One hundred sixty-four patients underwent total gastrectomy with a rho-shaped jejunal food pouch (rho loop) and roux-en-Y jejunojejunostomy. The rate of postoperative complications was very low (9.1 per cent), and the most important problem of anastomotic failure was never encountered. Only 4 of 164 patients (2.4 per cent) died within one month of surgery, and the causes of death were
pneumonia
(2 patients), cerebral hemorrhage (1), and
ileus
(1). The function of the newly constructed passage was estimated through an elaborate barium meal examination. The test revealed that the rho-shaped jejunal food pouch has adequate reservoir and mixing functions for ingested foodstuff. No reflux of barium meal into the esophagus was observed during the examination. In a follow-up study of the patients, only a few complained of symptoms of reflux esophagitis or dumping, but none of them needed any treatment.

From 1966 up to 1975 164 patients with a hernia incarcerata were seen. 21,3% of them died, in most cases of delayed diagnosis bowel resection was necessary. The prognosis depends on

(11,8%) and multimorbidity of the aged. The death was mainly related to peritonitis (43%) and
pneumonia
(26%). Only no delay in diagnosis, early surgery, intensive pre- and postoperative care will improve the results.

The acute enteritis in the postneonatal period with or without dyspepsia appeared with an incidence ranging from 18,9% to 27,6% during twenty years period (1956--1975) observations in a general children's autopsy material. Bacteriological and serological analysis may be of success and complete the pathologic-anatomical diagnoses. Infective enteritis often is accompanied by

and/or otitis media. The acute enteritis can always be detected as an important feature in regard to the cause of death. Most children die within less than twenty-four hours from hospital admission. The hemorrhagic, ulcerous and necrotic form of the enterocolitis predominate the perinatal and neonatal period, frequently combined with peritonitis and bowel perforation. Bacteriological examinations should be obtained. This form of the acute enteritis and enterocolitis of the newborns must be taken into consideration of the
ileus
symptoms differential diagnosis.

Analysis of the clinical and autopsy reports of 200 deaths following surgery for colorectal cancer from 1956 to 1974, at the Dept. of Surgery, University of Heidelberg, revealed that
pneumonia
(24.5%) was the most common cause of death followed by peritonitis (22%), pulmonary embolism (15.5%), advanced tumor disease (14%), cardiac failure (9.5%),
ileus
(5.5%), and others (9%). The explanation for the postoperative mortality rate of 12% (cancer of colon) and 13.2% (cancer of rectum) lies in the fact that 82.5% of those who died postoperatively were beyond the age of 60, and 40.5% beyond 70 years at the time of surgery. Moreover, in 50.5% advanced tumors with regional and/or distant metastases were found. In 55.5% severe preoperative complications (
ileus
: 38%, peritonitis: 11%, abscess formation or hemorrhage: 6.5%) required an emergency operation. Only 38.5% of the procedures were considered for cure. Besides the need for early recognition of the cancer, intensification of pre- and postoper treatment appears to be the predominant task in the effort to decrease postoperative mortality.

The diagnosis and successful control of systemic Aspergillus niger infection in 2 adult patients with acute leukemia is reported. During induction therapy, the first patient developed pulmonary infiltrates, skin lesions and abnormal liver function tests. Aspergillus niger was found on skin and liver biopsy. This patient was successfully treated with Amphotericin B and granulocyte transfusions and he remains in remission. The second patient developed a
pneumonitis
and adynamic
ileus
with positive sputum and stool cultures for Aspergillus niger. The infection only responded to Amphotericin B and granulocyte transfusions and the leukemia to cytoreductive chemotherapy. The patient later relapsed and died after a febrile illness. Fungi morpholocially consistent with Aspergillus were found in the liver at autopsy. Infection with A. niger is rare even in this patient population; however fungal infections have become an increasing problem. The need for a high index of suspicion, especially when an infection is unresponsive to antibacterial antibiotics, the various diagnostic tools, and the need for aggressive therapy are stressed. Amphotericin B is the chemotherapy of choice but may be insufficient in a severely neutropenic host where the simultaneous use of granulocyte transfusions might be lifesaving.

It is sometimes very difficult to diagnose dissecting aortic aneurysms (DAA), particularly in its early stage, due to manifold signs and symptoms. The purpose of this study is to clarify the reasons for such erroneous diagnoses. A total of 41 patients with DAA were referred to our hospitals for further examination and/or surgery from April 1986 to August 1989. In 18 of these patients, the diagnostic possibility of an underlying DAA was overlooked by the referring physicians. Among these 18 patients, 2 were mistakenly diagnosed as uncomplicated myocardial infarction (MI), one as
pneumonia
, 2 as cerebral infarction, 6 as acute abdominal disease, one as cholelithiasis, 5 as thrombosis of the lower extremities, and one as malignant metastasis to the pericardium. The following is the detail: In 2 cases thought to be uncomplicated MI, an expanding dissecting ascending aorta had crushed the lumen of the left coronary artery, causing MI, in turn, wasting clinical treatment and consuming precious time. In one case, enlargement of the descending aorta on the chest radiography was overlooked and the patient's symptoms were mistakenly attributed to
pneumonia
. In 2 cases in which symptoms of cerebral ischemia were thought to be attributed to cerebral thrombosis, the real cause turned out to be occlusion of the brachiocephalic artery following aortic dissection. Among 6 cases which were initially considered to have only acute abdominal disease, 3 presented with symptoms and signs of

Serum amylase level was determined in 129 cases (225 episodes) of chronic respiratory failure at acute exacerbation and in 59 cases (62 episodes) of
pneumonia
without respiratory failure as control. Cases with accompanying diseases, such as acute pancreatitis, parotiditis,
ileus
and renal dysfunction, which were expected to develop hyperamylasemia were excluded. The 225 episodes were divided according to the causes of acute exacerbation into 4 groups:

, bronchitis, right heart failure without infection, and others (e. g. hemoptysis). Hyperamylasemia (greater than 400 S-U) was observed in groups of
pneumonia
(15/40 = 35.5%) and bronchitis (12/95 = 12.6%), respectively but not in those of right heart failure without infection (0/73 = 0%) and other causes (0/17 = 0%). As a result, hyperamylasemia was found only under conditions of inflammation of lung parenchyma and bronchi with acute exacerbation of respiratory failure. On the other hand no hyperamylasemia was observed in 62 episodes of
pneumonia
alone without respiratory failure. It was concluded that both respiratory tract infection and acute respiratory failure are necessary factors for development of hyperamylasemia originating from lung or bronchi.
