Recurrent aspiration pneumonia is a potentially lethal problem, and its treatment is controversial. A variety of procedures have been advocated to prevent aspiration. These are reviewed briefly. We have been dissatisfied with established procedures because they usually require external approaches with considerable complexity and potential complications. Therefore, we attempted to obstruct the glottic airway by injecting both vocal cords with Teflon in a series of patients with recurrent aspiration pneumonia secondary to severe neurologic impairments. Since Teflon injection of the vocal cords bilaterally did not reliably prevent aspiration, we cannot recommend it for routine use in the treatment of chronic aspiration.

To determine whether gastrostomy and fundoplication prevent aspiration pneumonia in mentally retarded individuals, 105 cases of feeding gastrostomy were examined; 21 subjects had preoperatively diagnosed gastroesophageal reflux. Thirty-nine patients had gastrostomy and fundoplication, 66 patients had gastrostomy alone, and 60 subjects had had aspiration pneumonia before surgery. Recurrent aspiration pneumonia was least in those without preoperative gastroesophageal reflux and who had gastrostomy and fundoplication. Feeding gastrostomy does not prevent aspiration pneumonia. Gastrostomy with fundoplication reduces the likelihood of aspiration pneumonia but still allows a significant number to develop it.

Recurrent aspiration pneumonia can impede the continuation of enteral nutrition. Semi-solid feeds have been demonstrated to reduce the incidence of aspiration pneumonia but are difficult to administer via the nasogastric tube. A novel semi-solidifying enteral formula may be used instead to avoid the occurrence of severe gastroesophageal reflux.
