A new intervention for neovascular glaucoma is suggested: submuscular decompression multisclerectomy with collagen implant impregnated with 5-fluorouracyl. Immediate and remote (up to 2.5 years) results in 29 patients were analyzed. Intraoperative complications were observed in 24.1% cases: vitreous prolapse in 13.8% and hyphema in 10.3%. Ophthalmic tone compensation was attained in 96% cases in the early postoperative period and in 63% in remote period. Painful syndrome was arrested in 96 and 65% cases, respectively. The hypotensive effect of the operation is due to an increase in the volume of the eyeball and enhancement of uveoscleral discharge.

A 23-year-old Chinese woman presented with necrotizing scleritis 10 months after diode laser cyclophotocoagulation for the treatment of her neovascular glaucoma. There were areas of scleral thinning with uveal prolapse in the superior quadrant. The inflammation and progression of the staphyloma were controlled with systemic steroids. Diode laser cyclophotocoagulation may not be safe in patients suspected to have underlying systemic vasculitic diseases.

Five case histories are presented. Waldenstrom's macroglobulinaemia caused bilateral central retinal vein occlusion, proptosis was the presenting feature of retro-orbital plasmacytoma in relapsed multiple myeloma, a red painful eye was due to neovascular glaucoma in primary polycythaemia, bilateral VIth nerve palsy caused convergent squint and diplopia in meningeal relapse of acute lymphoblastic leukaemia and lymphoma of the eyelid caused complete ptosis. Interdisciplinary management is described. Ophthalmological lesions in haematological disease should be promptly recognized and managed. Collaboration between ophthalmology and haematology departments may be effective for palliative management.
