51 (30 men and 21 women) of 90 consecutive patients with acute non-granulomatous anterior uveitis were HL-A 27 positive. This frequency of 55-7 per cent compares with 8-2 per cent in controls. Twenty-three patients (18 men and 5 women) had in addition evidence of systemic disease, including ankylosing spondylitis, sacroiliitis and Reiter's syndrome, sometimes associated with psoriasis. Twenty-eight of 63 patients without evidence of systemic disease were HL-A 27 positive, suggesting that the
uveitis
in many of these cases has a similar aetiology to those with rheumatic disease. The
uveitis
associated with HL-A 27 is typically unilateral, associated with mechanical
ptosis
, and a painful diffusely red, photophobic, and lacrimating eye, generally lasting 3 weeks or more. Protein extravasation into the aqueous is considerable, cells are usually present in the aqueous and anterior vitreous, and keratic precipitates are never mutton fat in appearance. Recurrent episodes are characteristic. The association with HL-A 27 suggests that many if not most cases of non-granulomatous anterior uveitis have a close aetiological relationship to ankylosing spondylitis and Reiter's syndrome and it is likely that infective agents, leading to an unusual immunologically mediated inflammatory response in predisposed individuals, are involved. Ten patients with granulomatous anterior uveitis were HL-A 27 negative.

In a group of 2,281 eyes after extraction of a cataract and implantation of an iris clip lens (1,362 eyes after intracapsular extraction and 927 eyes after extracapsular extraction) motion of the retina occurred in 36 eyes (1.67%), incl. 27 after intracapsular extractions (1.98%) and 9 after extracapsular extractions (0.97%). In patients with pseudophakic retinal detachment after implantation of an iris clip lens the frequency of factors which might interfere with the development of this complication was investigated. The latter proved to be the following:

of the vitreous body, luxation of the implanted lens, posterior
uveitis
and myopia. Detachment of the retina was preceded by: peroperative
prolapse
of the vitreous body in 34.21%, luxation of the implanted lens in 28.95%, posterior
uveitis
in 36.85% and myopia (up to -4.0 Ds) in 21.05%. Anatomical and functional results after operation of pseudophakic motion were evaluated in 38 eyes (36 eyes operated on account of cataract in the authors' department, two in other departments). The minimal follow up period after operation of retinal detachment was six months. Anatomical restoration of the retina was achieved in 23 eyes (60.53%), in 9 eyes a residual motion remained (23.68%) and in four eyes total motion (10.53%). In two eyes keratopathy developed (5.26%). Central vision 6/6-6/12 was achieved in 18.42%, 6/15-6/36 in 21.05%, 6/60-3/60 in 28.95% and less than 3/60 in 31.58%. In the poorer anatomical and functional results after operation of pseudophakic motions participate in addition to the above mentioned ones difficult visualization of the retinal periphery and the extent of surgical trauma during surgery of detached retinas.

are shown. Cytology from severely damaged eyes is highly abnormal, improving as the ocular disease settles. In alkali burns a return to normal cytology does not correlate with return of corneal clarity. Loss of goblet cells is a common finding in the ocular diseases demonstrated, and appears to be a non-specific indicator of ocular problems.

Malignant lymphocytic lymphoma rarely involves the eye. In two patients we found ocular signs and symptoms as the initial presentation of systemic disease. In the first patient periorbital swelling and

of one year's duration ultimately proved to be the result of malignant lymphoma. After resolution of swelling and

with chemotherapy, the patient returned with diffuse iris involvement and
uveitis
with a hypopyon. In the second patient an acute change of refractive error proved to be the result of a lymphomatous deposit in the choroid. In both instances common symptoms were the initial manifestations of a diffuse malignant disease.

. Local and systemic corticosteroids and antihypertensive treatment were insufficient. Cryocoagulation of the bleb was performed. The inflammation rapidly subsided and intraocular pressure normalized without further therapy.

Posterior or apical involvement led to an early optic neuropathy, and myositic lesions were characterized by features of muscle infiltration. Management with steroids was effective and could be followed by serial CT studies.

We analyzed the postoperative complications in a prospective study of 3,429 cataract operations: over 95% phacoemulsification and 4% ECCE. In more than half of the procedures, the no-stitch technique with implantation of a standard PMMA IOL was used. Postoperative wound-closure complications were more frequent in the group of corneoscleral incision with double running suture: hypotony (under 5 mmHg) in 5%, wound dehiscence with iris

in 1.9%. In the series with the no-stitch technique, hypotony occurred in 1.8%. Other wound-closure complications were eliminated to a pro mille level.

We report a 62-year-old woman who was admitted to our hospital because of abrupt onset of
ptosis
, and alternating and recurrent bilateral external ophthalmoplegia in a short period without pupillary sphincter muscle abnormality. She had been suffering from
uveitis
of unknown origin for four years before admission, which was improved with the local administration of steroid. Her brain CT and MRI showed a parasellar mass lesion, and cerebral angiography revealed total occlusion of the right internal carotid artery with little arteriosclerotic change in other blood vessels. She was diagnosed as having sarcoidosis because of elevated serum creatinine kinase and lysozyme levels, and the pathological finding of granuloma in muscle biopsy. The oral administration of prednisolone resulted in disappearance of her external ophthalmoplegia completely in ten months. We postulate that in this patient, the alternating and recurrent external ophthalmoplegia in a short period was related to ischemic neuropathy caused by vascular lesion of neurosarcoidosis.

Purpose To describe 11 clinical cases of ulcerative keratitis in horses associated with beta-hemolytic Streptococcus equi in Florida, USA. METHODS: Retrospective clinical study (1996-99). RESULTS: Beta-hemolytic Streptococcus equi was cultured from 11 horses with deep ulcers, descemetoceles or iris

(n = 8), a suture abscess found with a penetrating keratoplasty for a stromal abscess (n = 1), and ulceration that developed following keratectomy/irradiation for corneal squamous cell carcinoma (n = 2). Beta-hemolytic Streptococcus equi subspecies zooepidemicus was found in 10 eyes and subspecies equi in one. Marked signs of
uveitis
including miosis and hypopyon were present in 8/11 (72.7%) eyes. Keratomalacia was severe in all eyes. The mean diameter of the ulcers associated with beta-hemolytic Streptococcus was 10.2 +/- 6.1 mm. Eight of the eyes required conjunctival flap surgery (four grafts dehisced) and one eye corneal transplantation. Two eyes were treated with medication only. Isolate sensitivity to antibiotics included ampicillin (6/11), bacitracin (11/11), cephalothin (11/11), chloramphenicol (11/11), gentamicin (5/11), polymyxin B (2/11), and tobramycin (1/11). All isolates were resistant to neomycin. The average healing time was 44.7 +/- 26.7 days. The visual outcome was positive in 8/11 eyes, and the globe retained in 9/11 eyes. CONCLUSIONS: Although Gram-positive bacteria predominate in the normal conjunctival microflora of horses throughout the world, Gram-negative bacteria and fungi are more often isolated from equine ulcers. Beta-hemolytic Streptococcus spp. are associated with a very aggressive ulcerative keratitis with the capability to digest conjunctival graft tissue. Clinical signs are pronounced. Aggressive surgical and intensive medical therapy with topical antibiotics and protease inhibitors is indicated.
