The authors propose a new technique for the surgical treatment of uterine
prolapse
. The technique consists of placing the myometrium between the bladder and the
vagina
after amputation of the cervix uteri and total excision of the endometrium. The bladder is then supported by a strong and well vascularized muscular tissue. Anterior transposition of the cervical pedicles and fixation of the myometrium to the anterior
vagina
ensure that the fitting is solid and in the correct direction. These modifications of the old technique of vesico-vaginal uterine interposition suppress the drawbacks which have gradually led to the rejection of the Wertheim-Schauta operation.

Listeria monocytogenes can cause sepsis and meningitis during the neonatal period. Six cases of early onset neonatal sepsis caused by Listeria monocytogenes are reported here. These cases were diagnosed in a private hospital at Santiago, Chile from December 1984 throughout November 1986. The incidence rate was 1.4 x 1,000 liveborns. Clinical findings included prematurity (6), meconium stained amniotic fluid (6), hepatomegaly (6), splenomegaly (6), maculopapular exanthem (4), anal

(3) and meningitis (1). Additionally 5 patients developed respiratory distress and 4 required ventilatory support. Overall mortality was 50% (3/6). All deaths were related to respiratory failure and occurred during the first week of disease. All patients received ampicillin and amikacin early in the course of their infection. Listeriosis of the newborn infant might be preventable by prompt recognition and treatment of maternal infections. Since Listeria infection in pregnancy is usually mild and symptoms and signs are nonspecific, prevention may be difficult. Pregnant women with fever of no clear origin or with an influenza like syndrome should be screened for listeriosis with cultures from blood,
vagina
and cervix samples.

A retrospective analysis of 59 patients who underwent colposacropexy (CSP) using Prolene (polypropylene) mesh is presented. Fifty-eight of the patients had undergone previous surgical treatment, including either vaginal or abdominal hysterectomy. Twenty-two patients underwent CSP alone, 24 had CSP and retropubic urethropexy (RPU), eight had CSP with anterior or posterior repair, or both, and five had CSP and RPU with anterior and posterior repair. The operations were associated with a minimum of intraoperative complications and acceptable postoperative problems. A postoperative questionnaire was sent to the patients with an 89 per cent response rate. None of the patients complained of protrusion from the
vagina
. It is concluded that, in the hands of experienced surgeons, CSP is a safe, efficacious operative procedure that should remain the procedure of choice for vaginal vault

since it restores the normal vaginal axis, maintains existing vaginal length and provides permanent care.

For prophylaxis of enterocele and of
prolapse
of the
vagina
following hysterectomy, the vaginal stump is fixed in at-risk patients to the sacro-uterine ligaments (known as McCall's suture) or to the sacro-spinal ligament (Amreich-Richter method). We report on the indications and results obtained in 101 sacro-spinal fixations and 211 McCall sutures in vaginal hysterectomy and 118 McCall sutures in abdominal hysterectomy. From 1975 to 1981 sacro-spinal fixation was only occasionally employed in prophylaxis of enterocele. After introduction of the McCall suture in 1982, the use of this method has been steadily increasing and has largely replaced sacrospinal fixation for prophylactic purposes. Nevertheless we are still using this often in cases of total
prolapse
, since in that situation, the fixation of the vaginal stump to the sacrouterine ligaments (in most cases weakly developed) is insufficient and does not offer enough support. Of a total of 350 McCall sutures performed to date, postrenal anuria occurred twice after kinking of the ureters, a typical complication that requires removal of the McCall suture. In a total of 174 sacro-spinal fixations of the vaginal stump for prophylactic or therapeutic indications, pronounced intraoperative haemorrhage took place in about 5% of the cases, whereas in one case, there was an abscess formation due to an infected haematoma. Technical details on both methods and on avoiding complications are discussed.

There was no death recorded, while the morbidity was mainly hypovolaemic shock with haemorrhagic anaemia.

Under field conditions a 97.8% success rate was achieved for caesarean operations performed on ewes with live or freshly dead lambs present in utero. Where lambs were in moderate to advanced stages of autolysis and emphysema the survival rate of the dam was 57.1%. Neither the presence of a vaginal

nor abortion appeared to have an influence on the survival rate of the ewe. Interference and trauma to the posterior reproductive tract prior to veterinary examination reduced the success rate of cases. Ewes with a history during gestation of a prolapsed

were 10 times more likely to require a caesarean operation to correct the dystocia than normal ewes. A range of haematological variables failed to give an accurate prognostic guide to the outcome of surgery. A caesarean operation is recommended in preference to euthanasia of a ewe that could have been considered a poor-risk surgical case. Fetotomy was not possible in cases where emphysematous lambs were present in utero.
