A 70-year-old man admitted to a local hospital because of facial muscle weakness, tinnitus and facial pain in left side, was then given corticosteroid with a tentative diagnosis of Bell's palsy and his symptoms gradually improved. Since these symptoms recurred six months later, he was referred to our neurological service. As his brain CT revealed diffuse thickening and enhancement of the dura mater, he was thought to have hypertrophic pachymeningitis (HP). Intravenous antibiotics were started for
aspiration pneumonia
and his neurological symptoms gradually improved. HP caused by bacterial infection was thus likely and antibiotics, rifampicin and metronidazole, were administered orally as an outpatient. However, one month later, these symptoms were worsened with headache and double vision. He was then rehospitalized. MR imagings of the head with gadolinium disclosed diffuse meningeal thickening and enhancement, especially of the left-sided cerebellar tentorium. Erythrocyte sedimentation rate and C-reactive protein were moderately elevated. Serum
angiotensin converting enzyme
was within normal range. The test for cytoplasmic antineutrohil cytoplasmic antibody (ANCA) in the serum was negative, however, that for perinuclear ANCA was positive at a titer of 99 EU. Lumbar puncture showed normal findings and negative culture results for bacteria, fungi or mycobacteria. Dural biopsy specimens showed non-specific granulomatous inflammation of the dura with epithelioid histiocytes and Langerhans type multinuclear giant cells with caseous necrosis, however, with no presence of fungi or tubercle bacilli. After the oral administration of cyclophosphamide (100 mg, daily) and prednisolone (40 mg, daily), his neurological symptoms and laboratory findings have been gradually improved and he is well one year after discharge. This case together with previous reports suggests that ANCA positive HP without evidence of other organ involvements may belong to the limited form Wegener's granulomatosis. In the literatures of idiopathic HP, the treatment effect with corticosteroid alone is initially favorable, but transient. On the other hand, using the combined therapy of cyclophosphamide and prednisolone, the remission has been achieved in more than 90% of patients with WG. These data suggest that P-ANCA positive HP should be treated with a combination of corticosteroid and cyclophosphamide.

The double contrast pharyngogram by use of computed radiography (DCP-CR) has been found to be useful in detection of asymptomatic swallowing dysfunction. Following the
DCP
-CR examination, we investigated the incidence of
aspiration pneumonia
in 143 patients with chronic cerebrovascular disease (CVD) for 3 years and the effects of
ACE
inhibitors on the prevention of pneumonia.
Aspiration pneumonia
occurred in 29 out of 143 patients, and more frequently in the elderly chronic CVD patients with multiple brain lesions.
Aspiration pneumonia
was confirmed in 26 out of 85 patients (30.6%) with abnormal barium adhesion to the pharyngeal wall on the double contrast pharyngogram image by
DCP
-CR; whereas pneumonia occurred in 3 out of 58 patients (5.2%) with normal findings of
DCP
-CR pharyngogram. Among chronic CVD patients with abnormal findings of
DCP
-CR pharyngogram, the incidence of
aspiration pneumonia
was significantly lower in the patients treated with
ACE
inhibitors than in those treated with other antihypertensive agents or without antihypertensive agents (chi 2 value = 7.163, p < 0.05). Accordingly,
ACE
inhibitors may prevent the
aspiration pneumonia
and reduce the incidence of
aspiration pneumonia
in the chronic CVD patients with abnormal
DCP
-CR pharyngogram images.

Pneumonia is a common cause of death in elderly people. A series of our studies have demonstrated that pneumonia in the elderly is characterized by silent aspiration, impaired swallowing and cough reflex, partly due to cerebral infarctions at basal ganglia. These infarctions probably induce the disruption of the specific central neurotransmitter system including dopamine and substance P, which plays an important role for swallowing and cough reflex. Use of
ACE
inhibitor and stimulation of the oral cavity by simple oral care, which are effective in increasing substance P. reduced the incidence of
aspiration pneumonia
. Moreover, use of a dopamine agonist such as amantadine hydrochloride and a folic acid supplement that are known to potentiate dopaminergic neurons also prevented
aspiration pneumonia
. For patients bedridden due to lowered ADL, it is essential for them to keep an upright position a few hours after meals to prevent
aspiration pneumonia
caused by the reflux of ingested foods. Also, administration of neuroleptics may cause
aspiration pneumonia
by suppression of dopaminergic neurons.
