The effects of two
calcium channel
blockers (verapamil and flunarizine) were evaluated on the naloxone-precipitated syndrome in morphine-dependent rats. The withdrawal signs in saline-treated rats were mainly diarrhea, body weight loss, jumping and
ptosis
. On i.p. administration, verapamil and flunarizine prevented diarrhea and body weight loss but not jumping. Verapamil also reduced the incidence of
ptosis
at the highest dose tested (40 mg/kg). Administered i.c.v., 160 micrograms verapamil reduced the body weight loss and the number of jumps without modifying diarrhea or
ptosis
. The results show that
calcium channel
blockers inhibit morphine abstinence syndrome manifestations through both peripheral and central mechanisms.

Ophthalmoplegic migraine is a rare condition, previously thought to represent a variant of migraine. Recent observations regarding its usual clinical presentation and common magnetic resonance imaging findings have given rise to speculation that this illness is more likely to represent an inflammatory cranial neuropathy. The recent revision of the International Headache Classification has reclassified ophthalmoplegic migraine from a subtype of migraine to the category of neuralgia. In this article, potential pathophysiological mechanisms are discussed. The typical clinical presentation of ophthalmoplegic migraine generally involves transient migraine-like headache accompanied by often long-lasting oculomotor, abducens or, rarely, trochlear neuropathy with diplopia and (if oculomotor nerve is involved) pupillary abnormalities and
ptosis
. Ophthalmoplegic migraine generally occurs in children, but a number of adult cases have been reported. Prognosis is good because symptoms almost always resolve, but, after several episodes, some deficits may persist. Differential diagnosis is rather large, although most other possible causes of ophthalmoplegia and headache have distinctive presentations or can be excluded with fairly straightforward diagnostic testing. Optimal prophylactic and acute treatment is still unclear, but migraine prophylactic medications such as b blockers and
calcium channel
blockers have been proposed. Steroids have been used with mixed results.

Numerous studies indicate that opioid tolerance involves a disruption in Ca2+ homeostasis. In vivo studies have indicated the involvement of dihydropyridine-sensitive (L-type) voltage-gated channels in morphine abuse. In this study, the effect of multiple administration of the dihydropyridine
calcium channel
blocker nifedipine (5 mg/kg/twice daily), given in combination with morphine, on the signs of morphine withdrawal and some biochemical parameters were assessed. Multiple morphine administration in increasing doses (from 5 to 40 mg/kg for 7 days) and consequent withdrawal after 18 h, induced writhing, squealing, diarrhea, teeth chattering, eyelid
ptosis
and wet-dog type shaking. Coadministration of nifedipine prevented the squealing, diarrhea and teeth chattering. On a biochemical level, the activity of brain nitric oxide synthase (NOS) and the quantity of cytochrome P450 in rat brain and liver were measured. Nifedipine treatment decreased the brain nNOS activity, induced by multiple administration of morphine. The quantity of liver cytochrome P450, after multiple coadministration of morphine and nifedipine, was also increased. The quantity of brain cytochrome P450 was not significantly changed by morphine and nifedipine alone or in combination. The results of our study suggest that nifedipine influences the effects of morphine both at a pharmacokinetic and a pharmacodynamic level.
