Efforts are underway to reduce the incidence of maternal mortality in the world, and concern is increasingly being expressed about the incidence of maternal morbidity. Maternal morbidity can be classified as 1) obstetric morbidity (direct, indirect, or psychological); 2) gynecological morbidity (direct, indirect, or psychological); and 3) contraceptive morbidity. Maternal morbidity has a debilitating effect on families and, thus, on entire societies. The complications leading to morbidity include ectopic pregnancy, spontaneous abortion, obstructed labor, abruptio placentae, severe postpartum hemorrhage, and uterine rupture. The longterm effects of these complications include uterine
prolapse
, urinary fistulas, and
Sheehan syndrome
(amenorrhea, failure of lactation, and infertility). In addition to conducting research to determine the extent and varieties of morbidity encountered in a population, a program to combat morbidity should 1) include an educational program for potential clients, 2) improve client confidence through the provision of necessary services like sanitation and of screening services, 3) network with other organizations providing services, and 4) incorporate proper epidemiological and record-keeping procedures.

Pituitary apoplexy is a clinical syndrome caused by an acute ischemic or hemorrhagic vascular accident involving a pituitary adenoma or an adjacent pituitary gland. Pituitary apoplexy may be associated with a variety of neurological and endocrinological signs and symptoms. However, isolated third cranial nerve palsy with
ptosis
as the presenting sign of pituitary apoplexy is very rare. We describe two cases of pituitary apoplexy presenting as sudden-onset unilateral
ptosis
and diplopia. In one case, brain magnetic resonance imaging (MRI) revealed a mass in the pituitary fossa with signs of hemorrhage, upward displacement of the optic chiasm, erosion of the sellar floor and invasion of the right cavernous sinus. In the other case, MRI showed a large area of insufficient enhancement in the anterior pituitary consistent with pituitary infarction or
Sheehan's syndrome
. We performed neurosurgical decompression via a transsphenoidal approach. Both patients showed an uneventful recovery. Both cases of isolated third cranial nerve palsy with
ptosis
completely resolved during the early postoperative period. We suggest that pituitary apoplexy should be included in the differential diagnosis of patients presenting with isolated third cranial nerve palsy with
ptosis
and that prompt neurosurgical decompression should be considered for the preservation of third cranial nerve function.

Pregnancy is often associated with ocular changes which may be more commonly transient but occasionally, permanent. It may be associated with the development of new ocular conditions, or can exacerbate pre-existing conditions. The ocular effects of pregnancy may be physiological or pathological or may be modifications of pre-existing conditions. Adnexial changes include chloasma, spider angiomas and
ptosis
. Anterior segment changes include a decrease in conjunctival capillaries and an increase in the granularity of conjunctival venules and in corneal curvature, changes in corneal thickness, refractive index, accommodation and refractive errors, and a decrease in intraocular pressure. Posterior segment changes include worsening of diabetic retinopathy, central serous chorioretinopathy, increased risk of peripheral vitreochorioretinal dystrophies and retinal detachment, and a beneficial effect on non-infectious uveitis. Systemic disorders of importance include preeclampsia, Graves disease and multiple sclerosis. Intracranial disorders with ocular effects in pregnancy include Pseudotumor cerebri, prolactinomas and

Hypothyroidism can cause a variety of signs and symptoms of the neuromuscular system. However,
ptosis
in a patient with hypothyroidism is very rare. We report here on a case of central hypothyroidism that was due to
Sheehan's syndrome
and it manifested as bilateral
ptosis
in a 51-yr-old woman. She complained of exertional dyspnea and weakness. About 25-yr ago, she had a history of severe postpartum vaginal bleeding. The laboratory studies demonstrated hypopituitarism with secondary hypothyroidism. The
ptosis
was improved by replacement of thyroid hormone. Hypothyroidism should be considered in the differential diagnosis of patients who manifest with
ptosis
and that prompt replacement of hormone can lead to a complete recovery.
