A prospective study of 18 critically ill patients with community-acquired lobar pneumonia was undertaken at Hillbrow Hospital, Johannesburg, in order to document the initial plasma hormonal and substrate profile as part of the stress response to the infection. The results of these studies, carried out before therapy, were compared with the results in a group of healthy fasting adults. Highly significant (P less than or equal to 0.005) increases in the mean plasma levels of adrenaline, noradrenaline, human growth hormone, cortisol, glucose and free fatty acids were noted in the study group, with a lesser increase in the
prolactin
concentration (P less than or equal to 0.01). The levels of dopamine, glucagon, insulin and adrenocorticotrophin did not show any significant change. No significant differences were found in the hormonal profile when comparing survivors with non-survivors. The neuro-endocrine hormonal and metabolic responses in
pneumonia
appear to be similar to those seen in other stress situations and failure of the initial stress response does not appear to contribute to the mortality of critically ill patients with community-acquired lobar pneumonia.

A 35 year old woman suffering from ACTH and
prolactin
(Prl) deficiency is described. Her symptoms of adrenal insufficiency appeared gradually after her first pregnancy in 1970; however, she conceived twice more and delivered healthy babies in 1972 and 1974, which she could not breast feed due to lack of milk. During an episode of
pneumonia
in 1977 she suffered acute adrenal insufficiency, after which she began treatment with hydrocortisone. Her pituitary reserve for TSH, GH, LH and FSH was normal, but her ACTH and Prl levels were undectable and did not respond to acute iv challenges of corticotrophin-releasing factor (CRF) and TRH, respectively. Autoantibodies, including antilactotroph titres, were negative, except for a positive pituitary immunofluorescence to ACTH. There was also no ACTH stimulation to a prolonged infusion of CRF followed by an acute iv bolus. These results, together with the gradual onset of symptoms which worsened after each pregnancy, suggest a possible autoimmune aetiology of her pituitary ACTH and Prl deficiencies.

The neuroleptics, well known as stimulants of
prolactin
release, are supposed to increase breast cancer incidence. To verify this hypothesis, we selected a group of 853 deaths of female inpatients with mental diseases recorded in the Dr. G. Marinescu Hospital between 1929 and 1978. Using proportionally and indirect standardization methods, we did not find any association between the incidence of breast cancer death and neuroleptic therapy, widely used in the above-mentioned hospital after 1959. Several authors reported a low cancer death incidence in mental patients. Between 1925 and 1978, 2168 deaths were recorded in this hospital. Besides death certificates, we also studied 1444 complete autopsy protocols (66.60% of all the deaths). Cancer deaths represented 1.94% of 1231 deaths recorded between 1925 and 1960. Cancer deaths represented 7.04% of 937 deaths in the period of 1961-1978, in comparison with 13.36% of the whole population of Romania (p less than 0.001). Among these 937 deaths, statistically significant lower cancer ratios than in general population were found in ten-year age groups, i.e. between 15 and 74 years in women and between 45 and 74 years in men. No case of leukemia was recorded over 1925-1978. Deaths from

, bronchopneumonia and cardio-vascular diseases are now frequent in major mental disorders. New prospective studies are required to elucidate the problem of cancer incidence in mental patients.

The radioimmunological method was applied in the study of ACTH, STH, and

, and in practically healthy children. A comparative analysis of hormonal parameters depending on the functional condition of the thymus demonstrated a directly proportional correlation between the STH,
prolactin
level, and the thymus hyperplasia; there proved to be a reverse relationship between the ACTH level and thymomegaly. Hypoadrenocorticotropinemia, hypersomatotropinemia, and hyperprolactinemia could be used as diagnostic criteria in thymomegaly.

A 42-year-old house wife presented with worsening headaches over 6 months in the absence of visual symptoms or symptoms suggestive of focal neurology. She was a life-long smoker. Systems review was unremarkable apart from secondary amenorrhoea and galactorrhoea of 6 months duration. Her serum
prolactin
was found to be 620 mU/l (60-400), FT4 12.6 nmol/l (9.8-23.1), TSH 1.38 mU/l (0.35-5.5), oestradiol < 73 pmol/l, LH and FSH of 4.4 and 12.6 mIU/l, respectively. She was on bromocriptine. A presumptive diagnosis of
pneumonia
, based on pyrexia and CXR findings, was made and she was started on IV antibiotics. Two days later she developed meningism and deterioration of conscious level. (Lumbar puncture results: no organisms, 312 neutrophils and 164 lymphocytes). CT scan revealed a 2.5-cm pituitary adenoma, with suprasellar extension. A repeat hormonal profile revealed FSH 1.4, LH < 0.3 mU/l, oestradiol < 73 pmol/l,
prolactin
488 mU/l (60-400), and low random cortisol at 29 nmol/l. T1-weighted MRI revealed a large pituitary mass with evidence of haemorrhage. The patient subsequently underwent a transsphenoidal exploration with resection of the pituitary lesion. Whilst awaiting the histopathology results, CT of chest revealed a 1. 5-cm diameter rounded well defined density in the right lower lobe associated with hilar, pre- and right para-tracheal lymphadenopathy. The histopathology of the pituitary lesion, obtained piecemeal, revealed fragments of fibrous tissue infiltrated by sheets of acidophilic
prolactin
-positive cells, in keeping with a prolactinoma. In addition, other fragments with blood clot included highly atypical epithelial cells with mitotic figures. These were negative for
prolactin
but showed HMFG-and CEA-positivity, excluding them from a pituitary lineage. Transbronchial biopsy revealed moderately differentiated adenocarcinoma, with evidence of lymphatic spread. The overall conclusion was of bronchogenic adenocarcinoma, metastasizing to a prolactinoma and complicated by apoplexy.
