A 71-year-old woman had complete uterovaginal and vesical prolapse associated with a large decubitus ulcer on the anterior vaginal wall. She sustained blunt trauma to the perineum after a fall, which resulted in rupture of the prolapsed bladder. The management of this rare problem is discussed.

Six women with advanced uterine prolapse underwent examination by drip infusion urography. A new hypothesis is proposed for the pathogenesis of bilateral hydroureteronephrosis: the ureters are entrapped by the hiatus genitalis, not against the bladder but against the fundus of the fully prolapsed uterus. Drip infusion urography should include an exposure area of about 15 centimeters below the pubis, and at least one exposure should be taken with the patient in the standing position. Thus, the hourglass appearance of the prolapsed bladder will not be overlooked and would be helpful in explaining the cause of the hydroureteronephrosis.

A 3-year-old girl presented with a prolapsed cutaneous vesicostomy, and symptoms and signs of acute intestinal obstruction. The vesicostomy had been performed when she was 19 months old for bilateral vesicoureteral reflux. Postoperatively, prolapse of the bladder from the stoma occurred almost daily. The prolapsed part retracted naturally when the patient was relaxed and reclining. Intestinal obstruction was due to strangulated herniation of the bowel into the prolapsed bladder via the stoma of vesicostomy. Treatment consisted of manual reduction and the stoma of the vesicostomy was revised with the patient under general anesthesia.

Bladder prolapse is extremely rare in children. Reports have been limited to pathologic bladder conditions such as the exstrophy-epispadias complex. We report an interesting presentation of bladder prolapse in a child with Williams syndrome with an apparently normal bladder.

Complete eversion and transurethral prolapse of the urinary bladder is rare. We report a case of complete eversion and prolapse of bladder that occurred due to self pulling out of an indwelling Foley catheter in a 72-year-old woman. She presented with retention of urine concurrent with complete uterine procidentia. An indwelling Foley catheter was given to relieve the retention. The senile lady pulled out the catheter resulting in complete transurethral prolapse with bladder eversion. Under injection Midazolam sedation and with application of xylocain jelly the prolapsed bladder could be reduced manually back through the urethra. Definite corrective surgery was done later for the uterine prolapse.
