A prospective, randomised study was performed on 52 patients undergoing routine limbal extracapsular cataract surgery to compare the effect of 10/0 nylon and 9/0 elastic polypropylene sutures on post-operative
astigmatism
. Both the magnitude and the axis of the
astigmatism
were measured by keratometry and refraction pre-operatively and at regular intervals for 3 months post-operatively. Both groups showed an increase in the magnitude of
astigmatism
immediately post-operatively. The polypropylene group showed a more rapid decrease in
astigmatism
over the first 7 days and subsequent stabilisation, although the only significant (p < 0.05) difference in
astigmatism
between the two groups was at 1 week post-operatively. Both groups showed 'with-the-rule'
astigmatism
immediately post-operatively, although more of the polypropylene group developed 'against-the-rule'
astigmatism
by the end of the study. Despite an early reduction in post-operative
astigmatism
in the polypropylene group, early spectacle prescribing would not be recommended because of the shift in the axis of
astigmatism
. In addition, the polypropylene group had a 4 times greater incidence of iris
prolapse
compared with the nylon group.

The results of a retrospective analysis of 49 eyes (40 patients) that underwent penetrating keratoplasty for keratoconus are presented. All grafts had the same size trephine for both host and donor corneas. Ninety-eight per cent of eyes achieved a post-operative visual acuity of 6/12 or better; 43% of eyes attained this level of acuity unaided, although more than half of these required spectacles or contact lenses to reach their best corrected acuity. The average post-operative spherical ametropia was -0.5 dioptre (SD 2.97 dioptres) and the average post-operative cylinder was -3.8 dioptres (SD 2.63 dioptres). Three eyes (6%) required keratorefractive surgery to reduce
astigmatism
. Wound integrity was satisfactory using the same (size) trephine for both host and donor, with no patient suffering a post-operative wound leak or iris

We report a Mexican family with two members (mother and daughter) affected with blepharophimosis hereditary syndrome:
ptosis
of the eyelids, blepharospasm and epicanthus inversus. The daughter presented: hyperopia,

, hemispheric asymmetry, superficial mental deficiency and ovarian cyst. The mother with ophthalmological affectation, present too border line intellectual quotient. Clinical findings in this family illustrate autosomal dominant trait; differential diagnosis should be taken into account for genetic counselling.

A 2-month-old female infant underwent intralesional corticosteroid injection for a capillary hemangioma that was causing amblyopia of the right eye from
ptosis
, globe displacement, and
astigmatism
. Forty-eight hours after the injection, the infant's parents noted that she was visually inattentive. On examination, she could not fixate or follow with either eye, and an afferent pupillary defect was present in the left eye. Ophthalmoscopy showed scattered areas of intraretinal hemorrhage in the right eye and extensive preretinal and intraretinal hemorrhages in the left eye. Ten months after injection, the hemangioma had greatly regressed, and visual acuity in the right eye was felt to be normal. The left eye had unsteady fixation with a persistent afferent pupillary defect and macular scarring. Retrograde flow of the corticosteroid suspension through the hemangioma's feeder vessels, probably originating from both ophthalmic arteries, is the most likely explanation for this complication.

Since January 1992 planned extracapsular cataract extraction (ECCE) is performed routinely with the no-stitch technique at our clinic. To minimize surgically induced
astigmatism
further, modified wound constructions for planned ECCE with on 1.1-mm tunnel width were evaluated. The follow-up time was up to 3 years postoperatively. For 250 eyes wound closure was performed prospectively either sutureless (n = 70), with a single perpendicular suture (n = 100) or cross sutures (n = 40) at the 12 o'clock position or sutureless in the temporal position (n = 40). The complication rate was 4% (filtering bleb, iris
prolapse
or transient hypotonia). There were no wound ruptures, but once endophthalmitis was observed. Late mean
astigmatism
after up to 3 years follow-up for vertical incision was 2.05 +/- 1.16 D (1.01 +/- 0.96 D preoperatively) for sutureless wound closure, 1.63 +/- 1.08 D (0.86 +/- 0.95 D) for perpendicular and 1.76 +/- 0.88 D (0.73 +/- 0.55 D) for cross-sutures. A computerized topographic analysis system (EyeSys [EyeSys Technologies, Houston, TX, U.S.A.]) was used to evaluate corneal astigmatic changes (central corneal power and astigmatic axis) after blepharoptosis surgery on 29 eyelids (22 patients). Fifteen nonoperated eyelids of study patients undergoing unilateral
ptosis
repair were used as controls. All patients were evaluated preoperatively and again 6 weeks after surgery. Fifteen operated eyelids and five controls were also examined 12 months after surgery to evaluate the stability of these measurements. In 72.4% of operated eyes, corneal topography demonstrated increased with-the-rule
astigmatism
6 weeks after surgery, 13.8% of surgical eyes showed increasing against-the-rule
astigmatism
, and in 13.8%, no change was noted from preoperative measurements. None of the 15 control eyes demonstrated any change in corneal astigmatic axis or significant change in central corneal power. By 12 months after surgery, all of the operated eyes showed a regression toward the amount and pattern of preoperative

. Only two of 15 eyes maintained a difference of > 0.3 D of central corneal power greater than their preoperative measurement at 1 year. These data suggest that corneal topography may be altered by changes in eyelid position, although the changes appear to be temporary in most patients.

and the effects of lid position on ocular development. Eyelid

was surgically induced in one eye each of white leghorn chickens within 48 hours after hatching. The chicks were raised under natural diurnal lighting. Thirty days after inducing

, the chicks were killed and their eyes enucleated and photographed in a sagittal view. Computerized image analysis was used to measure the size of the globe along five axes. Globe size was significantly greater in the superior part of eyes with

Thirty-two children who had been refracted in early infancy were re-refracted at the age of 7 to 8 years. Refraction comprised cycloplegic retinoscopy at the mean age of 11 weeks and noncycloplegic retinoscopy and subjective examination at the mean age of 94 months. The change in spherical equivalent refraction (SER) was highly negatively correlated with the initial refraction (-0.863), demonstrating emmetropization; however, the correlation between the refractive error at age 7 to 8 years and the refractive error in infancy was much weaker (+0.225). The SER was significantly less and the
astigmatism
was greater in infancy in children who were myopic at age 7 to 8 years. There was, however, extensive overlap in range between the SER for the two groups and the initial SER was not a good predictor of myopia at 7 to 8 years, although it may help to identify children who are unlikely to become myopic. Peripheral facial paresis is often accompanied by incomplete closure of the eyelids and may lead to varying degrees of keratopathy. Conservative therapeutic measures are often not sufficient. To achieve better lid closure tarsorraphy has been the primary method of treatment but has certain functional and cosmetic drawbacks for the patient. Alternatively gold weight implants have been used to close the upper lid by the force of gravity and if needed can be combined with further reconstructive facial surgery. From May 1994 to January 1997 29 patients with peripheral facial paralysis were treated with gold weight upper lid implants. Postoperative closure of the lids was sufficient in all cases, and there was a statistically significant decrease in lagophthalmos and improvement in keratopathy. Complications observed included
ptosis
(n = 5), cosmetically unacceptable bulging of the gold implant (n = 5), extrusion of the implant (n = 1) and the development of a low-grade corneal
astigmatism
(n = 7). In all cases of
astigmatism
correction was achieved by the fitting of cylinder glasses. In all, functional results achieved showed that the gold implant was superior to the cosmetically bothersome tarsorraphy.
