeding or
prolapse
is the predominant symptom.

The conditions associated with
prolapse
of the posterior leaflet of the mitral valve are multiple. The mechanisms of mitral valve prolapse as well as the pathogenesis of
pain
and ectopic impulse formation are reviewed. Propranolol appears to be the drug of choice for the symptomatic treatment of patients with this syndrome since it decreases myocardial oxygen demand and wall tension thus reducing or abolishing the discrepancy between myocardial oxygen demand and supply within the mitral apparatus. It has also been reported to modify the auscultatory findings associated with this condition. The frequency of this mitral valve abnormality in patients with obstructive coronary artery disease is reviewed. It appears that
prolapse
of the posterior leaflet scallops in patients with significant obstructive coronary artery disease represents an intermediate stage before mitral insufficiency occurs. This group of patients with papillary muscle dysfunction includes those with prolapsed leaflets without mitral insufficiency, those with systolic murmurs and compensated heart failure and others with progressive cardiac decompensation and severe mitral regurgitation.
