Immune reconstitution inflammatory syndrome
(
IRIS
) in HIV-1-infected patients is associated with an exaggerated inflammatory response against an opportunistic infection during highly active antiretroviral therapy. The only review on
IRIS
associated with Criptococcus neoformans reported 21 episodes including lymphadenitis, necrotizing
pneumonitis
, breast and cutaneous abscess, and cryptococcomas. To our knowledge this is the first report of
IRIS
associated with previous meningeal criptococcal infection which required neurosurgical intervention with placement of a ventriculo-peritoneal shunt to drain a CSF cyst formed by exclusion of the temporal horn of the right lateral ventricle. We demonstrate that this procedure is possible without complications such as cryptococcal dissemination into the peritoneum.

A small proportion of HIV-infected patients rapidly progress to AIDS; indeed, some individuals have been known to progress to AIDS within a year after primary infection.
Pneumonia
caused by Pneumocystis jirovecii (PCP) is the most frequent AIDS-defining illness. However, PCP can also rarely occur during primary HIV infection as a result of the severe immunosuppression that may accompany the early stage of HIV infection.
Immune reconstitution inflammatory syndrome
(
IRIS
) comprises two distinct syndromes: paradoxical
IRIS
and unmasking
IRIS
. Infections with Mycobacterium avium complex during antiretroviral therapy are almost always localized and related to
IRIS
. We describe an unusual case of PCP and Mycobacterium avium complex-
IRIS
that occurred less than 3 months after primary HIV infection.

Parasitic infections such as
Strongyloides stercoralis
and HIV have been reported to coexist, particularly in resource-limited settings such as India. In an immunocompromised host,
S. stercoralis
can progress to strongyloidiasis hyperinfection syndrome (SHS). However, SHS is not common in patients with advanced HIV disease.
Immune reconstitution inflammatory syndrome
(
IRIS
) developing after initiation of antiretroviral therapy (ART) can target multiple pathogens including
S
.
stercoralis
. The authors present here a 46-year-old HIV-infected female who was recently diagnosed with HIV-1 infection, started ART, and developed SHS. Her upper GI endoscopy revealed severe gastroduodenitis, and X-ray chest showed extensive bilateral
pneumonitis
. We could identify
S. stercoralis
in induced sputum and duodenal biopsy. We could also identify gut inflammation to restrict invading parasites. After receiving antihelminthic therapy, she showed improvement, a course of events that fit the diagnosis of unmasking
S. stercoralis
IRIS

Chemotherapy while revolutionizing cancer management by improving survival and quality of life; is also associated with several adverse effects. Lung is the most common organ affected in chemotherapy-related complications, due to either drug toxicity or more commonly due to infections caused by immunosuppression and less commonly due to immune-mediated injury. Radiology, when used in combination with clinical and lab data, can help reach the specific diagnosis or narrow down the differentials. The common radiological patterns of drug toxicity include pulmonary interstitial and airway infiltrates, diffuse alveolar damage, nonspecific interstitial pneumonia, eosinophilic
pneumonia
, cryptogenic organizing
pneumonia
, pulmonary hemorrhage, edema and hypertension. Cancer patients are immunosuppressed due to the underlying malignancy itself or due to therapy and are prone to a gamut of opportunistic infections including viral, bacterial, fungal and mycobacterial pathogens.
Immune reconstitution inflammatory syndrome
(
IRIS
), a well-known complication in HIV, is now being increasingly recognized in non-HIV patients with immunosuppression. Engraftment syndrome is specifically seen following hematopoietic stem cell transplant during neutrophil recovery phase. Pulmonary involvement is frequent, causing a radiological picture of noncardiogenic pulmonary edema. Thus, radiology in combination with clinical background and lab parameters helps in detecting and differentiating various causes of pulmonary complications. This approach can help alter potentially toxic treatment and initiate early treatment depending on the diagnosis.

Immune reconstitution inflammatory syndrome
(
IRIS
) after starting antiretroviral treatment for human immunodeficiency virus (HIV) infection has a wide variety of causes. Delayed diagnosis and treatment of
IRIS
is fatal. We report a case of a 21-year-old man with HIV infection and
Pneumocystis jirovecii
pneumonia
. The patient presented with fever and dyspnea with deterioration of pulmonary infiltrations 5 days after starting antiretroviral treatment. We reached the diagnosis of
IRIS
based on radial endobronchial ultrasound (EBUS)-guided lung biopsy. In conclusion, radial EBUS-guided lung biopsy via bronchoscopy is a valuable and minimally invasive technique for the rapid diagnosis of
IRIS
-associated
Pneumocystis jirovecii
pneumonia
