Tuberculous meningitis (TB meningitis) is a subacute meningitis known for its various form of initial manifestations, which often make early diagnosis difficult. The present case report demonstrates a patient with TB meningitis, who had initial manifestation of isolated right oculomotor nerve palsy. High vigilance is needed in diagnosing TB meningitis. A 75 year-old female was hospitalized due to acute onset of right side ptosis. Thorough neurological examination at admission revealed isolated right oculomotor nerve palsy. Brain magnetic resonance imaging and cerebral angiography showed no specific finding. Lumbar puncture was performed two days later due to low grade fever. Cerebrospinal fluid (CSF) study and the polymerase chain reaction on CSF confirmed the diagnosis of TB meningitis. Because TB meningitis is a chronic disease, cranial nerve palsies are common manifestations. This report suggests that TB meningitis should be a disease of differential diagnosis for isolated oculomotor nerve palsy.

Tuberculous meningitis (TBM) remains the most dangerous form of tuberculosis with high mortality and potential complications. The prompt diagnosis and treatment of this condition remains a key for better prognosis. A 39-year-old woman presented with severe headache, fever, nausea and vomiting, with a history of headache for a month. On examination, confusion, neck rigidity, ptosis and upward plantar reflexes were present. After 7 days of empiric treatment without resolution of her symptoms, she had another spinal tap performed. The diagnosis of TBM was performed by the GeneXpert MTB/RIF assay from her cerebrospinal fluid (CSF). Antitubercular chemotherapy was started. The patient subsequently improved. Where available, the GeneXpert assay should be used immediately in CSF samples of patients suspected of TBM as an adjunct to clinical algorithms to increase the chance of a prompt diagnosis and treatment.

Tuberculous meningitis (TBM) is a sub-acute / chronic meningitis known for its diverse manifestations which may lead to delayed diagnosis. An isolated oculomotor nerve palsy as an initial presentation of TB meningitis is quite rare. One such case has presented here; A 18 year female presented to us with ptosis of the left eye. Complete neurological examination revealed it to be a case of isolated 3rd cranial nerve palsy. Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) brain revealed no significant abnormality. Cerebrospinal fluid (CSF) analysis was done and diagnosis of Tuberculous Meningitis was confirmed. This case report focuses on the fact that tuberculous meningitis should be included in the differential diagnosis of isolated oculomotor nerve palsy.
