This case report affords the dynamic understanding of a patient's conversion reaction of unilateral ptosis of the eye. The specificity of this manifestation of psychopathology is shaped by the patient's unconscious ideas about disease and the suitability of this symptom for the symbolic representation of her primary conflict.

We report a case of a 14-year-old girl who presented, following a sudden onset, with bilateral ptosis, gait disturbance, difficulty swallowing and loss of appetite, right hypochondriacal pain, and frontal headache. Protracted neurological and medical examinations were unremarkable; neither was precipitating psychological stresses evident. The condition, which manifest as typical conversion disorder, lasted for one year. "Treatment" involving electrical stimulation of both eyes muscles and legs with positive reassurance resolved the symptom. This case supports the view that conversion disorder, not only involves a strong element of suggestion, but also incorporates socio-cultural sanctioned prescription.

A 13- year-old girl presented with unilateral psychogenic pseudoptosis. She had sudden-onset ptosis on the left side 3 weeks before presentation. Careful review of her medical history revealed multiple assessments for chronic pain symptoms over the previous 2 years. Physical examination revealed isolated ptosis on the left side, with no other abnormal neurologic findings. On sustained upward gaze, mild elevation of the upper eyelid accompanied by slight elevation of the lower eye lid was seen. Imaging of the brain, laboratory investigations and Electrophysiologic studies for neurogenic and myogenic pathologies were normal. Dramatic and sustained response with complete resolution of ptosis was observed after administration of placebo during the edrophonium test, thus confirming the diagnosis of psychogenic pseudoptosis. The patient was offered psychologic assessment and support. There was no recurrence of symptoms at follow-up after 2 months. Psychogenic pseudoptosis is an uncommon manifestation of conversion disorder in children. Early recognition of the condition and institution of appropriate psychologic therapeutic interventions are essential for good recovery.

This report describes the rare case of a 27-year-old female patient with conversion disorder who presented unilateral ptosis with ipsilateral muscle spasm of orbicular oculi. The co-existing of ptosis and muscle spasm of orbicular oculi indicates that, in accord with prior reports, the overactivity of orbicular oculi is essential in psychogenic pseudoptosis. The co-existing of unilateral ptosis and ipsilateral muscle spasm of orbicular oculi in the present case leads us to the conclusion that the overactivity of orbicular oculi is essential in psychogenic pseudoptosis.

Functional vision disorder (FVD) is a common problem seen in many neurologic and ophthalmologic practitioners' offices and may occur in isolation or in the presence of medical illness. This disorder presents with visual or oculomotor symptoms and manifests as vision loss in one or both eyes, visual field loss, double vision, oscillopsia, anisocoria, blepharospasm, or ptosis. Manual perimetry is the most effective method for determining functional visual loss, and the presence of a central scotoma in a functional visual field signifies that a neuropathophysiologic process is almost certainly present. The exact neuropathophysiologic mechanism of this disorder is unknown; however, information can be drawn from the small studies of FVD samples and studies examining neuropsychiatric factors in other conversion disorder semiologies. Psychological and psychiatric interventions can be useful in treating these patients.
