We report a 63 year old white man admitted to our hospital because of fever, productive cough and a severe respiratory failure. Bronchoalveolar lavage revealed a positive microscopy for acid-fast bacilli and a four drug scheme for tuberculosis was initiated. There was no improvement in the patient's condition and mechanical ventilation was needed. A thorax CT scan disclosed images that suggested lipoid
pneumonia
(lower lobes involvement, hypodensity and negative sign in the angiogram). At this point the patient's relatives indicated that he was a long term user of paraffin oil for
chronic constipation
. Treatment with high doses of steroids was initiated and after whole lung lavage, mechanical respiratory assistance could be interrupted. We comment the association of this entity with lung infections produced by atypical mycobacteria.

presumed to be secondary to aspiration has been recorded in a number of ATR-X cases. In this report we review the gastrointestinal phenotype of ATR-X syndrome in 128 cases. We also demonstrate that in two siblings, regurgitation was secondary to gastric pseudo-volvulus, a condition in which the stomach does not have a normal system of peritoneal ligaments and changes position with possible torsion around itself. Furthermore, ultra-short Hirschsprung disease with colonic hypoganglionosis was shown and this may contribute to the severe constipation affecting these children.

is a very common problem in the paediatric population, and a particularly frequent issue in the management of neurologically impaired children. The use of mineral oil in the treatment of constipation has been well accepted because of its efficacy and infrequent side effects. The case of a three and a half-year-old girl with spastic quadriplegic cerebral palsy, who was admitted to hospital for investigation of increasing tachypnea and respiratory distress over a two-month period, is presented. due to mineral oil aspiration, which is a recognized severe complication of this medication, and emphasizes the need for a heightened awareness among caregivers about the potential dangers of inappropriate mineral oil use.
