Several items characterize the papular acrodermatitis: 1. the clinical picture and the localisation 2. inguinal and axillary lymphadenitis 3. acute usually anicteric hepatitis 4. presence of hepatitis B antigen in all patients. Other members of the family are carriers of HB-Ag, too. One of them usually had icteric hepatitis weeks before or after PAC. At present we consider PAC as the clinical disease due to the primary infection of HB-Ag in childhood. The cycle of HB Ag is shown in a scheme. It suggests why the disease is rare, asymptomatic infections occurring in the majority of the cases. It appears in children without HB-antibodies and a particular reactive condition. After parenteral inoculation PAC and lymphadenitis do not develop. Some chronic HB Ag carriers may have become so after PAC in the childhood.

Left ventricular angiography frequently produces one or more premature atrial or ventricular contractions which may make quantitative volume analysis unreliable. Three hundred left ventricular power injections were analyzed and found to produce premature ventricular contractions (PVC's) in 28 per cent of injections and PAC's in 6 per cent. Forty-four per cent of injections starting near the end of the T wave were associated with PVC's, whereas those starting near the end of diastole during the P wave produced PVC's in only 11 per cent. Injections through a Gensini catheter were associated with PVC's in 24 per cent compared to 37 per cent when a pigtail catheter was used (p less than 0.05). The introduction of an 0.6 second delay in the development of peak injection pressures and ECG triggering to start the injection in the last half of the cardiac cycle did not alter the incidence of PVC's. The use of timed diastolic injections with pressure rate-rise delay is of little value in reducing injection induced arrhythmias.

The present investigation was carried out on 17 male patients, 2 of whom nephrectomized, affected by terminal renal failure on regular 4 h dialysis three times a week. Plasma prolactin (PRL), aldosterone (PA) and renin activity (PRA) were determined by radioimmunoassay before, and after the 1st, 2nd and 3rd h of hemodialysis. High levels of PRL were found in 9 nonnephrectomized patients; during the 1st and 2nd h of dialysis means values of PRL showed a slight tendency to increase, whilst during the 3rd h values decreased to predialysis concentrations. In one case a fall in PRL levels immediately after nephrectomy was observed but in another patient who came to our attention after surgery, PRL was significantly increased with values higher than those usually found in nonnephrectomized patients. PRA was elevated in 10 patients and showed no significant modifications during hemodialysis. PAC was high in 8 patients and decreased significantly during hemodialysis. A drop in sodium and potassium levels and a decrease in body weight during dialysis were also observed. These results suggest that in uremia the high levels of PRL are not consequent to hydroelectrolyte disorders or to a condition of hyperaldosteronism, since these parameters present a different pattern during hemodialysis.

A wild strain of Staphylococcus aureus which inactivates a wide variety of antibiotics has been found to inactivate pristinamycin IIA, an antistaphylococcal antibiotic. This phenomenon has been demonstrated to be plasmid mediated. The plasmid directs the biosynthesis of an acetyltransferase which is able to O-acetylate the drug. We propose to call the new enzyme PAC (IIA): Pristinamycin acetyltransferase.

Platelet activating factor (PAF) derived from antigen-stimulated, IgE-sensitized rabbit basophils acts on platelets to induce aggregation and secretion of their content of granule-bound vasoactive amines. Despite this, PAF did not activate platelet factor 3. In contrast, collagen induced aggregation, secretion and PF3 activation in the washed platelets. Other stimuli (ADP, C3b, thrombin) also initiated both secretion and PF3 activation. A wide dose range of PAC, including those giving maximal secretion and aggregation, were ineffictive in making PF3 available and the possibility that PAF inhibited PF3, or its generation, was also excluded. It is concluded that PAF is a unique stimulus for platelets and that secretion and aggregation are not necessarily accompanied by PF3 generation.

Previous studies have demonstrated a renal Na+ conservation during repeated days of exercise in the heat. The present study was intended to describe the role of plasma aldosterone (PAC) in reducing urine Na+ losses during and after 60 min of exercise (60% VO2max) in a warm environment (30 degrees C, 50-53% relative humidity). Additional measurements were made of plasma renin activity (PRA) in an effort to demonstrate the relationship between PRA and PAC. This study shows that a single bout of exercise can significantly reduce urine Na+, Cl-, and H20 excretions for up to 48 hours. Both PRA and PAC were significantly elevated during and immediately after exercise and returned to the pre-exercise level within 6-12 hours of recovery. Subsequently, ingestion of 180 mEq of Na+ each day with ad libitum water intake results in an increased NaCl storage and an expansion of the extracellular volume.
