From 1977 to 1981 39 patients with advanced ovarian carcinoma (stage III, IV, and relapses) were treated at the department of oncology. From 1977 to 1979 we followed to a great extent the ECOG protocol (EST 2875) which means combination chemotherapy by alternating cycles. Since 1980 we used for treatment the PAC I regimen. There was no obvious difference between the therapeutic results of regimen 1 (MTX/Thio followed by 5FU/CTX) and regimen 2 (MTX/Thio followed by VCR/ADM/5FU/CTX). The greatest complete remission rate (confirmed by a second-look laparotomy) was obtained by regimen 3 (PAC I). In 8 cases PAC I was used as a second line treatment. Only 3 partial remissions were seen.

A 44-year-old man was admitted to our hospital because of brain metastasis and intrapulmonary exacerbation of lung adenocarcinoma. Systemic chemotherapy (PAC + CBDCA) was administered, but neurological symptoms (muscle weakness of limbs and disorientation) appeared. Lumbar puncture and enhanced MRI of lumber vertebrae revealed meningeal carcinomatosis. MTX 20 mg/week (+ Ara-C 40 mg/week) was injected into the meningeal space. There was a clear improvement in the neurological symptoms, but it did not last long. Meningeal injection was performed 7 times. Whole-brain and whole-marrow radiation along with systemic chemotherapy gave a marked improvement in the symptoms and in the metastatic shadow on the chest CT scan.

Targeting the pathogenic pathway of chronic inflammation represents an unmet challenge for controlling disease activity, preventing functional disability, and maintaining an adequate quality of life in patients with rheumatic diseases. Abatacept, a novel molecule that inhibits co-stimulation signal, induces an inhibitory effect on the T-cells. This will further interfere with the activity of several cell lines, leading to the normalization of the immune response. In the latest years, abatacept has been extensively investigated in studies of rheumatoid arthritis for which it was recently approved as a second line biologic treatment in Romania. This review presents the clinical efficacy of abatacept in several rheumatic diseases and highlights the safety profile of this biological agent.
